The plan is shaped by the type and duration of diabetes, daily glucose readings and HbA1c, the exact medicines the patient takes and when they are taken, kidney function, lipids and blood pressure, weight and waist measurement, work shifts, sleep and physical activity, and the household food habits and budget that decide what is actually sustainable. That is why this page contains no portion sizes, no calorie targets and no sample menu. Those numbers belong to the individual patient; a menu published for everyone is either useless or unsafe for someone reading it. Nutrition genuinely changes outcomes in diabetes, but its limits must be stated plainly. **Eating well does not replace medication, does not remove the need for insulin, and does not cure type 1 diabetes**, and there is no single diet that suits every person with diabetes. What a good plan does is distribute carbohydrate across the day so that it matches the medication and the working schedule, reduce sharp swings both upward and downward, protect the kidneys and arteries, preserve muscle mass in patients who need to lose weight, and stay livable for years rather than weeks. The visit is also where common myths are corrected. Foods labelled sugar-free can still raise glucose because of their starch content, fruit is not universally forbidden, fresh juice is not a safe substitute for whole fruit, and brown bread is not a licence for unlimited quantities. Any major change in carbohydrate intake for a patient on insulin, or on medicines that stimulate insulin release, requires the physician to review the doses, because cutting carbohydrate without adjusting treatment can cause hypoglycaemia. Dietitian and doctor work together, not separately. Be sceptical of what is marketed outside the clinic: severe elimination diets without supervision, ketogenic eating or prolonged fasting in diabetes or kidney disease without a medical decision and monitoring, unregulated fat-burner supplements and herbal diabetes cures that may contain undeclared substances, and slimming injections promoted online without diagnosis, prescription or follow-up. Eligibility for any plan, its shape and how quickly it is adjusted are decided by clinical assessment and laboratory results, never by reading a web page. ## Preparation Bring a list of every medicine and supplement you take, with doses and timing exactly as written on the packaging, and your most recent laboratory results: HbA1c, kidney function and urine protein, lipids, and vitamin D if available. Record your glucose readings for three to seven days before the appointment, noting the time of each reading and its relation to meals, and write down what you actually ate on two ordinary days and one weekend day, without editing it. Tell the dietitian about any hypoglycaemia episodes and when they happened, about kidney, heart or thyroid disease, about pregnancy, plans for pregnancy or breastfeeding, about any intention to fast, and about diets you have tried and why you stopped. Do not stop any medicine or change a dose before the visit, and do not fast beforehand unless a fasting blood test was requested. ## Aftercare Introduce the plan gradually and monitor your glucose more closely during the first two weeks, recording readings with their times so they can guide adjustment at follow-up. Repeated hypoglycaemia is a reason to review doses with your physician, not to cut food at random. **Seek medical attention immediately if you have: severe hypoglycaemia such as tremor, cold sweating, confusion or loss of consciousness, or repeated low readings** · **dizziness or fainting while on the plan** · **unintentional weight loss, which is a sign of illness rather than success and must be investigated** · vomiting and abdominal pain with rapid breathing and an unusual breath odour · persistent high readings with severe thirst and heavy urination · swelling of the feet or reduced urine output.
