الصورة النموذجية تبدأ بألم غامض حول السرّة ينتقل خلال ساعات إلى أسفل يمين البطن ويزداد مع الحركة والسعال، مع فقدان الشهية والغثيان وربما حرارة خفيفة. لكن الصورة قد تكون غير نمطية عند الأطفال والحوامل وكبار السنّ. التشخيص سريري بالأساس ويُدعم بتحاليل الدم والتصوير بالأمواج فوق الصوتية أو الطبقي المحوري، وقرار الجراحة يُحدَّد بالفحص السريري والمراقبة داخل المستشفى، لا بقراءة صفحة ولا بنتيجة تحليل واحدة.

تُجرى العملية في معظم الحالات بالمنظار عبر ثلاث فتحات صغيرة: يُفصل مجرى الدم الصغير المغذّي للزائدة، وتُغلق قاعدتها، ثم تُخرَج داخل كيس. وقد يقرّر الجراح تحويل العملية إلى شقّ صغير مفتوح إذا كانت الزائدة منثقبة أو الالتصاقات كثيفة أو الرؤية غير آمنة، وهذا قرار سلامة لا يُعدّ فشلاً. التخدير عام في العادة.

في حالات مختارة من الالتهاب البسيط غير المعقّد يُطرح علاج بالمضادات الحيوية دون جراحة، لكنه لا يناسب كل مريض ويحمل احتمال عودة الالتهاب لاحقاً، والقرار يُتخذ داخل المستشفى بعد تقييم كامل ومتابعة قريبة.

حدود الإجراء: استئصال الزائدة يعالج التهاب الزائدة فقط. لا يعالج القولون العصبي، ولا آلام البطن المزمنة، ولا مشاكل المرارة أو المبيض، ولذلك لا تُستأصل الزائدة كحلّ لألم بطن غير مفسّر ولا وقائياً بلا التهاب. ومن الخرافات الخطيرة تناول مسكّن قويّ أو مليّن أو استخدام حقنة شرجية عند الشك بالتهاب الزائدة، لأن ذلك يخفي الأعراض ويزيد خطر الانثقاب: امتنع عن الأكل والشرب واتّجه إلى الطوارئ.

## خطوات الإجراء

### 1. التقييم العاجل والتشخيص

يفحص الجرّاح البطن ويسأل عن تسلسل الألم والغثيان والحرارة، ثم تُطلب تحاليل دم وتصوير بالأمواج فوق الصوتية أو الطبقي المحوري بحسب العمر والحالة. قد تُطلب مراقبة لساعات وإعادة فحص، لأن بعض الحالات تتوضّح بالوقت.

### 2. التحضير والتخدير

يُمنع الأكل والشرب، وتُعطى سوائل وريدية ومضاد حيوي وريدي قبل الجراحة ومسكّن مناسب. يقابلك طبيب التخدير لمراجعة أمراضك وأدويتك، ويُوقّع إقرار الموافقة بعد شرح الفائدة والمخاطر واحتمال التحويل إلى جراحة مفتوحة.

### 3. الدخول بالمنظار وتقييم البطن

بعد التخدير العام يُنفَخ البطن بغاز طبي وتُدخل الكاميرا عبر فتحة صغيرة قرب السرّة، ثم فتحتان عاملتان. يتفحّص الجرّاح الزائدة والأمعاء والحوض للتأكد من أن الالتهاب هو السبب، لأن حالات أخرى قد تُحاكي التهاب الزائدة.

### 4. استئصال الزائدة وتأمين القاعدة

تُفصل الزائدة عن التصاقاتها، ويُغلق شريانها الصغير، ثم تُغلق قاعدتها بمشابك أو خياطة أو دبّاسة وتُقطع وتُخرَج داخل كيس. إن وُجد قيح يُشفط ويُغسل الموضع، وقد يُترك أنبوب تصريف مؤقت عند الانثقاب.

### 5. الإفاقة والمراقبة والخروج

تُغلق الفتحات بخياطة بسيطة، وتستيقظ في غرفة الإفاقة ثم تنتقل للقسم. يُشجَّع النهوض والمشي في اليوم نفسه، وتبدأ السوائل ثم الطعام الخفيف بحسب تحمّلك. يُحدَّد موعد لمراجعة الجرح ومراجعة نتيجة الفحص النسيجي للزائدة.

## Procedure steps

### 1. Urgent assessment and diagnosis

The surgeon examines the abdomen and asks about the sequence of pain, nausea and fever, then blood tests and ultrasound or CT imaging are arranged according to age and circumstances. A few hours of observation with repeat examination may be advised, since some cases only declare themselves with time.

### 2. Preparation and anaesthesia

You are kept fasting and given intravenous fluids, a pre-operative intravenous antibiotic and suitable pain relief. The anaesthetist reviews your medical history and medicines, and consent is signed after the benefits, the risks and the possibility of conversion to open surgery have been explained.

### 3. Laparoscopic access and abdominal survey

After general anaesthesia the abdomen is insufflated with medical gas and a camera is inserted through a small incision near the navel, followed by two working ports. The surgeon inspects the appendix, the bowel and the pelvis to confirm that appendicitis is the cause, because other conditions can mimic it.

### 4. Removing the appendix and securing the base

The appendix is freed from its attachments, its small artery is sealed, then the base is closed with clips, a suture or a stapler, divided and removed inside a bag. If pus is present it is suctioned and the area irrigated, and a temporary drain may be left when the appendix has perforated.

### 5. Recovery, monitoring and discharge

The port sites are closed with simple sutures, you wake in the recovery room and move to the ward. Getting up and walking the same day is encouraged, and fluids then light food are started as tolerated. A follow-up appointment is arranged to check the wound and review the histology report of the appendix.

## التحضير قبل الإجراء

إن كان لديك ألم مستمر في أسفل يمين البطن مع غثيان أو حرارة فامتنع عن الأكل والشرب واتّجه إلى الطوارئ، ولا تأخذ مسكّناً قوياً ولا مليّناً ولا حقنة شرجية قبل الفحص لأنها تخفي العلامات. أخبر الطبيب بكل أدويتك، وخصوصاً مسيّلات الدم والأسبرين وأدوية السكري وأدوية المناعة والمكمّلات العشبية. أخبره بأي حساسية دوائية أو مشكلة سابقة مع التخدير، وبأمراضك المزمنة وعمليات البطن السابقة. أخبري الطبيب إن كان هناك احتمال حمل قبل أي تصوير. أزِل المجوهرات وطلاء الأظافر والعدسات اللاصقة، وأحضر تقاريرك وتحاليلك السابقة وبطاقة التأمين، ورتّب مَن يرافقك ويعيدك إلى البيت بعد الخروج.

## العناية بعد الإجراء

انهض وامشِ في اليوم نفسه فهذا يقلّل خطر التجلّط والالتصاقات، وابدأ بالسوائل ثم الطعام الخفيف بحسب تحمّلك. خُذ المسكّنات كما وُصفت لك، وأكمل المضاد الحيوي إن وُصف. حافظ على الجرح نظيفاً وجافاً، وتجنّب النقع في حوض أو مسبح حتى يسمح لك الطبيب. تجنّب رفع الأثقال والمجهود العنيف عادةً من أسبوعين إلى أربعة أسابيع بحسب نوع العملية وتوجيه جرّاحك. توقّع انتفاخاً بسيطاً وألماً في الكتف بعد المنظار وهو مؤقت. \*\*راجع الطوارئ فوراً عند: حرارة مرتفعة أو قشعريرة، ألم بطن يزداد بدل أن يخفّ، انتفاخ مع تقيّؤ متكرّر، توقّف خروج الريح والبراز، احمرار أو صديد أو انفتاح في الجرح، دوخة أو تسارع نبض، ضيق نفس أو ألم صدر، ألم وتورّم في الساق.\*\*

## About this service

An appendectomy is an operation to remove the appendix, a small blind-ended tube attached to the beginning of the large intestine in the lower right side of the abdomen, when it becomes inflamed and painful. Acute appendicitis is managed as an emergency, because continuing inflammation can perforate the appendix and spread infection through the abdominal cavity.

The classic picture begins as vague pain around the navel that shifts within hours to the lower right abdomen and worsens with movement or coughing, together with loss of appetite, nausea and sometimes a low-grade fever. The presentation can be atypical in children, in pregnancy and in older adults. Diagnosis is mainly clinical, supported by blood tests and ultrasound or CT imaging; the decision to operate is made through clinical examination and in-hospital observation, not by reading a web page or a single test result.

Most appendectomies are done laparoscopically through three small incisions: the small blood supply of the appendix is divided, its base is secured, and the specimen is removed inside a retrieval bag. The surgeon may convert to a small open incision when the appendix is perforated, when adhesions are dense, or when the view is not safe. That is a safety decision, not a failure. General anaesthesia is used in most cases.

In selected patients with simple, uncomplicated inflammation, antibiotic treatment without surgery may be discussed. It does not suit everyone, it carries a risk that the inflammation returns later, and the choice is made in hospital after a full assessment with close follow-up.

Limits of the procedure: an appendectomy treats appendicitis and nothing else. It does not treat irritable bowel syndrome, chronic abdominal pain, gallbladder disease or ovarian problems, so the appendix is not removed as a solution for unexplained abdominal pain, nor preventively when it is not inflamed. One dangerous myth is taking a strong painkiller, a laxative or an enema when appendicitis is suspected: this masks the signs and raises the risk of perforation. Stop eating and drinking and go to the emergency department.

## Preparation

If you have persistent lower right abdominal pain with nausea or fever, stop eating and drinking and go to the emergency department; do not take a strong painkiller, a laxative or an enema before assessment, as they mask the signs. Tell the doctor about every medicine you take, especially blood thinners, aspirin, diabetes medicines, immune-suppressing drugs and herbal supplements. Report any drug allergy or previous problem with anaesthesia, along with chronic illnesses and earlier abdominal operations. Tell the doctor if pregnancy is possible before any imaging. Remove jewellery, nail polish and contact lenses, bring previous reports, test results and your insurance card, and arrange someone to accompany you home after discharge.

## Aftercare

Get up and walk on the same day, as this lowers the risk of clots and adhesions, and start with fluids then light food as tolerated. Take pain relief as prescribed and complete any prescribed antibiotic course. Keep the wound clean and dry and avoid soaking in a bath or pool until your surgeon allows it. Avoid heavy lifting and strenuous effort for roughly two to four weeks, depending on the type of operation and your surgeon's advice. Mild bloating and shoulder-tip discomfort after laparoscopy are expected and temporary. \*\*Go to the emergency department immediately if you develop: a high fever or shivering; abdominal pain that increases instead of easing; distension with repeated vomiting; no passage of gas or stool; redness, pus or opening of the wound; dizziness or a racing pulse; shortness of breath or chest pain; pain and swelling in the leg.\*\*
