## خطوات الإجراء

### 1. التقييم السريري وتحليل المشية

يبدأ التقييم بسؤالك عن موضع الألم وتوقيته ومسافة المشي التي تحتملها وطبيعة عملك وساعات وقوفك، ثم يُفحص شكل القدم واقفاً وجالساً ومشية القدم الحافية، وتُقاس مرونة الكعب ووتر أخيل وتُفحص مواضع الجساءة والكالو. عند مريض السكري يُضاف فحص الإحساس والنبض، لأن نتيجته تُغيّر نوع النعل ومادته.

### 2. فحص حذائك الحالي

يُفحص الحذاء الذي تلبسه فعلاً لأن نمط تآكل النعل الخارجي يكشف كيف تحمل قدمك الوزن، ويُنظر إلى عمق الصندوق الأمامي وصلابة القاعدة وحالة البطانة الداخلية. كثير من الحالات تتحسّن بتغيير الحذاء وحده قبل التفكير في نعل مُخصّص، وأحياناً يكون الحذاء نفسه سبب الشكوى.

### 3. أخذ القياس أو القالب

إن كان النعل المُخصّص مناسباً، يُؤخذ شكل قدمك بطبعة رغوية أو قالب جبسي أو مسح رقمي، مع تحديد أي منطقة يجب تفريغها وأي منطقة يجب دعمها. يستغرق ذلك دقائق وهو غير مؤلم، ويُوثّق معه وزنك ومستوى نشاطك لأنهما يُحدّدان صلابة المادة المستخدمة.

### 4. التصنيع والقياس الأول

يُصنع النعل أو يُختار الجاهز المناسب، ثم تُجرّبه داخل الحذاء نفسه الذي ستستخدمه لا حافياً، ويُلاحظ الطبيب مشيتك وأنت منتعله. تُعدّل الحواف وسماكة الدعامات في هذه الجلسة، وقد يحتاج الأمر أكثر من تعديل، ووجود ضغط موضعي مؤلم عند القياس علامة على أن التعديل ناقص لا على أنك تحتاج وقتاً للتأقلم.

### 5. التأقلم المتدرّج والمتابعة

تبدأ الاستخدام بساعة أو ساعتين في اليوم الأول ثم تزيد تدريجياً على مدى أيام إلى أسبوعين. تُحدَّد مراجعة لتقييم الاستجابة وتعديل ما يلزم، ثم مراجعات دورية لأن مواد النعل تفقد مرونتها مع الاستخدام والوزن فتحتاج استبدالاً، وتُقصر فترات المراجعة كثيراً عند مريض السكري أو صاحب التشوّه المتقدّم.

## Procedure steps

### 1. Clinical assessment and gait analysis

Assessment starts with questions about where the pain is, when it appears, how far you can walk, and what your work and standing hours are. The clinician then examines the shape of the foot sitting and standing, watches you walk barefoot, tests heel and Achilles flexibility, and inspects callus and pressure points. For a patient with diabetes, sensation and pulses are also tested, because the result changes both the design and the material of the device.

### 2. Inspecting your current shoes

Your actual everyday shoes are examined, because the wear pattern on the outsole shows how you load the foot. The depth of the toe box, the stiffness of the sole and the state of the internal lining are all checked. Many complaints improve with a change of footwear alone before any custom device is considered, and sometimes the shoe itself is the cause of the problem.

### 3. Taking the measurement or mould

If a custom device is appropriate, the shape of your foot is captured with a foam impression, a plaster mould or a digital scan, while the clinician marks which zones must be offloaded and which must be supported. This takes only minutes and is painless. Your weight and activity level are recorded as well, because they determine how firm the material needs to be.

### 4. Manufacture and first fitting

The device is manufactured, or a suitable prefabricated one is selected, and then tried inside the very shoe you intend to wear rather than barefoot, while the clinician watches you walk. Edges and post thickness are adjusted in this session, and more than one adjustment may be needed. A sharply painful pressure point at fitting means the device needs modification, not that you simply need time to get used to it.

### 5. Gradual break-in and follow-up

You start with one or two hours on the first day and increase gradually over several days to two weeks. A review visit is scheduled to judge the response and fine-tune the device, followed by periodic checks, because insole materials lose their resilience with use and body weight and eventually need replacement. Review intervals are much shorter for people with diabetes or with advanced deformity.

## About this service

Orthopedic insoles and prescription footwear are supportive devices chosen or manufactured to change how body weight is distributed across the sole of the foot. They shift pressure away from a painful spot or away from an area at risk of ulceration, support the arch, and can correct a mild heel tilt. An insole may be a prefabricated device in standard sizes, or a custom orthosis made over a mould taken from your own foot. Sometimes the shoe itself is the treatment, prescribed with specific features: a wide, deep toe box, a stiff sole that does not bend in the middle, a low heel, and internal padding that lines the areas where friction occurs.

These devices are used for plantar fasciitis and heel pain, painful flat feet, forefoot and metatarsal pain, hammer toes, hallux valgus, arthritis of the foot and ankle joints, minor leg length differences, and during recovery after fractures or foot surgery. Their single most important use is in the diabetic foot, where the goal is offloading: taking pressure off high-pressure zones to prevent an ulcer or to allow an existing one to heal.

The limits of insoles must be stated plainly. An insole does not correct a bony deformity and does not rebuild a collapsed arch. It will not straighten a bunion or a fixed hammer toe, and it does not replace stretching and strengthening exercises or weight reduction when those are needed. What it does is reduce pain and improve tolerance for standing and walking by redistributing load. Buying an insole online based on a description of your pain is not a substitute for examining the foot and observing how you walk.

In the diabetic foot in particular, the absence of pain is false reassurance. Nerve damage removes protective sensation, so an ulcer can form under an ill-fitting insole without the person ever complaining. Offloading is the neglected cornerstone of diabetic foot ulcer care, and any new shoe or insole for a person with diabetes requires daily foot inspection during the first weeks of use. Which device, which material and which shoe modification suit you is decided by clinical examination and gait assessment, not by reading.

## Preparation

Bring every pair of shoes you actually wear: work shoes, sports shoes and indoor shoes, even if they are worn out, because their condition is diagnostic information. Wear the socks you normally use, and book your visit at a time when your pain is at its usual level rather than after a long rest. Tell the clinician if you have diabetes, neuropathy, poor circulation or rheumatoid arthritis, whether you have had foot or ankle surgery or a previous fracture, and mention any recent change in your weight, pregnancy or activity level. Bring any previous X-rays or reports if you have them, and do not buy an over-the-counter insole before the assessment, because it may increase pressure in exactly the wrong place. Describe any treatment you have already tried and whether it helped or made the pain worse.

## Aftercare

Increase wearing time gradually and do not use a new insole for a whole day from the start. Expect a strange feeling or mild pulling in the arch or calf during the first two weeks. Inspect your feet after taking the device off each day of the break-in period, looking for redness that lasts more than half an hour, blisters or skin abrasion. \*\*See your doctor promptly if a wound, blister or ulcer appears, or if you develop new numbness or pain that increases instead of settling after two weeks.\*\* If you have diabetes, examine the soles of both feet with a mirror every day, and \*\*any wound, black spot, swelling, odour or discharge on a diabetic foot needs to be seen the same day and must never wait for a routine appointment\*\*. Do not modify the device yourself by cutting or heating it, never share insoles with another person, and replace them when the material flattens or your pain returns.
