## Procedure steps

### 1. Assessment and identifying the pain source

A neurological examination of strength, sensation and reflexes, matching the pain distribution to the compressed level on MRI, review of medicines and comorbidities, and exclusion of conditions that require surgery rather than an injection.

### 2. Preparation and image guidance

You lie on your front or side, the skin is disinfected, and the target level is identified with fluoroscopy or ultrasound, with vital signs monitored throughout.

### 3. Local anaesthesia and needle placement

The skin is numbed, then a fine needle is advanced gradually towards the target space. You remain awake and able to describe what you feel, which adds to both safety and accuracy.

### 4. Confirming position, then injecting

Contrast may be used to confirm that the medicine will spread in the correct space and has not entered a blood vessel, then the mixture is injected slowly. A brief sensation of pressure or fullness is expected.

### 5. Observation and linking the result to rehabilitation

A short period of observation before discharge, recording pain levels before and after so benefit can be judged objectively, then starting the physiotherapy programme inside the window of relief rather than resting through it.

## About this service

A therapeutic spinal injection places medication precisely into a defined space in or around the spine, such as the epidural space or the area surrounding a nerve root. It contains an anti-inflammatory medicine of the corticosteroid class together with a local anaesthetic, and it is delivered under X-ray or ultrasound guidance so the drug reaches the intended target with the smallest necessary volume.

Its purpose is to calm the inflammation around an irritated nerve, the very inflammation that produces the burning pain radiating into the leg or arm along with numbness, as happens in disc herniation and spinal canal stenosis. When the injection works, pain settles enough for the patient to move again and engage properly with physiotherapy, and that is its genuine role.

For this reason it must be clear what the injection does not do. It does not remove a herniated disc or push it back into place, it does not widen a narrowed canal, it does not repair spinal instability, and it does not treat the underlying structural problem on its own. It opens a window of pain relief that enables rehabilitation rather than replacing it. Patients who rely on the injection alone, without strengthening the trunk muscles, correcting loading and movement patterns and managing weight, usually see symptoms return. Its effect is inherently temporary and varies widely between individuals, repetition is limited, and it is not repeated when it has produced no clear benefit.

Some situations have no place for an injection at all and call for urgent surgical assessment instead: progressive muscle weakness in a limb, numbness in the saddle area, or loss of bladder or bowel control. Injections are also postponed in active infection, clotting problems, or when blood thinners cannot be safely managed around the procedure.

Candidacy is decided by clinical examination and by matching symptoms to the level of pathology seen on imaging, because not every episode of back pain is a candidate.

## Preparation

Tell your doctor about every medicine you take, especially blood thinners, anticoagulants and aspirin, and do not stop any of them yourself but ask about safe timing. Mention if you have diabetes, because steroids can temporarily raise blood sugar and you may need closer monitoring for a few days; if you are pregnant or might be; and if you have any allergy to contrast, local anaesthetic or antiseptics, an active infection or fever, a clotting disorder or a weakened immune system. Bring your MRI reports and X-ray images with you. Follow any fasting instructions you are given. Wear comfortable clothing and arrange for someone to drive you, since a temporary heavy feeling in the leg is possible and driving straight after the procedure is not advised.

## Aftercare

Rest for the remainder of the day without lying down for long stretches, move gently, and avoid heavy lifting and repeated bending on the first day. Expect pain to increase slightly once the local anaesthetic wears off before improvement begins, and note that the steroid effect may take several days to appear. Monitor your blood sugar closely if you are diabetic, and do not be alarmed by mild facial flushing or a night or two of poor sleep. Most importantly, start physiotherapy inside the window of relief, because the injection enables rehabilitation and does not replace it. Record how much the pain improved and for how long, as this determines the next step. \*\*Seek care immediately for sudden or progressive leg weakness, loss of bladder or bowel control, numbness in the saddle area, fever and chills with worsening back pain, a severe headache that worsens on standing and eases on lying flat, or redness and discharge at the injection site.\*\*
