## خطوات الإجراء

### 1. تقييم الندبة وتحديد نوعها وتوقيتها

يفحص الطبيب الندبة من حيث العرض والارتفاع واللون والمرونة واتجاهها بالنسبة لخطوط الجلد، ويسأل عن عمرها وعن تاريخ الجدرة الشخصي والعائلي وطريقة شفاء جروحك السابقة، ثم يقرّر إن كان التصحيح الآن مناسباً أو الانتظار أفضل.

### 2. اختيار التقنية وتخطيط الاتجاه

تُرسم خطّة الإجراء على الجلد قبل التخدير: استئصال بسيط، أو إعادة توجيه بتقنية هندسية لتفكيك الشدّ، أو استئصال متسلسل، أو إضافة حجم للندبة المنخفضة، مع شرح الشكل المتوقّع للندبة الجديدة وطولها المحتمل الذي قد يزيد على الأصلي.

### 3. الاستئصال وإعادة الخياطة بطبقات

يُجرى الإجراء عادةً بتخدير موضعي؛ تُستأصل الندبة أو يُعاد تشكيلها، ويُخفَّف الشدّ عن حواف الجرح، ثم يُغلق بطبقات مع خياطة تجميلية دقيقة تحت الجلد لتقليل توسّع الندبة لاحقاً. الحالات الواسعة قد تحتاج تخديراً عاماً.

### 4. البروتوكول المساعد بعد الخياطة

### 1. Assessing the scar, its type and the right timing

Your doctor examines the width, height, colour, pliability and orientation of the scar relative to the natural skin lines, asks about its age, any personal or family history of keloid and how your previous wounds healed, then decides whether revision now is appropriate or waiting is wiser.

### 2. Choosing the technique and planning the direction

The plan is drawn on the skin before anaesthesia: simple excision, geometric redirection to break tension, serial excision, or adding volume beneath a depressed scar, with a clear explanation of the shape of the new scar and the fact that it may be longer than the original.

### 3. Excision and layered closure

The procedure is usually done under local anaesthetic: the scar is excised or reshaped, tension is released from the wound edges, and closure is performed in layers with fine subcuticular suturing to reduce later widening. Extensive cases may require general anaesthesia.

### 4. The adjunct protocol after closure

After suture removal or as your doctor instructs, scar-improving measures begin: silicone sheets or gel, pressure, massage and sun protection, with intralesional injection or other measures added by your doctor for hypertrophic scars and keloids. Consistency at this stage makes a genuine difference to the final appearance.

### 5. Long-term follow-up and judging the result

The scar is followed for months because its colour and texture change slowly, and a touch-up session or laser treatment may be needed for colour match. The final result is usually judged after six months to a year and never in the first weeks, when redness is at its peak.

## About this service

Surgical scar removal, more accurately called scar revision, is a group of surgical and adjunct techniques that reshape a troublesome scar or reposition it so that it becomes less noticeable and functionally acceptable. It is used for scars that are widened, depressed, raised or hardened, for scars that tether a joint, an eyelid or the corner of the mouth, and for scars that run against the natural lines of the skin.

One fact must come first: no procedure removes a scar completely. Every full-thickness skin wound heals with a scar, and the aim of revision is to convert a poor scar into a better one that is finer, closer in colour, hidden along a natural line and free of tension. Patients who expect total disappearance are disappointed, while those who understand the real goal are far more satisfied with the outcome.

The technique is chosen according to the type of scar: excision with careful layered closure and fine cosmetic suturing; redirecting the scar with geometric techniques such as a zigzag closure to break up tension; serial excision for wide scars; autologous fat grafting to lift a depressed scar; and laser or dermabrasion to improve texture and colour. Keloids and hypertrophic scars usually need an adjunct protocol with silicone, pressure therapy and intralesional injection, and patients must know that keloids are well recognised for their tendency to recur after excision alone.

Timing matters. It is usually best to wait until the scar has matured and its redness has settled, which typically takes six months to a year or longer, unless the scar restricts movement or distorts a structure, in which case earlier surgery is planned. Whether you are a candidate, and which technique suits you, are determined by clinical examination and your doctor's assessment of your skin type and healing history, not by result photographs online.

## Preparation

Tell your doctor if you or your family have a history of keloid or hypertrophic scarring, because this changes both the plan and the adjunct protocol. Report every medicine and supplement you take, especially blood thinners, antiplatelet drugs, steroids, immune-modifying treatment and supplements that may increase bleeding, and do not stop anything on your own. Also mention if you are using an oral acne treatment or have had radiotherapy to the area. Stop smoking and nicotine several weeks beforehand if possible, since it impairs healing and predisposes the wound edges to separating. Avoid sun exposure and tanning of the area before the procedure, bring photographs to document the current state, ask about correct timing if the scar is recent, and plan a day or two of rest if the site is over a mobile area.

## Aftercare

Keep the wound clean and dry as your doctor instructs and never remove adhesive strips or sutures yourself. Avoid any movement that stretches the wound, along with sport and heavy lifting, for about two to three weeks or as dictated by the site, and do not smoke while healing. Start silicone sheets or gel and massage on the date your doctor specifies rather than earlier, and continue them for weeks to months, since this is the part that makes the difference. Protect the scar from sun with high-factor sunscreen or clothing for six months to a year to limit pigmentation. Expect temporary redness and firmness that settle gradually. \*\*Seek care immediately if you develop fever, spreading redness with warmth, pus or odour, escalating pain, separation of the wound edges, continued bleeding, or rapid firm overgrowth of the scar beyond the original wound margins, which may be an early keloid needing prompt treatment.\*\*
