## Procedure steps

### 1. Reviewing the order and operative report

The nurse starts by reading the physician's order and the operative report to establish the date of closure, the type and number of sutures or staples, and whether the order calls for complete removal or alternate removal that leaves every other stitch in place. The patient is asked about diabetes, immune disease, anticoagulants or steroid therapy, since these slow healing and can change the decision.

### 2. Assessing the wound before touching it

The wound is examined in good light: are the edges united, is there spreading redness, swelling, discharge or a small gap? If any sign suggests infection or incomplete healing, removal is stopped and the treating physician is contacted before proceeding. It is this pre-removal assessment, not speed, that prevents a wound from opening up.

### 3. Cleaning and aseptic preparation

Hands are cleaned and clean gloves applied, the wound surface is cleansed with a suitable antiseptic to lift crusts and debris caught around the threads, and sterile single-use instruments are prepared: forceps with a stitch cutter or a staple extractor. Household implements and instruments reused without proper sterilisation are never acceptable.

### 4. Removing the sutures one by one

Each stitch is lifted slightly with forceps and cut at skin level next to the knot, then the thread is drawn out gently in one direction so that the part which lay outside the skin is not dragged through the wound tissue. The number of sutures removed is counted and matched against the recorded number, so that no thread is left forgotten under a crust.

### 5. Covering, documenting and follow-up instructions

The area is cleaned again and covered with a light dressing or supportive adhesive strips if the physician has asked for them, particularly over sites exposed to tension. The procedure, the state of the wound and the suture count are then documented, and the patient receives written instructions on care, restricted activities, warning signs and the follow-up appointment.

## About this service

Suture removal is a straightforward nursing procedure in which surgical thread is taken out of the skin once the wound edges have knitted together well enough to hold without external support. Stitches are not a treatment in themselves; they are a temporary scaffold that holds the edges close while the body rebuilds its own tissue. That is why the timing of removal is a clinical decision made by the surgeon or the clinician who closed the wound, based on the site of the wound, the quality of its blood supply and the patient's overall condition.

Typical timing varies by region of the body. Stitches on the face and neck are usually removed earlier, because blood supply there is rich and the cosmetic result matters; the back, the limbs and areas near joints need longer because they are under greater tension. Removing sutures too early may allow the wound to open, while leaving them in far too long can imprint visible suture marks and provoke inflammation around the thread.

Not every stitch is meant to be removed. Absorbable sutures dissolve on their own and need no removal, and some wounds are closed with adhesive strips or skin staples that require their own dedicated removal instrument. For that reason the first step for any provider is to establish the type of closure from the operative report rather than guessing by eye.

When the service is requested at home, it is delivered \*\*on a physician's order with a written care plan\*\* that states the appointment, the number of sutures and what to do if something looks wrong, not on the request of the patient or family alone. A licensed nurse carries out that order, assesses, documents and escalates, but \*\*does not diagnose and does not change any medication dose\*\*, and does not independently decide to bring the removal forward or delay it. \*\*Always verify the licence of both the nurse and the centre or company employing them\*\*, because Jordan's home-care market includes unqualified providers, and cutting sutures with unsterile or household scissors is a well-known cause of wound infection.

## Preparation

Tell the nurse or physician the date and place of your operation and bring the operative report or discharge sheet if you have it, because these define the type of suture and the intended removal date. Also mention diabetes or any immune weakness, blood-thinning medicines or steroids, and any allergy to antiseptics or medical adhesive. Do not soak the dressing or the wound in water before the appointment, do not apply ointments or household substances over the threads, and do not pick off crusts. \*\*Never cut the stitches yourself and never use household scissors\*\*; removal with an unsterile instrument or at the wrong time is a common cause of wound infection and wound breakdown. Wear loose clothing that exposes the wound easily, and if the wound is on the scalp avoid dye or styling products beforehand. If spreading redness, discharge or fever appears before the appointment, contact the physician rather than waiting for the visit.

## Aftercare

Keep the site clean and dry for the first day, then wash it gently with water and mild soap and pat it dry rather than rubbing. Avoid long baths, swimming pools and the sea until your physician allows them. Avoid heavy lifting and exercise that pulls on the area for the period your physician specifies, because a wound regains its strength gradually over weeks, not in a day. Once the skin is fully closed, protect the scar from the sun with clothing or sunscreen, which reduces darkening of the mark. Mild itching and numbness around the wound are common and settle over time. Seek care immediately if you notice: \*\*the wound edges separating or fluid or pus draining\*\*, \*\*spreading redness or hot, tender swelling\*\*, \*\*high fever or chills\*\*, \*\*bleeding that does not stop with light pressure\*\*, \*\*pain that increases after having improved\*\*. In home care, \*\*a change in level of consciousness, difficulty breathing, high fever, or bleeding from a catheter or tube site means urgent contact or emergency care, not waiting for the next visit\*\*.
