وأهمّ ما يجب فهمه هو حدّ الإجراء: الإزالة تُخلّصك من \*\*النموّة\*\* لا من \*\*الفيروس\*\*. قد يبقى الفيروس كامناً في الجلد المحيط، وقد تعود الثآليل — خصوصاً في الأشهر الأولى بعد العلاج — وعودتها لا تعني بالضرورة فشل العلاج ولا إصابة جديدة ولا خطأ من أحد. كذلك فإن إزالة الثآليل لا تعني حماية كاملة للشريك، لأن الفيروس قد ينتقل من جلد يبدو سليماً. ولا تستخدم إطلاقاً مذيبات الثآليل المتاحة للجلد العادي على المنطقة التناسلية؛ فهي تسبّب حروقاً وتقرّحات وتصبّغات وقد تُفاقم الحالة. واللقاح المضاد لفيروس الورم الحلمي وقائي ولا يعالج ثؤلولاً موجوداً، ومع ذلك تُناقَش قيمته للحماية المستقبلية.

ونضيف طمأنة صادقة: العدوى بفيروس الورم الحلمي شائعة جداً بين الناس، ووجودها لا يعني بالضرورة تعدّد علاقات ولا سلوكاً معيّناً، وقد تظهر بعد وقت طويل من التعرّض. الثآليل التناسلية نفسها حميدة وليست سرطاناً، لكن بعض أنماط الفيروس الأخرى مرتبطة بتغيّرات في عنق الرحم، ولهذا تُنصح المريضة بالالتزام بفحص عنق الرحم المخصّص لذلك حسب توجيه طبيبها.

## خطوات الإجراء

### 1. الفحص السريري وتحديد الامتداد

يُفحص الجلد والأغشية المخاطية في المنطقة التناسلية وحول الشرج بإضاءة جيّدة لتحديد عدد الثآليل وأماكنها، ويُسأل عن المدّة والأعراض والحمل والمناعة، ويُناقَش فحص الأمراض المنقولة جنسياً وإبلاغ الشريك بسرّية تامّة.

### 2. اختيار طريقة الإزالة

تُختار الطريقة بحسب عدد الثآليل وحجمها وموقعها وحالة المريض: علاج موضعي في البيت، أو تبريد، أو كيّ كهربائي، أو ليزر، أو استئصال جراحي للنموّات الكبيرة، مع استثناء ما هو ممنوع في الحمل.

### 3. التعقيم والتخدير الموضعي

تُنظَّف المنطقة وتُعقَّم، ويُستخدم كريم مخدّر أو حقن تخدير موضعي بحسب الطريقة والموقع، ويُنتظر حتى يعمل التخدير بالكامل، وتُشرح للمريض الإحساسات المتوقّعة خلال الجلسة حتى لا يفاجئه شيء.

### 4. الإزالة الفعلية وضبط النزف

تُزال الثآليل بالطريقة المختارة مع الحفاظ على أقلّ قدر من الأذى للجلد السليم المحيط، ويُضبَط أي نزف بسيط، وتُوضع ضمادة أو مرهم بحسب الموقع. قد لا يُعالَج كل شيء في جلسة واحدة إن كانت الثآليل كثيرة أو منتشرة.

### 5. المتابعة وكشف العودة ومناقشة اللقاح

يُحدَّد موعد مراجعة لتقييم الشفاء وكشف أي عودة مبكراً، وتُعالَج الثآليل الجديدة بسرعة لأنها أسهل وهي صغيرة، ويُناقَش اللقاح المضاد لفيروس الورم الحلمي كوقاية مستقبلية، ومتابعة فحص عنق الرحم عند النساء.

## Procedure steps

### 1. Clinical examination and mapping the extent

The genital and perianal skin and mucosa are examined under good light to map the number and location of warts. Duration, symptoms, pregnancy and immune status are reviewed, and testing for other sexually transmitted infections and confidential partner notification are discussed.

### 2. Choosing the removal method

The method is selected according to number, size, site and patient factors: home-applied topical treatment, freezing, electrocautery, laser, or surgical excision for larger growths, excluding any option contraindicated in pregnancy.

### 3. Antisepsis and local anaesthesia

The area is cleaned and disinfected, and either anaesthetic cream or injected local anaesthesia is used depending on method and site. Full onset is awaited, and the sensations to expect during the session are explained in advance so nothing takes the patient by surprise.

### 4. The removal itself and bleeding control

The warts are removed by the chosen method while sparing as much surrounding healthy skin as possible, minor bleeding is controlled, and a dressing or ointment applied according to site. When warts are numerous or widespread, not everything is treated in one session.

### 5. Follow-up, recurrence check and vaccine discussion

A review appointment is set to assess healing and detect recurrence early, since new warts are easier to treat while small. The HPV vaccine is discussed as future prevention, along with continued cervical screening for women.

## التحضير قبل الإجراء

أخبري الطبيب إن كنتِ حاملاً أو تخطّطين للحمل أو مرضعة، فبعض العلاجات ممنوعة تماماً في الحمل. اذكر كل أدويتك وخاصة مسيّلات الدم والكورتيزون وأدوية المناعة، وأي حساسية لمخدّر موضعي أو لاصق طبي، وأي مرض مزمن مثل السكري، وأي حالة نقص مناعة. لا تستخدم أي مذيب ثآليل أو كريم من الصيدلية على المنطقة قبل الفحص، ولا تحاول قصّ الثؤلول أو حرقه أو ربطه في البيت. لا تحلق المنطقة بالموس في اليوم نفسه لأن الجروح الصغيرة قد تنشر الفيروس وتؤلم مع العلاج؛ واسأل العيادة عن تعليماتها بالتحديد. البس ملابس داخلية قطنية وبنطالاً فضفاضاً يوم الجلسة، ورتّب ألّا تكون لديك مواعيد تتطلّب جلوساً طويلاً أو مجهوداً بعدها.

## العناية بعد الإجراء

توقّع في الأيام الأولى احمراراً وحرقة وانتفاخاً خفيفاً أو نزّاً بسيطاً في موضع العلاج، وقد تتقشّر المنطقة أو تتقرّح سطحياً ثم تشفى. نظّف بلطف بماء وصابون معتدل مرّة إلى مرّتين يومياً، وجفّف بالتنشيف لا بالفرك، وضع فقط ما وصفه لك الطبيب. لا تنزع القشور ولا تفتح أي فقاعة. تجنّب المسابح والبانيو والساونا وإزالة الشعر بالشمع أو الموس في المنطقة حتى يكتمل الشفاء، وتجنّب العلاقة الجنسية حتى يشفى الجلد ووفق توجيه طبيبك، واستخدم الواقي بعدها فهو يقلّل الانتقال ولا يمنعه كلّياً. الْبس القطن الفضفاض وتجنّب العرق الشديد أياماً. راجع فوراً عند: \*\*حرارة أو قشعريرة\*\*، \*\*ألم وتورّم متزايد مع احمرار ينتشر أو خروج قيح أو رائحة كريهة\*\*، \*\*نزيف لا يتوقّف بالضغط اللطيف عشر دقائق\*\*، \*\*عدم القدرة على التبوّل أو ألم شديد عند التبوّل\*\*، \*\*تقرّح واسع أو تغيّر لون الجلد إلى داكن أو أسود\*\*. وراجع أيضاً — بلا قلق مبالَغ — عند ظهور نموّات جديدة، فعلاجها مبكراً أسهل.

## About this service

Genital warts are benign skin growths caused by human papillomavirus, appearing on or around the genitals or the anal area in various forms: soft small bumps, cauliflower-shaped growths, or flat patches that are hard to see. Removal is a procedure aimed at clearing the visible growths and relieving symptoms and discomfort, and it may be performed by freezing, electrocautery, laser, surgical excision, or with topical treatments prescribed by the doctor and applied by the patient at home under precise instructions.

The choice of method is not a matter of preference. It depends on the number, size and site of the warts, on pregnancy, on immune status, and on what has already been tried. Some sensitive sites require a particular technique, and certain topical treatments are absolutely contraindicated in pregnancy, so all of this is decided by direct clinical examination.

The single most important thing to understand is the limit of the procedure: removal clears the \*\*growth\*\*, not the \*\*virus\*\*. The virus may persist in the surrounding skin, and warts can recur — particularly in the first months after treatment. Recurrence does not necessarily mean the treatment failed, nor a new infection, nor anyone's mistake. Removal also does not fully protect a partner, because the virus can be transmitted from skin that looks normal. Never use wart removers sold for ordinary skin on the genital area; they cause burns, ulceration and pigment change and can make matters considerably worse. The HPV vaccine is preventive and does not treat an existing wart, although its value for future protection is worth discussing.

An honest reassurance also belongs here: HPV infection is very common, its presence does not imply multiple partners or any particular behaviour, and warts may appear long after exposure. Genital warts themselves are benign and are not cancer, though other types of the same virus family are linked to cervical changes, which is why women are advised to keep up the dedicated cervical screening their doctor recommends.

## Preparation

Tell the doctor if you are pregnant, planning pregnancy or breastfeeding, since some treatments are entirely contraindicated in pregnancy. Mention all your medicines, especially blood thinners, corticosteroids and immune-modifying drugs, any allergy to local anaesthetic or adhesive dressings, any chronic condition such as diabetes, and any cause of reduced immunity. Do not apply any pharmacy wart remover or cream to the area before the examination, and never try to cut, burn or tie off a wart at home. Do not shave the area with a razor on the same day, as small cuts can spread the virus and sting during treatment; ask the clinic for its specific instruction. Wear cotton underwear and loose trousers on the day, and arrange not to have commitments afterwards that require long sitting or physical exertion.

## Aftercare

In the first days expect redness, stinging, mild swelling or slight oozing at the treated site; the area may crust or superficially ulcerate before healing. Clean gently with water and a mild soap once or twice daily, pat dry rather than rubbing, and apply only what your doctor prescribed. Do not pick crusts or open any blister. Avoid swimming pools, baths, saunas and hair removal by wax or razor in the area until healing is complete, avoid sexual contact until the skin has healed and as your doctor advises, and use condoms afterwards, remembering they reduce transmission without eliminating it. Wear loose cotton clothing and avoid heavy sweating for several days. \*\*Seek immediate review for fever or chills\*\*, \*\*increasing pain and swelling with spreading redness, pus or foul smell\*\*, \*\*bleeding that does not stop with ten minutes of gentle pressure\*\*, \*\*inability to pass urine or severe pain on passing it\*\*, or \*\*extensive ulceration or skin turning dark or black\*\*. Also return, without undue alarm, if new growths appear, since early treatment is easier.
