تحذير مهم: محاولة الإخراج في المنزل بعود قطن أو ملقط أو دبوس شعر تدفع الجسم أعمق نحو الطبلة وتجرح جلد القناة وتُصعّب الإجراء لاحقاً، وهي من أشيع أسباب تحويل حالة بسيطة إلى حالة تحتاج غرفة عمليات. ولا يُغسل الأذن بالماء إذا كان الجسم عضوياً كبذرة أو حبّة عدس لأنه ينتفخ ويزيد الانحصار، ولا إذا كان هناك شكّ في ثقب طبلة أو وجود أنبوب تهوية.

حدود الإجراء: الاستخراج يزيل السبب فحسب، ولا يعالج نقص سمع كان موجوداً قبل الحادثة، ولا يشفي التهاباً مزمناً في الأذن، ولا يُصلح أذى وقع على الطبلة أثناء محاولات الإخراج المنزلية. وقد يبقى احتقان أو ألم أو نقص سمع خفيف لأيام حتى يتعافى جلد القناة. كذلك لا يُنجَز كل جسم في العيادة؛ فالجسم العميق أو الملتصق أو الطفل الصغير غير المتعاون قد يستلزم إخراجاً تحت تخدير عام حفاظاً على سلامة الأذن.

طريقة الإخراج ووقته يُحدَّدان بالفحص السريري وبالتنظير المباشر للأذن، لا بوصف هاتفي ولا بصورة، لأن نوع الجسم وشكله وموقعه بالنسبة للطبلة هو ما يحكم الخطة كلها.

## خطوات الإجراء

### 1. الفحص بالمنظار وتحديد الجسم

يفحص الطبيب الأذن بمنظار أو مجهر ليحدّد نوع الجسم وعمقه وعلاقته بالطبلة، وهل هناك جرح أو نزف أو التهاب مصاحب. هذا الفحص هو ما يقرّر إن كان الإخراج ممكناً في العيادة أم يحتاج غرفة عمليات.

### 2. تهيئة المريض وتثبيت الرأس

يُشرح الإجراء ببساطة، ويُجلَس المريض أو يُستلقى بوضعية مريحة مع تثبيت لطيف للرأس. عند الأطفال يُثبَّت الطفل بمساعدة أحد الوالدين، لأن حركة مفاجئة واحدة قد تجرح القناة أو الطبلة.

### 3. اختيار الأداة والتسكين الموضعي عند الحاجة

تُنتقى الأداة بحسب شكل الجسم: الشفط للأجسام الملساء، الخطّاف الزاويّ لما يمكن تجاوزه، والكلاّبة للأجسام التي يمكن قبضها. وقد يُستخدم مخدّر موضعي أو مادة تُسكِّن حشرة حيّة قبل إخراجها، بقرار الطبيب.

### 4. الاستخراج تحت الرؤية المباشرة

يُخرَج الجسم بحركة واحدة محسوبة تحت الرؤية، مع تجنّب الدفع للعمق. إن تعذّر ذلك بعد محاولات محدودة أو ظهر نزف أو ألم كبير، يوقف الطبيب المحاولة ويحوّل الحالة إلى تخدير عام بدلاً من الإصرار.

### 5. إعادة فحص القناة والطبلة والتوصيات

بعد الإخراج تُفحَص القناة والطبلة للتأكّد من عدم بقاء أجزاء وعدم وجود ثقب أو خدش عميق. قد يوصي الطبيب بقطرات لفترة قصيرة إن كان الجلد متسحّجاً، ويُحدّد موعد مراجعة إن لزم تقييم السمع.

## Procedure steps

### 1. Otoscopic assessment

The ear is examined with an otoscope or microscope to establish the nature and depth of the object, its relation to the eardrum, and whether there is an associated laceration, bleeding or infection. This examination decides whether clinic removal is feasible or theatre is needed.

### 2. Preparation and head stabilisation

The procedure is explained in simple terms and the patient is seated or lying comfortably with the head gently stabilised. With children a parent helps to hold the child still, since a single sudden movement can injure the canal or the eardrum.

### 3. Choosing the instrument

The instrument is matched to the object: suction for smooth objects, an angled hook for objects that can be passed, and forceps for graspable objects. Local anaesthetic may be used, or a live insect immobilised before retrieval, at the clinician's discretion.

### 4. Extraction under direct vision

The object is retrieved in one controlled movement under vision, avoiding any push deeper. If it cannot be freed after a limited number of attempts, or if bleeding or significant pain occurs, the clinician stops and arranges removal under general anaesthesia rather than persisting.

### 5. Re-examination and advice

After removal the canal and eardrum are re-inspected to confirm no fragments remain and there is no perforation or deep abrasion. Short-course drops may be advised if the skin is grazed, and a follow-up visit is arranged if hearing needs to be assessed.

أخبر الطبيب بنوع الجسم قدر ما تعرف، ومتى دخل الأذن، وما جُرّب في المنزل، وهل حدث نزف أو ألم شديد أو نقص سمع أو دوار بعده. أخبره أيضاً إن كان لديك أو لدى طفلك جراحة أذن سابقة أو ثقب في الطبلة أو أنبوب تهوية أو التهاب أذن متكرّر، وإن كنت تستخدم مسيّلات للدم. لا تحاول الإخراج بعود قطن أو ملقط أو دبوس، ولا تقطّر ماءً أو زيتاً أو أي سائل في الأذن قبل الفحص، وامنع الطفل من العبث بأذنه. إن توفّر جسم مشابه للذي دخل فأحضره معك، فهو يساعد في اختيار الأداة. وإن أُخبرت باحتمال الحاجة إلى تخدير عام فالتزم بتعليمات الصيام التي تُعطى لك. البطارية الزرّية أو الجسم الحادّ أو الحشرة الحيّة أو النزف: توجّه فوراً بلا انتظار موعد.

## العناية بعد الإجراء

حافظ على جفاف الأذن بضعة أيام: لا سباحة، وتجنّب دخول الماء أثناء الاستحمام بحاجز قطني مدهون بالفازلين إن أوصى الطبيب بذلك. لا تُدخل أي شيء في الأذن بعد الإجراء، ولا تستخدم أعواد القطن مطلقاً. استخدم القطرات التي وُصفت لك بالمدّة التي حدّدها الطبيب فقط، وسكّن الألم بما اتُّفق عليه معه. من الطبيعي أن تشعر بامتلاء خفيف أو حساسية في القناة أو نقص سمع بسيط يتحسّن تدريجياً خلال أيام. راجع فوراً عند \*\*نزف مستمر من الأذن\*\*، أو \*\*ألم متزايد مع تورّم أو احمرار خلف الأذن أو حرارة\*\*، أو \*\*إفرازات صفراء كريهة\*\*، أو \*\*دوار شديد أو تقيّؤ\*\*، أو \*\*نقص سمع جديد أو مستمر\*\*، أو \*\*ضعف في حركة عضلات الوجه\*\*، أو \*\*طنين لم يكن موجوداً\*\*. وإن كان الجسم بطارية زرّية فالمراجعة المبكّرة ضرورية حتى بعد الإخراج لتقييم الجلد والطبلة.

## About this service

Removal of a foreign body from the ear is a procedure to extract an object lodged in the external ear canal. It is performed in an ENT clinic under direct light and magnification using purpose-made instruments: fine forceps, an angled hook, gentle suction, or irrigation in carefully selected cases. The aim is to retrieve the object with the least possible trauma to the canal skin and the eardrum.

In children the objects most often seen are beads, pieces of foam, seeds, broken cotton-bud tips and fragments of pencils or toys. In adults it is more often retained cotton, an insect that entered during sleep, or part of a hearing aid. Some situations are urgent and should not wait for a routine appointment: a button battery, because it causes rapid chemical injury to the skin; sharp objects; a live insect; and any object accompanied by bleeding, severe pain or sudden hearing loss.

An important warning: trying to remove the object at home with a cotton bud, tweezers or a hairpin pushes it deeper towards the eardrum, grazes the canal skin and makes later removal harder. This is one of the commonest reasons a simple case ends up needing a general anaesthetic. The ear should also not be irrigated when the object is organic, such as a seed or a lentil, because it swells and becomes more tightly wedged, nor when a perforated eardrum or a ventilation tube is suspected.

What the procedure does not do: extraction removes the cause only. It does not treat hearing loss that existed before the incident, it does not cure chronic ear disease, and it does not repair damage inflicted on the eardrum during home attempts. Mild blockage, discomfort or slightly reduced hearing may persist for a few days while the canal skin heals. Not every object can be dealt with in the clinic either; a deeply wedged or adherent object, or a small child who cannot cooperate, may need removal under general anaesthesia.

The method and the timing are decided by clinical examination and direct otoscopy, not by a phone description or a photograph, because the nature, shape and position of the object relative to the eardrum govern the whole plan.

## Preparation

Tell the doctor what the object is as far as you know, when it entered the ear, what has already been tried at home, and whether bleeding, severe pain, hearing loss or dizziness followed. Mention any previous ear surgery, a known perforation, a ventilation tube, recurrent ear infections, or blood-thinning medication. Do not attempt removal with a cotton bud, tweezers or a pin, and do not instil water, oil or any liquid before the ear has been examined; keep a child from poking at the ear. If you have a spare object of the same type, bring it, as it helps in choosing the instrument. If general anaesthesia is a possibility, follow the fasting instructions you are given. A button battery, a sharp object, a live insect or bleeding all warrant immediate attention rather than a routine appointment.

## Aftercare

Keep the ear dry for a few days: no swimming, and keep water out while showering, using a petroleum-jelly-coated cotton plug if your doctor advises it. Put nothing into the ear afterwards and never use cotton buds. Use any prescribed drops only for the duration specified, and manage pain as agreed with your clinician. Mild fullness, canal tenderness or slightly muffled hearing that improves over a few days is expected. Seek care immediately for persistent bleeding, increasing pain with swelling or redness behind the ear or fever, foul yellow discharge, severe dizziness or vomiting, new or persisting hearing loss, weakness of the facial muscles, or new tinnitus. If the object was a button battery, early review is needed even after successful removal so the canal skin and eardrum can be assessed.
