## Procedure steps

### 1. Assessment and a small test patch

The tattoo is assessed: its age, colours, ink density, site and number of layers, along with your skin type and any history of keloid or pigmentation problems. A small area is then treated as a test — essential for coloured cosmetic tattoos — and several weeks are allowed to judge the response before treating the whole tattoo.

### 2. Preparing the area, numbing and eye protection

The area is cleaned and a topical anaesthetic may be applied well before the session to reduce discomfort, with cooling used during treatment. Both the patient and the treating team wear goggles matched to the laser wavelength, because eye protection is a mandatory step that is never skipped.

### 3. The laser session

Laser pulses are passed over the tattoo; most people describe a sensation like the snap of a hot rubber band. The skin over the ink turns temporarily white — an expected endpoint that fades within minutes to hours — and swelling, redness, pinpoint bleeding or blisters may appear over the following day.

### 4. Aftercare and the interval between sessions

The site is covered with a prescribed ointment and a light dressing; blisters must not be opened, crusts must not be picked, and swimming, steam rooms and strenuous exercise are avoided for some days. Complete sun protection between sessions is stressed, then at least six to eight weeks are allowed for the body to finish clearing the fragmented pigment.

### 5. Reviewing results and planning further sessions

The area is photographed before each session so fading can be compared objectively, and settings or wavelength are adjusted according to the remaining colour. Expectations are revisited if certain colours prove stubborn, and a decision is made about when the result has reached the best the case allows, at which point sessions stop.

## About this service

Tattoo removal is a cosmetic procedure in which pigment trapped in the dermis is broken into fine particles that the body gradually clears through the lymphatic system. The most widely used tool is a laser delivering ultra-short pulses tuned to tattoo pigments. The laser does not remove skin; it targets the pigment granules with minimal damage to surrounding tissue, which is why multiple sessions and adequate intervals between them are needed for the body to clear the fragmented ink.

\*\*The limits must be clear before the first session: removal is gradual and usually needs a number of sessions six to eight weeks apart or longer, and no method guarantees the skin will look exactly as it did before the tattoo. The realistic outcome is substantial fading, which may leave a faint shadow or a difference in texture or pigmentation.\*\* Not all inks behave alike: black and dark blue respond best, while green, light blue, yellow, white and earth tones respond less well and may not clear completely.

Cosmetic tattoos on the eyebrows, lips and eyelids deserve special mention. Some of these pigments contain metal oxides that can darken or change colour immediately after a laser pulse instead of fading, so a small test patch is treated first and assessed weeks later. Darker skin is more prone to hyperpigmentation or loss of pigment after sessions, and a tattoo covered by a newer one or drawn with dense ink is harder and needs more sessions. Surgical excision with stitches is an option for very small tattoos, but it exchanges the tattoo for a permanent linear scar.

Eligibility is decided by clinical examination. The assessment covers skin type, history of keloid and pigmentation problems, current medicines, any inflammation or allergic reaction in the tattoo, and any mole or growth within it. \*\*Moles lying inside a tattoo must be examined before treatment starts, because the laser alters their appearance and makes later monitoring difficult; and any sore, growth or change inside a tattoo that fails to heal needs assessment to rule out skin cancer.\*\*

Finally, some widespread and harmful practices: creams sold for tattoo removal, rubbing the area with salt or abrasives, and burning with caustic substances do not lift pigment out of the dermis. They cause wounds, infection and permanent scarring worse than the tattoo itself, and they make later laser removal harder and less effective.

## Preparation

Tell the doctor how old the tattoo is and what ink was used if you know, whether it covers an older tattoo, and whether it has ever itched, swollen or reacted. List all your medicines, especially anything that increases light sensitivity or bleeding, any immunosuppressive treatment, and any recent course of oral vitamin A derivatives, which normally postpones the session. Mention a history of keloid, pigmentation problems, or cold sores if the tattoo is near the lip. Women should mention pregnancy or breastfeeding, as treatment is usually deferred. Avoid tanning and sun exposure on the area for two weeks beforehand, do not shave the site closely the day before, and apply no exfoliating or perfumed creams to it. Arrive with the area clean and free of make-up and oils, and wear loose clothing that will not rub the site afterwards.

## Aftercare

Cool the area as the clinic advises, apply the prescribed ointment and cover it with a light clean dressing. Do not open blisters, pick crusts or rub the site — this is the commonest cause of permanent scarring and pigmentation. Avoid swimming, steam rooms, saunas and strenuous exercise for several days, and do not shave over the area until it has healed. Protect the site from the sun with a high-factor sunscreen or clothing until the next session, because sun-induced pigmentation defeats the treatment. \*\*Seek review immediately if: throbbing pain increases after the first two days; redness spreads with local heat or red streaks; there is pus or odour; you develop a fever; extensive blisters form; or an ulcer fails to heal. See the doctor as well for severe itching and swelling of the tattoo or a body-wide rash after a session, and for any growth, change or sore inside the tattoo, which calls for assessment to rule out skin cancer.\*\* Never bring the next session forward on your own: the interval is part of the treatment.
