علاج اضطرابات الذاكرة مسار طبي يبدأ بتحديد سبب تدهور الذاكرة والوظائف الإدراكية قبل وصف أي دواء. النسيان المتكرّر ليس تشخيصاً بحدّ ذاته، بل عَرَض قد ينشأ عن أسباب قابلة للتصحيح تماماً مثل نقص فيتامين ب12، أو خلل الغدة الدرقية، أو الاكتئاب، أو اضطرابات النوم وانقطاع النفس النومي، أو أثر جانبي لدواء يتناوله المريض، أو نقص السوائل والأملاح، وقد ينشأ عن مرض تنكّسي مثل ألزهايمر أو عن خرف وعائي تالٍ لجلطات صغيرة متكرّرة.

لذلك يقوم التقييم على مقابلة سريرية مفصّلة مع المريض ومع شخص مقرّب يعرف تسلسل التغيّر، ثم اختبارات إدراكية مكتبية، ثم تحاليل دم مستهدَفة ومراجعة كاملة للأدوية، ثم تصوير الدماغ لاستبعاد الأسباب البنيوية. هذا الترتيب مهمّ لأن تصوير الدماغ وحده لا يشخّص سبب ضعف الذاكرة، والنتيجة «الطبيعية» في التصوير لا تنفي وجود مشكلة إدراكية حقيقية.

من المهم أن يكون الهدف واضحاً منذ البداية: في الأمراض التنكّسية مثل ألزهايمر لا يوجد حتى اليوم علاج يشفي المرض أو يعيد الخلايا العصبية التي فُقدت. الهدف الواقعي هو إبطاء أثر المرض على الحياة اليومية، والحفاظ على أكبر قدر من الوظيفة والاستقلالية، وضبط الأعراض المرافقة مثل التخبّط والقلق واضطراب النوم، ودعم من يقدّم الرعاية. أمّا الأسباب القابلة للعلاج فقد تتحسّن تحسّناً واضحاً بعد تصحيحها، وهذا وحده سبب كافٍ لعدم تأجيل التقييم.

حدود العلاج يجب أن تُقال صراحة: الأدوية المستخدمة في الخرف لا توقف تقدّم المرض، والمكمّلات الغذائية ومنتجات «تنشيط الذاكرة» المتداولة لا تعالج ألزهايمر ولا تمنعه، ولا يوجد فحص واحد يحسم التشخيص. في المقابل، ضبط ضغط الدم والسكر والدهون، والنشاط البدني المنتظم، وعلاج مشكلات النوم والسمع، والنشاط الذهني والاجتماعي، هي أكثر ما يملكه الطبيب اليوم لإبطاء التدهور. الترشيح لأي دواء أو برنامج تأهيل إدراكي يُحدَّد بالفحص السريري ونتائج التقييم، لا بقراءة صفحة على الإنترنت.
Treatment of memory disorders is a diagnostic pathway before it is a prescription. Forgetfulness is a symptom, not a diagnosis, and the first task is to identify why memory and thinking have declined. Some causes are fully correctable: vitamin B12 deficiency, thyroid dysfunction, depression, poor sleep and obstructive sleep apnoea, side effects of medicines the patient already takes, alcohol use, or disturbed sodium and hydration. Other causes are degenerative, such as Alzheimer's disease, or vascular, following repeated small strokes. Each of these leads to a different plan.

Assessment therefore starts with a structured interview with the patient and with a close informant who can describe how the change unfolded over time, followed by office-based cognitive testing, targeted blood work, a full medication review, and brain imaging to exclude structural causes. The order matters. Imaging alone cannot diagnose the cause of memory loss, and a scan reported as normal does not rule out a genuine cognitive disorder.

The goal of care must be stated honestly. For degenerative disease such as Alzheimer's there is no treatment today that cures the illness or restores neurons that have already been lost. The realistic aim is to slow the impact of the disease on daily life, preserve function and independence for as long as possible, manage associated symptoms such as agitation, anxiety and disturbed sleep, and support the caregiver. Where a correctable cause is found, cognition may improve substantially once it is treated, which is reason enough not to postpone evaluation.

The limits deserve equal emphasis. Medicines used in dementia do not halt disease progression, over-the-counter supplements and memory-boosting products do not treat or prevent Alzheimer's disease, and no single test settles the diagnosis. What genuinely helps is control of blood pressure, diabetes and cholesterol, regular physical activity, treatment of sleep and hearing problems, and continued mental and social engagement. Whether any medication or cognitive rehabilitation programme is appropriate is decided by clinical examination and test results, not by reading about it online.

ألم العصب الخامس، أو العُصاب الألمي لثلاثي التوائم، ألم عصبي في الوجه يوصف بأنه صاعق أو كالصعقة الكهربائية أو كالطعن، يأتي في نوبات قصيرة تتراوح من ثوانٍ إلى نحو دقيقتين، ويصيب عادة شقاً واحداً من الوجه في منطقة الخدّ أو الفكّ أو حول العين. ما يميّزه أن محرّضاته تافهة في الظاهر: لمسة خفيفة على الوجه، تفريش الأسنان، المضغ، الحلاقة، شرب سائل بارد، أو هبّة هواء.

السبب الأشيع ضغط وعاء دموي على جذر العصب عند خروجه من جذع الدماغ، ما يؤدي إلى تآكل الغلاف المُيَليني وإطلاق نبضات ألم غير منضبطة. وقد يكون السبب مرضاً آخر مثل التصلّب اللويحي أو كتلة تضغط على مسار العصب، ولهذا يُطلب تصوير بالرنين المغناطيسي، خصوصاً عند صغر السنّ، أو إصابة الجانبين، أو وجود نقص حسّي أو ضعف عضلي مستمر في الوجه.

يبدأ العلاج دوائياً، بأدوية مضادّة للاختلاج تعمل على تهدئة الإفراغ الكهربائي المرضي في العصب، وتُرفَع تدريجياً تحت مراقبة الطبيب ومع فحوص مخبرية دورية في بعض الأدوية. عند فشل الدواء أو عدم تحمّل آثاره الجانبية، تُطرح خيارات تداخلية: إجراءات عبر الجلد على العقدة ثلاثية التوائم مثل الكيّ بالترّدد الحراري أو الضغط بالبالون أو حقن الغليسرول، أو جراحة تخفيف الضغط الوعائي الميكروي التي تفصل الوعية عن العصب، أو جراحة إشعاعية موجّهة.

حدود العلاج مهمّة: هذا الألم ليس ألم أسنان، ومسكّنات الألم الشائعة ومضادّات الالتهاب لا تُسكّنه عادة، وأخطر ما يحدث للمريض أن تُخلع له أسنان سليمة بحثاً عن سبب لا وجود له في الفم — وهذا خطأ متكرّر ولا يُصلَح بعد وقوعه. كذلك لا يوجد إجراء يضمن نتيجة دائمة؛ فالانتكاس ممكن بعد أي خيار، وبعض الإجراءات قد تترك تنميلاً أو خمولاً حسّياً في الوجه. اختيار العلاج المناسب يُحدَّد بالفحص السريري والتصوير وشدّة النوبات وحالتك الصحية العامة، لا بقراءة عامة.
Trigeminal neuralgia is a facial nerve pain described as electric, shock-like or stabbing. It comes in brief paroxysms lasting from seconds to about two minutes, usually on one side of the face, over the cheek, jaw or around the eye. What characterises it is how trivial the triggers are: a light touch to the face, brushing teeth, chewing, shaving, drinking something cold, or a draught of air. Between attacks the face can feel entirely normal, which is why the condition is so often mistaken for a dental problem.

The commonest cause is a blood vessel compressing the nerve root where it leaves the brainstem, eroding the myelin sheath so that the nerve fires uncontrolled pain signals. Less often the cause is another disease, such as multiple sclerosis or a lesion along the nerve pathway. This is why MRI is requested, particularly in younger patients, when both sides are affected, or when there is persistent facial numbness or weakness rather than pure paroxysmal pain.

Treatment begins with medication. Anticonvulsant drugs that calm abnormal electrical discharge in the nerve are the first line, introduced gradually under supervision, with periodic blood monitoring for some agents. If medication fails or its side effects cannot be tolerated, interventional options are discussed: percutaneous procedures on the trigeminal ganglion such as radiofrequency thermal lesioning, balloon compression or glycerol injection; microvascular decompression surgery, which separates the offending vessel from the nerve; or targeted stereotactic radiosurgery.

The limits must be explicit. This is not toothache, and ordinary painkillers or anti-inflammatory tablets usually do not relieve it. The most damaging outcome for patients is the extraction of healthy teeth in search of a cause that is not in the mouth, a frequent error that cannot be undone. Equally, no procedure guarantees a permanent result: relapse is possible after every option, and some interventions leave facial numbness or reduced sensation. Which treatment suits you is determined by clinical examination, imaging, attack severity and your general health, not by general reading.

شلل العصب السابع، المعروف بشلل بيل، ضعف مفاجئ في عضلات نصف الوجه ينشأ عن خلل في العصب الوجهي، فيصبح إغلاق العين في الجهة المصابة صعباً، وتتدلّى زاوية الفم، ويسيل الماء عند الشرب، وقد يصحبه ألم خلف الأذن، وثقل في السمع أو حساسية مفرطة للصوت، وتغيّر في تذوّق مقدّمة اللسان. الضعف يشمل الجبهة والعين والفم في الشقّ نفسه، ويظهر عادة خلال ساعات إلى يوم أو يومين.

التمييز الأهم في اللحظة الأولى هو بين شلل العصب السابع المحيطي وبين الجلطة الدماغية. في شلل بيل يُصاب نصف الوجه كاملاً بما فيه الجبهة، ولا يوجد ضعف في الأطراف. أمّا إذا كانت الجبهة سليمة أو الرفع فوق الحاجب ممكناً، أو رافق الضعف تلعثماً في الكلام أو ضعفاً في يد أو رجل أو تشوّشاً في الوعي، فهذه حالة إسعافية يجب فيها التوجّه إلى الطوارئ فوراً لأن الجلطة لها نافذة علاج زمنية ضيقة.

علاج شلل بيل يقوم على البدء المبكر: يُوصف كورتيزون فموي في أسرع وقت ممكن، وعادة يُستهدف بدؤه خلال الثلاثة أيام الأولى من ظهور الأعراض، ويُضاف مضادّ فيروسي في حالات مختارة بقرار الطبيب. إلى جانب ذلك تأتي حماية العين، وهي الأهمّ لمنع مضاعفة دائمة: قطرات مرطّبة نهاراً، ومرهم وتغطية العين ليلاً، لأن العين التي لا تُغلق تتقرّح قرنيّتها. ثم يبدأ تأهيل عصبي عضلي للوجه بتمارين أمام المرآة وتدريب على الحركات المتناسقة.

الحدود يجب أن تُقال: لا يوجد علاج يُسرّع عودة العصب بشكل مضمون، ومعظم المرضى يتحسّنون تحسّناً جيداً لكن قسماً منهم يبقى لديه ضعف متبقٍّ أو حركات مرافقة غير مقصودة. المضادّ الحيوي لا دور له في شلل بيل، والتحفيز الكهربائي المبكر لا يوجد دليل قوي على أنه يُسرّع الشفاء، والتدليك العنيف لا يفيد. أمّا الاعتقاد الشائع بأن سبب المرض هو الهواء البارد أو المكيّف فليس سبباً مثبتاً، والأخطر منه أن ينتظر المريض «الشفاء وحده» فتفوته نافذة العلاج المبكر. الترشيح للعلاج ونوعه يُحدَّدان بالفحص السريري لا بالقراءة.
Facial nerve palsy, commonly called Bell's palsy, is a sudden weakness of the muscles of one half of the face caused by dysfunction of the facial nerve. Closing the eye on the affected side becomes difficult, the corner of the mouth droops, liquid escapes while drinking, and there may be pain behind the ear, altered hearing or sound sensitivity, and changed taste at the front of the tongue. The weakness involves the forehead, eye and mouth on the same side, and usually develops over hours to a day or two.

The most important distinction in the first minutes is between peripheral facial palsy and a stroke. In Bell's palsy the whole half of the face is affected, forehead included, and there is no limb weakness. If the forehead is spared and the eyebrow can still be raised, or if the facial weakness comes with slurred speech, arm or leg weakness, or altered awareness, this is an emergency and the patient must go to the emergency department immediately, because stroke treatment has a narrow time window.

Management of Bell's palsy depends on starting early. Oral corticosteroid is prescribed as soon as possible, ideally begun within the first three days of symptom onset, and an antiviral agent is added in selected cases at the clinician's discretion. Alongside this comes eye protection, which is the single most important step in preventing permanent damage: lubricating drops during the day, ointment and taping or a patch at night, because an eye that cannot close is at risk of corneal ulceration. Neuromuscular facial rehabilitation with mirror exercises and coordinated-movement training then follows.

The limits should be stated plainly. No treatment guarantees a faster return of nerve function. Most patients recover well, but a proportion is left with residual weakness or unwanted linked movements. Antibiotics have no role in Bell's palsy, there is no strong evidence that early electrical stimulation speeds recovery, and vigorous massage does not help. The popular belief that cold air or air conditioning causes the condition is not established, and the more harmful belief is that one should simply wait for spontaneous recovery, which wastes the early treatment window. Whether and how you are treated is decided by clinical examination, not by reading.

مرض باركنسون مرض عصبي تنكّسي يصيب مجموعة من خلايا الدماغ المسؤولة عن إنتاج الدوبامين، فتظهر أعراض حركية مميّزة: بطء الحركة، وتصلّب العضلات، ورجفة تظهر عادة أثناء الراحة، ثم اضطراب في التوازن والمشي في المراحل الأبعد. لكن المرض ليس حركياً فقط؛ فكثير من المرضى يعانون قبل الأعراض الحركية أو معها من ضعف حاسّة الشمّ، والإمساك، واضطراب سلوك النوم، وهبوط الضغط عند الوقوف، وتغيّرات المزاج والقلق، وهذه الأعراض غير الحركية قد تكون أكثر تأثيراً على جودة الحياة من الرجفة نفسها.

التشخيص سريري بالأساس: يعتمد على الفحص العصبي وتوزيع الأعراض واستجابتها للعلاج، ويُستخدم التصوير لاستبعاد أسباب أخرى لا لتأكيد المرض. من المهم استبعاد الباركنسونية الناتجة عن أدوية معيّنة أو عن اعتلال وعائي، ومن المهم أيضاً معرفة أن الرجفة ليست دائماً باركنسون؛ فالرجفة الأساسية أكثر شيوعاً وتختلف في صفاتها وعلاجها تماماً.

الهدف من العلاج يجب أن يكون واضحاً وصادقاً: لا يوجد اليوم دواء يشفي مرض باركنسون أو يوقف تنكّس الخلايا. الهدف هو السيطرة على الأعراض، والحفاظ على أكبر قدر ممكن من الحركة والاستقلالية والوظيفة اليومية، وتأخير المضاعفات مثل السقوط، وإدارة الأعراض غير الحركية. ويُبنى ذلك على ثلاث ركائز: خطة دوائية مُفردة تعتمد على تعويض الدوبامين أو دعم عمله وعلى توقيت الجرعات بدقّة، وبرنامج تأهيل حركي منتظم، وتدبير الأعراض غير الحركية بالتوازي.

التأهيل ليس إضافة تجميلية بل جزء من العلاج: تدريب المشي بإيقاع خارجي، وتمارين الاتّساع الحركي والتوازن والقوّة، وتدريب مكثّف للصوت لمن ضعف صوته، وعلاج وظيفي لتبسيط مهام البيت. في مراحل معيّنة، وعند مرضى مختارين تكون استجابتهم للدواء جيدة لكن مع تقلّبات مزعجة، يُطرح خيار تحفيز الدماغ العميق أو أنظمة إعطاء الدواء المستمر. وحدود هذه الخيارات صريحة: تحفيز الدماغ العميق لا يشفي المرض ولا يوقف تقدّمه، ولا يُحسّن عادة الأعراض التي لا تستجيب للدوبامين مثل اضطراب التوازن المتقدّم أو التدهور الإدراكي. كذلك لا تُبدّل المكمّلات الغذائية مسار المرض. الترشيح لأي خيار يُحدَّد بالفحص السريري وتقييم فريق مختصّ.
Parkinson's disease is a degenerative neurological condition affecting brain cells that produce dopamine, which produces its characteristic motor features: slowness of movement, muscle rigidity, a tremor that typically appears at rest, and later difficulty with balance and walking. The disease is not only motor. Many patients experience, before or alongside the motor symptoms, reduced sense of smell, constipation, sleep behaviour disorder, blood pressure drops on standing, and changes in mood and anxiety. These non-motor symptoms often affect quality of life more than the tremor itself.

Diagnosis is primarily clinical, based on neurological examination, the pattern of symptoms and their response to treatment; imaging is used to exclude other causes rather than to confirm the disease. It is important to exclude parkinsonism caused by certain medicines or by vascular disease, and equally important to know that tremor is not always Parkinson's: essential tremor is more common and differs entirely in its features and its management.

The goal of treatment must be honest and explicit. No medicine available today cures Parkinson's disease or halts the degeneration of cells. The aim is to control symptoms, preserve as much movement, independence and daily function as possible, delay complications such as falls, and manage non-motor symptoms. This rests on three pillars: an individualised drug plan built on replacing dopamine or supporting its action, with careful attention to dose timing; a consistent movement rehabilitation programme; and parallel management of non-motor symptoms.

Rehabilitation is part of treatment, not an optional extra: gait training with external cueing, amplitude, balance and strength exercises, intensive voice training for those whose speech has weakened, and occupational therapy to simplify daily tasks. At certain stages, in selected patients who still respond well to medication but suffer troublesome fluctuations, deep brain stimulation or continuous drug delivery systems are discussed. Their limits are clear: deep brain stimulation does not cure the disease or stop its progression, and it generally does not improve symptoms that do not respond to dopamine, such as advanced balance impairment or cognitive decline. Dietary supplements do not change the course of the illness. Suitability for any option is determined by clinical assessment and specialist team review.

فحص السائل الشوكي، أو البزل القطني، إجراء تشخيصي تُسحب فيه عيّنة صغيرة من السائل الدماغي الشوكي بإبرة رقيقة تُدخل بين فقرات أسفل الظهر تحت تخدير موضعي. هذا السائل يحيط بالدماغ والحبل الشوكي، وتحليله يعطي معلومات لا يعطيها الدم ولا التصوير: وجود عدوى أو التهاب، أو خلايا غير طبيعية، أو مؤشّرات مناعية، أو دم قديم، إضافة إلى قياس ضغط السائل نفسه.

يُطلب الفحص في حالات محدّدة: الشكّ بالتهاب السحايا أو التهاب الدماغ، وتقييم التصلّب اللويحي وأمراض المناعة العصبية، والشكّ بنزف تحت العنكبوتية عندما يكون التصوير سلبياً والقصة مقنعة، وارتفاع الضغط داخل القحف مجهول السبب حيث يكون قياس الضغط جزءاً من التشخيص، وبعض حالات اعتلال الأعصاب والتهاب الجذور، والشكّ بانتشار خلايا سرطانية إلى السحايا. ويُستخدم المسار نفسه أحياناً لإعطاء أدوية داخل القراب أو تخدير نصفي.

الخرافة الأكثر شيوعاً وضرراً هي الاعتقاد أن الإبرة «تُغرز في النخاع الشوكي وتسبّب الشلل». الحقيقة أن البزل يُجرى في مستوى أسفل من نهاية الحبل الشوكي، فلا يوجد حبل شوكي في موضع الإبرة لتُصيبه، والإجراء يُجرى بمعالم تشريحية واضحة وبتوجيه بالأمواج فوق الصوتية أو التنظير الشعاعي عند الحاجة. أمّا المضاعفة الأكثر واقعية فهي الصداع بعد البزل، وهو ألم يزيد بالوقوف ويخفّ بالاستلقاء، وغالباً يتحسّن بالسوائل والراحة والمسكّنات، ويُعالَج بإجراء بسيط إن استمرّ.

حدود الفحص يجب أن تُذكر: البزل القطني وسيلة تشخيص لا علاج بذاته، إلا في حالات محدّدة مثل تفريغ السائل لتخفيف الضغط أو إعطاء دواء داخل القراب. نتيجة طبيعية لا تنفي كل الأمراض، وبعض التحاليل المتخصّصة تحتاج أياماً إلى أسبوعين. وهناك حالات يُمنع فيها البزل أو يُؤجّل: الشكّ بكتلة أو ارتفاع ضغط قحفي مع عجز عصبي أو تشوّش وعي قبل التصوير، وعدوى في جلد موضع الإبرة، واضطراب تخثّر شديد أو نقص صفيحات، والعلاج بمضادّات التخثّر التي يجب تنظيم إيقافها بقرار الطبيب. الترشيح للإجراء وتوقيته يُحدَّدان بالفحص السريري ونتائج التصوير والتحاليل.
A lumbar puncture, also called a spinal fluid test, is a diagnostic procedure in which a small sample of cerebrospinal fluid is withdrawn through a fine needle inserted between the vertebrae of the lower back under local anaesthetic. This fluid surrounds the brain and spinal cord, and analysing it provides information that neither blood tests nor imaging can give: evidence of infection or inflammation, abnormal cells, immune markers, or old blood, along with a direct measurement of the fluid pressure itself.

The test is requested in specific situations: suspected meningitis or encephalitis; assessment of multiple sclerosis and other neuroimmune diseases; suspected subarachnoid haemorrhage when imaging is negative but the history is convincing; idiopathic intracranial hypertension, where measuring the opening pressure is part of the diagnosis; certain neuropathies and radiculopathies; and suspected spread of cancer cells to the meninges. The same route is sometimes used to deliver medication into the spinal fluid or for spinal anaesthesia.

The most common and most harmful myth is the belief that the needle is pushed into the spinal cord and can cause paralysis. In reality the puncture is performed at a level below where the spinal cord ends, so there is no spinal cord at the needle site to injure. The procedure uses clear anatomical landmarks, with ultrasound or fluoroscopic guidance when needed. The realistic complication is post-puncture headache, a pain that worsens on standing and eases on lying flat; it usually improves with fluids, rest and simple analgesia, and can be treated with a straightforward procedure if it persists.

The limits should be stated. Lumbar puncture is a diagnostic tool rather than a treatment, except in specific situations such as removing fluid to relieve pressure or delivering intrathecal medication. A normal result does not exclude every disease, and some specialised assays take days to two weeks. There are also situations where the procedure is contraindicated or postponed: suspected mass lesion or raised intracranial pressure with neurological deficit or altered consciousness before imaging, infection of the skin at the puncture site, significant coagulopathy or low platelets, and treatment with anticoagulants, whose interruption must be arranged by the treating doctor. Whether and when the test is appropriate is decided by clinical assessment alongside imaging and blood results.
