إيكو القلب للأطفال، أو تخطيط صدى القلب، فحص تصويري يستخدم الموجات فوق الصوتية لإظهار تشريح القلب وحجراته وصماماته وسماكة عضلته واتجاه تدفّق الدم داخله. لا يستخدم إشعاعاً، ولا يحتاج إبراً في الحالات المعتادة، ولا يؤلم الطفل؛ كل ما يشعر به هو الجلّ البارد وحركة المجس على صدره. وهو الفحص الأول والأهم في تشخيص عيوب القلب الخلقية ومتابعتها.

يُطلب الفحص عادة عند سماع لغط في القلب، أو زُرقة حول الشفتين، أو تعب وتعرّق أثناء الرضاعة، أو ضعف في النمو والوزن، أو تنفّس سريع متكرّر، أو خفقان أو إغماء مع المجهود، أو ألم صدر مع الجهد، أو وجود تاريخ عائلي لأمراض عضلة القلب أو موت قلبي مبكر، أو قبل بعض العمليات. وللفحص تطبيق قبل الولادة يُسمّى إيكو الجنين، يُجرى عبر بطن الأم لتقييم قلب الجنين عند وجود شك أو تاريخ عائلي أو سكري حمل، ويسمح بالتخطيط للولادة في مكان مهيَّأ إن كان هناك عيب مهم. كما أن الفحص هو الوسيلة الأساسية لكشف ضيق الشريان الأورطي ومتابعته، وهو عيب يُعالَج بالقسطرة أو الجراحة بحسب العمر والشدّة.

ولا بدّ من معرفة حدود الفحص: الإيكو يُصوّر البنية والوظيفة ولا يسجّل كهربية القلب، فاضطراب النظم يحتاج تخطيطاً كهربائياً أو جهاز هولتر لتسجيل ساعات؛ وهو لا يُظهر الشرايين التاجية بتفصيل كافٍ، ولا يشخّص أمراض الرئة أو الحساسية التي قد تسبّب أعراضاً مشابهة. ومعظم اللغط الذي يُسمع عند الأطفال لغط بريء لا يعني مرضاً، ونتيجة طبيعية اليوم لا تعني استحالة ظهور مشكلة لاحقاً، فبعض العيوب تتّضح مع النمو. جودة الصورة تتأثّر بتعاون الطفل ووزنه ونافذة التصوير.

الترشيح للفحص وتفسير نتائجه يُحدَّدان بالفحص السريري عند طبيب قلب الأطفال، لا بقلق الأهل ولا بمقارنة تقارير الآخرين. والتقرير يُقرأ دائماً مع القصة السريرية للطفل ومنحنى نموّه.
Echocardiography for children, often simply called a heart echo, is an imaging test that uses ultrasound to display the anatomy of the heart, its chambers and valves, the thickness of its muscle and the direction of blood flow inside it. It involves no radiation, needs no needles in routine cases, and does not hurt: all the child feels is cool gel and the probe moving over the chest. It is the first and most important test for diagnosing and monitoring congenital heart disease.

The test is usually requested when a murmur is heard, when there is bluish discolouration around the lips, tiring or sweating during feeds, poor growth and weight gain, repeated fast breathing, palpitations or fainting with exertion, chest pain on effort, a family history of cardiomyopathy or early cardiac death, or before certain operations. It also has a prenatal application known as fetal echocardiography, performed through the mother's abdomen to assess the fetal heart when there is a suspicion, a family history or gestational diabetes, allowing delivery to be planned in an equipped centre if a significant defect is found. Echocardiography is likewise the main tool for detecting and following coarctation of the aorta, a narrowing treated by catheter intervention or surgery depending on age and severity.

Its limits must be understood. An echo shows structure and function but does not record the heart's electrical activity, so a rhythm disturbance needs an electrocardiogram or a Holter recording over hours; it does not display the coronary arteries in adequate detail, and it does not diagnose lung disease or allergy that may cause similar symptoms. Most murmurs heard in children are innocent and do not mean disease, and a normal study today does not make a later problem impossible, since some defects only become apparent with growth. Image quality also depends on the child's cooperation, body size and imaging window.

Whether the test is needed, and how its findings should be read, are decided by clinical examination with a paediatric cardiologist rather than by parental worry or by comparing reports with other families. The report is always interpreted alongside the child's clinical story and growth curve.
