## Procedure steps

### 1. Genetic counselling and informed consent

The indication, family history and previous results are reviewed, and the alternatives are explained: blood-based screening, amniocentesis after about fifteen weeks, and the option of no diagnostic test at all. Consent is signed once the risks and the meaning of each possible result are understood.

### 2. Ultrasound mapping

Gestational age, fetal heartbeat, number of fetuses, placental position and fluid volume are confirmed, and the route is chosen: through the abdominal wall in most cases, or through the cervix if the placental position requires it.

### 3. Sterile preparation and needle insertion

The skin is sterilised and locally anaesthetised for the abdominal route, then a fine needle is advanced under live ultrasound guidance into the placental tissue, away from the amniotic sac. Most women describe pressure and period-like cramping rather than sharp pain.

### 4. Sampling and adequacy check

A small quantity of villi is aspirated and checked immediately for adequacy and for absence of maternal tissue; a second pass in the same session is occasionally needed. The sample is sent for a rapid preliminary test and then for full chromosomal culture.

### 5. Post-procedure monitoring and results

The fetal heartbeat is documented afterwards, anti-D immunoglobulin is given to rhesus-negative women, and you are observed briefly before leaving. The rapid result usually arrives within days and the full result may take about two weeks, and both are explained in a dedicated visit or call.

## About this service

Chorionic villus sampling is a diagnostic test in which a small amount of chorionic villus tissue — placental tissue that carries the same genetic make-up as the fetus — is drawn with a fine needle under ultrasound guidance so that the fetal chromosomes or a specific gene can be examined. It is performed early in pregnancy, usually between about the eleventh and fourteenth weeks, which is precisely why it is chosen when an early answer is needed.

The test is usually offered after a high-risk screening result (cell-free fetal DNA or combined first-trimester screening), when an ultrasound finding raises concern, when a known inherited condition runs in the family, after a previous pregnancy affected by a chromosomal condition, or in consanguineous couples with a specific testable genetic disease. The essential difference is that screening tests give a probability, whereas villus sampling gives a diagnosis on the sample obtained.

Its limits deserve to be stated plainly. It answers the specific questions that were requested; it does not screen for every disease and cannot promise a healthy baby. It does not measure intellectual ability, and it does not show structural malformations — that is the job of the second-trimester anatomy scan. It does not replace the rest of antenatal care, and it is not used for social sex selection. In a small number of cases the result is inconclusive, or a chromosomal mosaicism confined to the placenta appears, and amniocentesis is then requested later for confirmation.

Risks include cramping and light spotting in many women, and rarely fluid leakage or infection, along with a small additional risk of miscarriage that your doctor should explain using the figures of the centre performing it rather than generic numbers. The test is not performed before about the tenth to eleventh week, because sampling earlier than that was linked with limb development problems. The decision is ultimately yours, taken after genetic counselling that makes clear what would change in the management of the pregnancy after each possible result, and suitability is determined by clinical and ultrasound assessment rather than by reading.

## Preparation

Fasting is not usually required, but ask the centre about bladder filling, since some routes need a moderately full bladder. Tell your doctor your blood group and rhesus status, and report any active or chronic infection — particularly hepatitis B or C or HIV — because this changes the assessment and precautions taken. List all your medicines, especially blood thinners and aspirin, any drug or antiseptic allergy, any bleeding or cramping in this pregnancy, and any previous uterine surgery. Bring your earlier scan reports and screening results. Arrange for someone to accompany and drive you, and plan to rest for the remainder of the day without heavy work or long travel.

## Aftercare

Expect mild cramping and light spotting that may last a day or two. Rest relatively for the first twenty-four to forty-eight hours and avoid heavy lifting, strenuous exercise and intercourse for as long as your doctor advises. Use only the painkiller you were prescribed, do not use vaginal washes or pessaries on your own initiative, and check your temperature if you feel unwell. Do not miss the results appointment or your next scan. \*\*Seek emergency care immediately if you have vaginal bleeding as heavy as a period or more, fluid leaking from the vagina, regular painful contractions, a temperature of 38C or above or shivering, foul-smelling discharge, or severe abdominal pain that does not settle.\*\*
