تقشير وتجديد الجلد بالليزر إجراء جلديّ يُوجّه طاقة ضوئية بطول موجيّ يمتصّه ماء الجلد، فيُزيل طبقات دقيقة جدّاً من السطح أو يُسخّن الأدمة دون إزالة السطح، فيتحفّز إنتاج كولاجين جديد ويتحسّن الملمس والتصبّغ خلال أسابيع من الترميم. أي أن النتيجة لا تظهر من الجهاز بل من استجابة الجلد له.

هناك عائلتان: الليزر الاستئصالي الذي يُزيل السطح فيُعطي تغييراً أوضح مقابل نقاهة أطول واحمرار أسابيع، والليزر غير الاستئصالي الذي يعمل تحت السطح فيُعطي تحسّناً تدريجياً بجلسات أكثر ونقاهة أقصر. وكلٌّ منهما يُستخدم بنمط جزئيّ يعالج أعمدة دقيقة ويترك جلداً سليماً بينها ليُسرّع الشفاء. الاختيار بين هذه الخيارات ودرجة شدّتها يُحدَّد بالفحص السريري لنوع بشرتك وسمكها ومشكلتها الأساسية، لا بالاسم التجاري للجهاز.

دواعيه الواقعية: الخطوط الدقيقة حول العين والفم، خشونة الملمس واتّساع المسامّ، آثار حبّ الشباب السطحية والمتوسّطة، التصبّغات الشمسية والكَلَف بحذر شديد، وبعض الندبات. وفي البشرة المتوسّطة والداكنة المنتشرة في الأردن يرتفع خطر فرط التصبّغ التالي للالتهاب، لذلك تُخفَّض الإعدادات وتُطوَّل الفواصل وتُشدَّد الحماية من الشمس، وقد يُفضَّل تحضير الجلد بعلاج موضعي قبل البدء.

حدود الإجراء يجب أن تُقال بصراحة: الليزر يُجدّد سطح الجلد وجودته ولا يشدّ الترهّل الحقيقي ولا يرفع الجلد المتدلّي؛ ذلك عمل جراحي أو خيوط أو أجهزة أخرى. ولا يُزيل الحفر العميقة تماماً، وقد تحتاج علاجات مركّبة. ولا يوقف تقدّم العمر، فالنتيجة تحتاج صيانة. ولا يُستخدم على حبّ شباب نشط ملتهب قبل السيطرة عليه. ولا يمنع عودة التصبّغ إن استمرّ التعرّض للشمس بلا واقٍ، فالحماية الشمسية تحدّد النتيجة أكثر من الجهاز نفسه.

وهناك حالات يؤجَّل فيها أو يُمنع: الحمل والإرضاع، عدوى جلدية نشطة أو هربس فمويّ متكرّر بلا وقاية، استخدام حديث لمشتقات فيتامين A الفموية لعلاج حبّ الشباب، ميل لتكوين الجدرة، تسمير حديث أو تعرّض شمسيّ كثيف، اضطرابات مناعية أو تخثّرية، وتوقّعات غير واقعية عن نتيجة جلسة واحدة.
Laser skin resurfacing and rejuvenation is a dermatological procedure that delivers light energy at a wavelength absorbed by water in the skin, either removing extremely thin layers of the surface or heating the dermis without removing the surface. New collagen formation is stimulated, and texture and pigmentation improve over the weeks of remodelling that follow. In other words, the result comes from how the skin responds rather than from the device itself.

There are two families. Ablative lasers remove the surface and give a more obvious change in exchange for a longer recovery with weeks of redness. Non-ablative lasers work beneath an intact surface and give gradual improvement with more sessions and shorter downtime. Both are commonly used in a fractional pattern that treats microscopic columns and leaves untreated skin between them to speed healing. Which option and what intensity suit you is decided by clinical examination of your skin type, thickness, and main concern, not by the brand name of a machine.

Realistic indications include fine lines around the eyes and mouth, rough texture and enlarged pores, superficial to moderate acne scarring, sun-induced pigmentation and melasma treated with great caution, and some scars. In the medium and darker skin tones common in Jordan, the risk of post-inflammatory hyperpigmentation is higher, so settings are reduced, intervals lengthened, and sun protection enforced strictly, and preparing the skin with a topical regimen beforehand is often preferred.

The limits deserve plain language. Laser renews the surface and quality of the skin; it does not tighten true laxity or lift sagging tissue, which is the work of surgery, threads, or other modalities. It does not completely erase deep ice-pick scars, and combined treatments are often needed. It does not stop ageing, so results require maintenance. It is not used on actively inflamed acne before that is controlled. And it does not prevent pigmentation from returning if sun exposure continues without protection, since sun protection influences the outcome more than the device does.

Some situations mean postponing or avoiding treatment: pregnancy and breastfeeding, active skin infection or recurrent cold sores without prophylaxis, recent use of oral vitamin A derivatives for acne, a tendency to keloid scarring, recent tanning or heavy sun exposure, immune or clotting disorders, and unrealistic expectations of what one session can achieve.

علاج الأوعية الدموية بالليزر إجراء يستخدم أطوالاً موجية يمتصّها الهيموغلوبين داخل الأوعية السطحية الصغيرة، فتتحوّل الطاقة إلى حرارة داخل الوعاء نفسه فيتخثّر ويُغلَق ثم يمتصّه الجسم تدريجياً، بينما يبقى الجلد المحيط سليماً نسبياً لأن الطاقة موجّهة إلى اللون الأحمر لا إلى الجلد.

يُستخدم في الشعيرات الدموية الظاهرة على الوجه والأنف، والحمرة الثابتة وتوسّع الأوعية المصاحب للوردية، والوحمة الخمرية عند الأطفال والكبار، والوحمات الدموية السطحية بعد تقييم اختصاصي، وبعض الأوعية العنكبوتية الدقيقة على الساق، والآفات الوعائية الصغيرة كالبقع الياقوتية. أمّا اختيار طول الموجة وحجم البقعة ومدّة النبضة فيُحدَّد بحسب عمق الوعاء وقطره ولون البشرة، ويُحدَّد الترشيح بالفحص السريري لا بالصور المتداولة.

ما يمكن انتظاره: تلاشي تدريجي للأوعية المعالَجة بعد جلسة إلى عدّة جلسات، مع كدمة أو احمرار مؤقّت بينهما. والوحمة الخمرية تحتاج سلسلة جلسات مبكرة قد تفتّح لونها كثيراً لكنها قد لا تُمحى تماماً، والوردية يتحسّن احمرارها الظاهر بشكل ملموس.

وحدود الإجراء صريحة: الليزر يُغلق الأوعية المرئية ولا يشفي الوردية كمرض مزمن، والاحمرار قد يعاود ويحتاج جلسات صيانة وعلاجاً موضعياً وتجنّباً للمُحرّضات مثل الحرارة والشمس والأطعمة الحارّة. ولا يُعالج دوالي الساق الحقيقية ولا الأوعية الكبيرة المتموّجة، فهذه تحتاج تقييماً وعائياً وقد تحتاج تصليباً أو إجراءً آخر، ومحاولة معالجتها بالليزر السطحي تُهدر الوقت. ولا يُفتّح البقع البنية أو الكَلَف لأن هذه مشكلة صباغ لا وعاء. ولا يمنع ظهور شعيرات جديدة مع الشمس والعمر.

وهناك احتياطات مهمّة: التسمير أو التعرّض الشمسي الحديث يرفع خطر الحرق وفرط التصبّغ خصوصاً في البشرة المتوسّطة والداكنة، ومسيّلات الدم تزيد الكدمات، والحمل يؤجّل الجلسات، وأي وحمة تنمو بسرعة أو تنزف أو تتقرّح تحتاج تشخيصاً قبل أي علاج تجميلي.
Vascular laser therapy uses wavelengths that are absorbed by haemoglobin inside small superficial blood vessels. The energy becomes heat within the vessel itself, which then coagulates, closes, and is gradually cleared by the body, while the surrounding skin stays relatively unharmed because the energy is targeted at red colour rather than at skin.

It is used for visible facial capillaries on the cheeks and nose, persistent flushing and telangiectasia associated with rosacea, port-wine stains in children and adults, superficial haemangiomas after specialist assessment, some fine spider veins on the leg, and small vascular lesions such as cherry angiomas. The choice of wavelength, spot size, and pulse duration depends on vessel depth and calibre and on skin tone, and suitability is determined by clinical examination rather than by images circulating online.

What can be expected is gradual fading of the treated vessels after one to several sessions, with temporary bruising or redness in between. Port-wine stains need a series of sessions started early and may lighten considerably without disappearing completely, while the visible redness of rosacea usually improves noticeably.

The limits are explicit. Laser closes visible vessels; it does not cure rosacea as a chronic condition. Redness can return and may need maintenance sessions alongside topical treatment and avoidance of triggers such as heat, sun, and spicy food. It does not treat true leg varicose veins or large tortuous vessels, which need vascular assessment and possibly sclerotherapy or another procedure; treating them with a superficial laser simply wastes time. It does not lighten brown patches or melasma, because those are pigment rather than vascular problems. And it does not prevent new capillaries from appearing with sun exposure and age.

Important precautions apply. Recent tanning or sun exposure raises the risk of burns and hyperpigmentation, particularly in medium and darker skin tones; blood thinners increase bruising; pregnancy defers treatment; and any lesion that grows rapidly, bleeds, or ulcerates needs a diagnosis before any cosmetic treatment is considered.
