## Procedure steps

### 1. History taking and drawing the family tree

The counsellor asks about diseases in the family, ages at diagnosis and causes of death, about miscarriages and child deaths, about whether and how closely the couple are related, and about medicines and exposures in pregnancy. Accounts are documented with reports wherever possible, because risk estimation depends on a confirmed diagnosis rather than a general description.

### 2. Estimating probability and explaining inheritance

The expected pattern of inheritance is explained with simple diagrams, and the chance of being affected or of carrying the gene is estimated for each pregnancy separately, with the clarification that this is a fixed probability unaffected by the outcome of a previous pregnancy. What remains unknown in the case, and the limits of the estimate's accuracy, are stated openly.

### 3. Presenting testing options and their limits

Options are laid out neutrally: carrier testing for both partners, non-invasive and invasive prenatal tests with their risks and timing windows, preimplantation testing, and simply continuing with follow-up. For each, what it detects and what stays outside its range is explained, along with the chance of an inconclusive result, and informed consent covering confidentiality is then signed.

### 4. Supporting the decision without directing it

The couple are given enough time and space to ask every question, a second session is arranged if needed, and a written summary of the figures and options is provided. The counsellor does not recommend a particular reproductive decision, and the couple are told to report any pressure from family or others, because the decision belongs to them alone.

### 5. Follow-up and coordinating relatives' testing

Once a result is available, a session explains it and arranges what follows: pregnancy monitoring, newborn care, or a surveillance programme. Cascade testing of relevant relatives is coordinated with their consent, and a future review is scheduled to reinterpret any uncertain variant as knowledge advances.

## About this service

Genetic counselling is a specialised medical consultation in which your medical and family history is analysed to estimate the chance that a genetic condition is present or may be passed to your children, and in which the available options are explained together with what each one means and where it stops. It is primarily an informational and supportive process, not an authority that issues a decision about marriage, conception, or continuing a pregnancy.

Counselling is sought before marriage, particularly between relatives, before or during pregnancy, when there is a family history of an inherited disease or a birth defect, when a child has developmental delay, a malformation or a metabolic disorder, after recurrent miscarriage or the death of fetuses or children, when a pregnancy screening result is abnormal, when there is a family history of cancer or inherited heart disease, and to explain a previous genetic test result whose meaning has confused the family.

During the session a three-generation family tree is drawn, reports and diagnostic images for affected relatives are reviewed, and the likely pattern of inheritance is explained: dominant, recessive, sex-linked or more complex patterns. All options are then presented neutrally: carrier testing for both partners, prenatal tests with their types, risks and timing windows, preimplantation genetic testing with assisted reproduction, follow-up without testing, and other ways of building a family. For each option the counsellor explains what it gives, what it cannot give, and what it involves in waiting time, cost or procedural risk.

Genetic counselling explains \*\*probabilities, not certainties\*\*. The chance is stated for each pregnancy separately; it does not fall because an earlier pregnancy was unaffected, a low probability does not mean impossible, and a high one does not mean inevitable. Any test result may also be \*\*inconclusive\*\*, and this is explained in advance so that it does not come as a shock. Because a genetic result concerns siblings, children and other relatives to whom it may also apply, \*\*its confidentiality is discussed before any testing\*\*: who receives it, which relatives might benefit from knowing, in what wording it would be shared, and the fact that nobody is told without your permission.

\*\*The decision remains yours and your partner's, and is made free of pressure from any quarter.\*\* The counsellor explains and does not recommend a particular reproductive decision, and you are entitled to take time, to decline testing, or to change your mind about a choice you have made. The limits are stated plainly: counselling does not prevent a disease from occurring, does not guarantee a healthy child, and does not detect every condition, since many malformations have no known genetic cause or are not genetic at all. Nor does it replace routine pregnancy care and care of the child after birth. The estimate is built on clinical assessment, the family tree and documented reports, not on reading or on unverified family accounts.

## Preparation

Before the appointment, gather three generations of information from your family and your partner's: known diseases and ages at diagnosis, causes of death and age at death, recurrent miscarriages and child deaths, and whether and how closely you are related. Bring reports for affected relatives and any previous genetic test result in full rather than in summary, with the permission of those concerned, and bring pregnancy, ultrasound and screening reports if you are pregnant, because some tests have timing windows that delay can close. Attend with your partner if the question is reproductive, since the decision is shared, and write down your questions and worries in advance. Decide beforehand who you want present and who you do not, because the information is sensitive and concerns other people, and avoid passing on relatives' information without their knowledge where possible.

## Aftercare

Ask for a written summary of the session stating the probabilities as figures and the options discussed, because memory is unreliable in an emotionally charged consultation. Do not make a decision in the session itself if you are not ready, discuss your choice freely with your partner, and ask for a clarification appointment if something was unclear or a new question has arisen. Discuss with the counsellor how and when to inform relatives and in what wording, and request a general family letter if you want the information shared without disclosing your own details. Ask for psychological support if you feel anxious or guilty, which is common and not a weakness. Keep your reports, and return after some years, because the interpretation of genetic variants changes as knowledge advances.

\*\*Seek care immediately if there is: bleeding, fluid leakage, abdominal pain, fever or cramping after a diagnostic procedure in pregnancy such as amniocentesis or chorionic villus sampling; a clear reduction in fetal movements; new symptoms in an affected child such as lethargy, refusal to feed, repeated vomiting or a seizure; or severe distress or thoughts of self-harm after receiving a result.\*\* And if anyone pressures you towards a reproductive decision, tell your team, because the decision is yours.
