## Procedure steps

### 1. History and full background

The mother is asked about pregnancy, the type of delivery, when the first feed happened and whether mother and baby were separated, about her medicines and medical conditions including thyroid disease, polycystic ovary syndrome and postpartum bleeding, about previous breast surgery, her sleep and family support, and about what she has already tried and how it worked.

### 2. Observing a complete feed

A real feed is watched from the beginning: early hunger cues, how the baby is brought to the breast, how wide the mouth opens and how the lips sit, the rhythm of sucking and whether swallowing can be heard, the length of the feed and the baby's behaviour afterwards. Direct observation reveals what no verbal description of the problem can.

### 3. Examining mother and baby and weighing

The breast and nipple are examined for cracks, engorgement, lumps or infection, the baby's mouth is checked to assess the tongue, its movement and the palate, and the baby is weighed on an accurate scale and compared with the growth chart and birth weight, together with a count of daily wet and dirty nappies.

### 4. A practical plan for positioning and emptying

The mother is coached hands-on in one or two positions that suit her body and her baby, in achieving and releasing a correct latch without pain, in feeding rhythm and in emptying the breast by hand or pump when needed, and in nipple care. If supplementary feeding is required, its amount, method and a plan for reducing it later are explained.

### 5. Close follow-up with weight and plan review

A near-term appointment is set to re-weigh the baby and review what was and was not achievable, and the plan is adjusted according to results rather than hopes. The baby is referred to the paediatrician, or the mother to her doctor, when there is jaundice, poor weight gain, mastitis or any symptom needing medical treatment.

## About this service

A lactation consultation is a practical assessment in which a full feed is observed from start to finish, and the baby's latch, positioning, sucking strength and swallowing are evaluated. The baby's mouth is examined for tongue-tie or a palate problem, the mother's breasts are examined, and the baby is weighed with a review of the growth chart and the number of wet nappies. The session ends with a written, workable plan: specific positions, a feeding rhythm, a method for emptying the breast, and dates to re-check weight.

The first principle of this consultation is practical support without blame. The mother's decision is respected whatever it is, whether exclusive breastfeeding, mixed feeding or formula, and the goal is a well-fed baby and a mother in good physical and mental health, not passing a test. Guilt does not increase milk supply; accurate information and hands-on support are what change outcomes. A mother who chooses to stop deserves safe guidance on how to wean and how to feed her baby just as much as one who chooses to continue.

Certain warning signs require immediate review. In the baby: weight below expected limits or failure to regain birth weight within the usual period, few wet nappies, lethargy and refusal to feed, a weak cry with a dry mouth, or deepening jaundice. In the mother: severe nipple pain or bleeding cracks, engorgement with fever, chills and exhaustion, or a red painful lump. These need assessment, not endurance, because severe pain is not a normal part of breastfeeding but a sign of an incorrect latch or of infection.

The limits should be stated plainly. Consultation cannot guarantee sufficient milk in every case: some mothers have genuinely insufficient glandular tissue, previous breast surgery, hormonal disorders, or a long separation from the baby, and in those situations the aim is the best possible breastfeeding alongside safe supplementary nutrition rather than blame. It does not replace the paediatrician when a newborn is unwell, no medicines or doses are prescribed here, and herbal galactagogues are not a substitute for frequent effective emptying and a correct latch; some are not safe during breastfeeding and should never be taken without advice.

## Preparation

Come at a time when your baby is ready to feed rather than asleep after a large feed, because observing a real feed is the most important part of the session, so delay a feed slightly before the appointment if you can. Bring the baby's health record, the birth report and any documentation of jaundice or special-care admission, and write down the number and length of feeds and the number of wet and dirty nappies over the last two days. Bring the pump, bottles, teats and nipple shields you actually use so they can be assessed as they are. Write down your medicines, supplements and any herbal products, your chronic conditions, and details of any previous breast surgery. Wear clothing that makes feeding easy, bring a support person who can hear the plan with you, and write down your questions and worries honestly and without embarrassment.

## Aftercare

Use the position you were taught and feed according to your baby's hunger cues rather than the clock, empty the breast well at each feed because frequent effective emptying is the real driver of supply, and use the pump as your plan specifies. Care for the nipple with gentle drying and a few drops of your own milk, and release the latch gently rather than pulling. Drink to thirst, eat adequate meals, sleep when your baby sleeps and accept help with housework. Keep a record of nappies and weights so they can be compared at the next visit. Seek care immediately if your baby becomes lethargic or refuses to feed, has few wet nappies, loses weight or fails to gain, or becomes more jaundiced, or if you develop fever with chills and a red painful lump in the breast, a bleeding crack that is not improving, or overwhelming sadness or thoughts of harm. Do not measure your worth by millilitres; whatever you decide is respected.
