الجراحة والإجراءات التداخلية لعلاج الألم المزمن مجموعة تدخّلات تُوجَّه إلى مصدر الألم أو إلى مسار نقله العصبي عندما لا تكفي المحاور الأخرى من الخطة. والألم المزمن — أي الألم المستمرّ أو المتكرّر أكثر من ثلاثة أشهر — حالة قائمة بذاتها وليس مجرّد عَرَض ينتظر سبباً؛ إذ تتغيّر معالجة الإشارة في الأعصاب والحبل الشوكي والدماغ فيصير الجهاز العصبي نفسه جزءاً من المشكلة، ولذلك يتأثّر النوم والمزاج والقدرة على العمل، ولا يُقاس النجاح بصفر ألم بل بتحسّن الوظيفة وجودة الحياة.

لهذا تُبنى الخطة على محاور متعدّدة تعمل معاً: تأهيل حركي متدرّج يعيد الحركة بلا خوف، ومعالجة النوم، وعلاج نفسي معرفي سلوكي موجّه للألم، وتدبير دوائي مُراقب، وإجراءات تداخلية عند الحاجة. أمّا التدخّلات نفسها فتشمل حقناً موضعية موجّهة بالتصوير لتحديد مصدر الألم وتخفيفه، والكيّ بالترددات الحرارية للأعصاب الحسّية الصغيرة في مفاصل العمود، وتحفيز الحبل الشوكي أو الأعصاب المحيطية بمولّد يُزرع بعد تجربة مؤقّتة، والمضخّة الدوائية المزروعة في حالات مختارة، وجراحة تحرير عصب مضغوط أو تثبيت فقرة عند وجود سبب بنيوي واضح مطابق للأعراض.

وحدود هذه التدخّلات مهمّة بقدر فوائدها: الإجراء لا «يستأصل» الألم المزمن ولا يُنهي الحاجة إلى التأهيل، وأثر بعض الحقن مؤقّت ويُقاس بما يسمح به من عمل تأهيلي. ووجود تغيّرات في صورة الرنين لا يعني أنّها سبب ألمك، فكثير من التغيّرات المرتبطة بالعمر توجد عند أشخاص بلا أي ألم؛ ولذلك فإنّ جراحة تُقرَّر على الصورة وحدها بلا مطابقة سريرية قد تزيد الألم لا تخفّفه. وتكرار عمليات العمود الفقري بلا مؤشّر واضح سبب معروف لألم أشدّ وأعقد.

أمّا التدبير الدوائي فله قواعد صارمة: \*\*المسكّنات الأفيونية ليست علاجاً أولياً للألم المزمن غير السرطاني، وإن استُخدمت فتحتاج إشرافاً دقيقاً وأهدافاً وظيفية مكتوبة ومراجعة دورية للفائدة والضرر وخطة خروج متدرّجة من البداية\*\*، مع الانتباه لخطر الاعتماد وتراجع الفائدة مع الوقت وتفاعلها الخطير مع المهدّئات. و\*\*الإفراط في مسكّنات الصداع لأكثر من أيام قليلة في الأسبوع يسبّب صداع فرط استخدام الدواء، فيصير الدواء نفسه مصدر الألم\*\*؛ والحلّ هو سحبه بخطة طبية لا زيادته. وترشيحك لأي إجراء تداخلي، وتوقيته وتسلسله، يُحدَّد بالفحص السريري وتقييم شامل، لا بالقراءة.
Interventional and surgical treatment of chronic pain covers procedures directed at the source of pain or at the nerve pathway carrying it, used when the other parts of a pain plan are not enough on their own. Chronic pain — pain that persists or recurs for more than three months — is a condition in its own right rather than a symptom waiting for a cause. Signal processing in the nerves, spinal cord and brain changes, so the nervous system itself becomes part of the problem, which is why sleep, mood and the ability to work are all affected. Success is therefore not measured as zero pain but as improved function and quality of life.

For that reason the plan is built on several axes working together: graded physical rehabilitation that restores movement without fear, treatment of sleep, pain-focused cognitive behavioural therapy, carefully supervised medication, and interventional procedures when indicated. The procedures themselves include image-guided local injections used both to identify and to relieve a pain source, radiofrequency treatment of the small sensory nerves supplying spinal joints, spinal cord or peripheral nerve stimulation using a generator implanted after a temporary trial, an implanted drug delivery pump in selected cases, and surgery to release a compressed nerve or stabilise a spinal segment when there is a clear structural cause that matches the symptoms.

The limits matter as much as the benefits. A procedure does not remove chronic pain or end the need for rehabilitation, and the effect of some injections is temporary and is judged by the rehabilitation work it makes possible. Changes seen on an MRI scan do not prove they are the cause of your pain, since many age-related findings are present in people with no pain at all; surgery decided on imaging alone, without clinical correlation, can leave pain worse rather than better. Repeated spinal operations without a clear indication are a well-recognised route to more severe and more complex pain.

Medication follows strict rules. \*\*Opioid analgesics are not a first-line treatment for chronic non-cancer pain, and if they are used they require close supervision, written functional goals, regular review of benefit against harm, and a gradual exit plan agreed from the outset\*\*, with attention to dependence, waning benefit over time and dangerous interaction with sedatives. \*\*Overusing painkillers for headache on more than a few days each week causes medication-overuse headache, in which the drug itself becomes the source of the pain\*\*; the answer is a supervised withdrawal plan, not a higher dose. Whether any interventional procedure suits you, and in what order and timing, is decided by clinical assessment, not by reading.
