حدود الإجراء: لا يوجد دواء ولا كريم يُذيب الكيس، والكمّادات الدافئة تخفّف الالتهاب ولا تُزيل الكيس. وكل استئصال يترك أثراً بطول الشقّ، وقد يكون بارزاً عند أصحاب الاستعداد للجدرة أو في الصدر والكتف والظهر. والإزالة لا تمنع ظهور أكياس أخرى في مواضع مختلفة، وبعض الناس لديهم استعداد لتكرارها. \*\*والكتلة التي تكبر بسرعة، أو تلتصق بالجلد وتُثبّته، أو تتقرّح وتنزف، أو ترافقها قسوة غير معتادة أو تضخّم في العقد اللمفية المجاورة، ليست كيساً بالضرورة؛ ويجب فحصها وإرسال النسيج المستأصل إلى المختبر لاستبعاد سرطان الجلد والأورام الأخرى.\*\*

وتصحيحاً لعادة شائعة: فتح الكيس بإبرة أو شفرة في المنزل أو عصره بقوّة يسبّب التهاباً وخرّاجاً وتليّفاً يجعل الاستئصال لاحقاً أصعب وأثره أكبر، وقد ينشر العدوى في مناطق خطرة كوسط الوجه. والترشيح لموعد الجراحة وطريقتها يحدّده الفحص السريري لا حجم الكتلة في الصورة.

## خطوات الإجراء

### 1. الفحص وتمييز الكيس عن غيره

يجسّ الطبيب الكتلة ليحدّد حجمها وعمقها وحركتها وعلاقتها بالجلد، ويبحث عن الفتحة المركزية المميّزة للكيس البشراني، ويميّزها عن الورم الشحمي والعقدة اللمفية والخرّاج وأورام الجلد. وقد يطلب صورة موجات فوق صوتية إن كانت الكتلة عميقة أو غير واضحة.

### 2. تحديد التوقيت المناسب للإجراء

إن كان الكيس هادئاً يُحدَّد موعد الاستئصال الكامل مباشرة. وإن كان محمرّاً مؤلماً أو صار خرّاجاً فتُجرى أولاً خطوة تصريف وتهدئة، ويُؤجَّل الاستئصال أسابيع حتى يهدأ الالتهاب، لأن ذلك يقلّل النكس ويُصغّر الأثر ويجعل العمل أدقّ.

### 3. التخدير الموضعي والاستئصال الكامل للجدار

يُنظَّف الجلد ويُخدَّر موضعياً، ثم يُفتح شقّ صغير فوق الكيس ويُفصَل الجدار عن النسيج المحيط ويُستأصل كاملاً مع محتواه بأقلّ تمزّق ممكن. وإن تمزّق الجدار يُنقّى الجيب من بقاياه بعناية، لأن أي جزء متبقٍّ من الجدار قد يعيد تكوّن الكيس.

### 4. الإغلاق وإرسال النسيج للمختبر

يُوقَف النزف ويُغلق الجرح بخياطة مناسبة لموضعه، وقد يُترك تصريف صغير في الأكياس الكبيرة أو الملتهبة. ويُرسَل النسيج المستأصل إلى المختبر للتأكيد، فهذه الخطوة تكشف الحالات التي تبدو كيساً بسيطاً وهي ليست كذلك.

### 5. العناية بالجرح والمتابعة

تُشرح لك طريقة تنظيف الجرح وتغطيته وموعد نزع الخيوط، وما يجب تجنّبه من رياضة وسباحة وضغط على المنطقة. وتُراجَع الحالة لتقييم الشفاء والأثر، وتُبلَّغ نتيجة المختبر، ويُشرح لك ما يجب مراقبته لاكتشاف النكس مبكراً إن حدث.

## Procedure steps

### 1. Examination and distinguishing the cyst

The doctor palpates the lump to judge its size, depth, mobility and attachment to the skin, looks for the central punctum typical of an epidermoid cyst, and distinguishes it from a lipoma, a lymph node, an abscess and skin tumours. An ultrasound scan may be requested if the lump is deep or its nature is unclear.

### 2. Choosing the right timing

If the cyst is quiescent, a date for complete excision is set straight away. If it is red and painful or has become an abscess, drainage and settling come first and excision is deferred for some weeks until the inflammation subsides, because waiting lowers the recurrence rate, keeps the scar smaller and makes the surgery more precise.

### 3. Local anaesthetic and complete wall excision

The skin is cleaned and numbed, a small incision is made over the cyst, and the wall is separated from the surrounding tissue and removed intact with its contents, with as little rupture as possible. If the wall does tear, the cavity is carefully cleared of remnants, because any fragment left behind can reform the cyst.

### 4. Closure and laboratory examination

Bleeding is controlled and the wound closed with sutures suited to the site; a small drain is occasionally left in large or previously infected cysts. The excised tissue is sent to the laboratory for confirmation — a step that catches the occasional lesion which looked like a simple cyst but was not.

### 5. Wound care and follow-up

You are shown how to clean and dress the wound, when the stitches come out, and what to avoid — exercise, swimming and pressure on the area. A review appointment assesses healing and the scar, the laboratory result is reported, and you are told what to watch for so that a recurrence, if it happens, is picked up early.

## التحضير قبل الإجراء

أخبر الطبيب بمدّة وجود الكتلة وسرعة تغيّر حجمها وعدد المرّات التي التهبت فيها أو فُتحت سابقاً، فالكيس الذي التهب مراراً يكون جداره متليّفاً وأصعب في الاستئصال. اذكر كل أدويتك، وخصوصاً مضادات التخثّر ومضادات الصفيحات والكورتيزون والأدوية المثبِّطة للمناعة، ولا توقف شيئاً من نفسك. اذكر السكري وأمراض القلب وصمّامات القلب الصناعية وحساسيتك للمخدّر الموضعي أو المطهّر أو اللاصق، وأي ميل لتكوّن جدرة. اذكري الحمل أو الرضاعة. لا تعصر الكيس ولا تفتحه ولا تضع عليه مادة كاوية قبل الموعد. إن كان الكيس في فروة الرأس فاغسل شعرك قبل الموعد ولا تستخدم زيوتاً أو مثبّتات، والبس ملابس واسعة تُسهّل الوصول للمنطقة، ورتّب من يوصلك إن كان الاستئصال واسعاً.

## العناية بعد الإجراء

أبقِ الجرح نظيفاً وجافاً وبدّل الغيار بحسب التعليمات، ولا تنزع الخيوط ولا القشرة بنفسك، ولا تضع مراهم غير موصوفة. تجنّب السباحة والحمّامات العامة والرياضة العنيفة ورفع الأثقال وشدّ الجلد حتى يأذن الطبيب، وامتنع عن الضغط على موضع الجرح أثناء النوم إن أمكن. واحمِ الأثر من الشمس عدّة أشهر لتقليل التصبّغ. \*\*راجع فوراً عند: ألم نابض متزايد بعد اليوم الثاني أو الثالث، أو احمرار يتوسّع مع سخونة موضعية، أو تصريف قيحي أو رائحة كريهة، أو حرارة وقشعريرة، أو تورّم متزايد وتصلّب حول الجرح، أو نزف لا يتوقّف بالضغط المتواصل، أو تباعد حواف الجرح.\*\* وراجع طبيبك إذا عادت كتلة في الموضع نفسه بعد أسابيع أو أشهر — فهذا يعني بقاء جزء من الجدار ويحتاج تقييماً. \*\*واسأل عن نتيجة الفحص النسيجي ولا تفترض أنها سليمة.\*\*

## About this service

Sebaceous cyst is the everyday name for a benign lump under the skin lined by a wall of skin cells and filled with a characteristically odorous keratin and oil material; epidermoid and pilar cysts are the commonest types. They appear most often on the scalp, face, neck, back and behind the ear. A cyst is not a dangerous tumour, but it grows slowly and may become inflamed or rupture under the skin, turning red, hot and painful.

Removal is a minor surgical procedure under local anaesthetic, \*\*and its central rule is that the entire cyst wall must be excised along with its contents. Emptying, squeezing or aspirating the contents shrinks the lump temporarily and the cyst then returns, because the wall that produces the material is still in place.\*\* Excision is performed through a small incision or a small circle of skin over the cyst, keeping the wall intact and unruptured where possible to reduce recurrence, and the wound is then closed with stitches and dressed.

Timing matters. If the cyst is inflamed or has turned into a painful abscess, the first step is usually to drain and settle the infection, and definitive excision is postponed for several weeks until the tissue calms down. Operating at the peak of inflammation makes the wall more likely to tear, raises the recurrence rate, widens the resulting scar and blurs the boundaries the surgeon needs to follow.

The limits are clear. No medicine or cream dissolves a cyst, and warm compresses ease inflammation without removing anything. Every excision leaves a mark the length of the incision, which can be raised in people prone to keloid or on the chest, shoulder and back. Removal does not prevent new cysts elsewhere, and some people are simply prone to them. \*\*A lump that grows quickly, becomes tethered to the skin, ulcerates or bleeds, feels unusually hard, or comes with enlarged nearby lymph nodes, is not necessarily a cyst: it must be assessed and the excised tissue sent to the laboratory to rule out skin cancer and other tumours.\*\*

One common habit needs correcting: opening a cyst with a needle or blade at home, or squeezing it hard, causes infection, abscess formation and scarring that make later excision harder and its scar larger, and can spread infection in risky areas such as the central face. Eligibility and timing are decided by clinical examination, not by how big the lump looks in a photograph.

## Preparation

Tell the doctor how long the lump has been there, how fast it has changed in size, and how many times it has become inflamed or been opened before — a cyst that has flared repeatedly has a fibrous wall and is harder to remove cleanly. List all your medicines, especially anticoagulants, antiplatelet agents, steroids and immunosuppressants, and stop nothing on your own. Mention diabetes, heart disease, prosthetic heart valves, allergy to local anaesthetic, antiseptics or dressings, and any tendency to keloid. Women should mention pregnancy or breastfeeding. Do not squeeze or open the cyst, and do not apply caustic substances before your appointment. If the cyst is on the scalp, wash your hair beforehand and use no oils or styling products; wear loose clothing that gives access to the area, and arrange a lift home if a wider excision is planned.

## Aftercare

Keep the wound clean and dry and change the dressing as instructed; never remove stitches or crusts yourself and do not apply unprescribed ointments. Avoid swimming, public baths, strenuous exercise, heavy lifting and anything that stretches the skin until your doctor agrees, and try not to sleep pressing on the wound. Protect the scar from the sun for several months to limit pigmentation. \*\*Seek review immediately if: throbbing pain increases after the second or third day; redness spreads with local heat; there is pus or a foul smell; you develop fever or chills; swelling and hardness around the wound increase; bleeding does not stop with sustained pressure; or the wound edges separate.\*\* Also see your doctor if a lump returns at the same site after weeks or months, since this means part of the wall remained and needs assessment. \*\*Ask for the histopathology result and do not assume it was normal.\*\*
