التشخيص سريري أولاً: ألم يبدأ حول السرّة أو في أعلى البطن ثم ينتقل ويتركّز في أسفل يمين البطن خلال ساعات، مع فقدان الشهية والغثيان وربما قيء وحرارة خفيفة، ويزداد الألم بالحركة والسعال والضغط. تُسانده تحاليل الدم والبول والسونار أو التصوير الطبقي المحوري عند الحاجة، ويُطلب فحص حمل لكل امرأة في سنّ الإنجاب لأن الحمل خارج الرحم وأمراض المبيض تحاكي الأعراض نفسها. الوقت عامل حاسم: التأجيل يزيد احتمال الانثقاب.

المنظار هو الطريق المفضّل في الأغلب لأن الألم بعده أقلّ والجرح أصغر والعودة للنشاط أسرع، ويسمح باستكشاف تجويف البطن كاملاً وهو ميزة مهمّة عند النساء وعند التشخيص غير المؤكّد. تُربَط قاعدة الزائدة وتُقطع مساريقها وتُخرَج داخل كيس لمنع تلويث الجرح، ويُشفَط أي قيح ويُغسَل التجويف، وقد يُوضَع تصريف في الحالات المعقّدة. في حالات الالتهاب الشديد أو الخُراج أو التصاقات كثيفة قد يقرّر الجراح التحويل إلى جراحة مفتوحة، وهذا قرار سلامة.

هناك مسار آخر يُطبَّق في حالات مختارة غير معقّدة وهو العلاج بالمضادات الحيوية وحدها دون جراحة، لكن احتمال تكرار الالتهاب بعده أعلى، وقد يحتاج المريض إلى جراحة لاحقاً؛ والقرار بين المسارين طبي ويعتمد على شدّة الحالة والتصوير وحالة المريض. أما وجود خُراج كبير فقد يُعالَج أولاً بتصريف ومضادات ثم جراحة لاحقة إن لزم.

وحدود العملية واضحة: استئصال الزائدة يعالج التهاب الزائدة فقط ولا يعالج أسباب ألم البطن الأخرى مثل القولون العصبي وحصوة الكلى وأمراض المبيض والتهاب الأمعاء، ولا يؤثّر على الهضم بعد إزالتها. ومن الخرافات الخطيرة أن ألم الزائدة يُحلّ بمسكّن وراحة ونوم عليه، أو بملين أو حقنة شرجية أو كمّادة ساخنة؛ هذه تؤخّر التشخيص وقد ترفع خطر الانثقاب. الترشيح للعملية وتوقيتها يُحدَّدان بالفحص السريري والتصوير، لا بالقراءة.

## خطوات الإجراء

### 1. تقييم طارئ وتشخيص

يُفحَص البطن بدقة ويُقاس النبض والحرارة، وتُطلب صورة دم وتحليل بول وفحص حمل لكل امرأة في سنّ الإنجاب، وسونار أو تصوير طبقي محوري عند الحاجة. الهدف تمييز التهاب الزائدة عن أسباب أخرى للألم يمينَ أسفل البطن قبل قرار الجراحة.

### 2. التحضير العاجل

يُمنع الأكل والشرب، وتُعطى سوائل وريدية ومسكّن آمن ومضاد حيوي وقائي قبل الشقّ بقرار الفريق، ويُقيَّم التخدير سريعاً مع مراجعة الأدوية ومسيّلات الدم والأمراض المزمنة. تُشرَح العملية واحتمال التحويل إلى جراحة مفتوحة ويُوقَّع الإقرار.

### 3. الدخول بالمنظار واستكشاف البطن

بعد التخدير العام تُعمَل ثلاثة شقوق صغيرة، ويُنفَخ البطن بغاز ثاني أكسيد الكربون، وتُدخَل الكاميرا. يُستكشَف التجويف لتأكيد التشخيص وتقييم شدّة الالتهاب ووجود انثقاب أو قيح، وللتحقّق من الأعضاء المجاورة خصوصاً المبيضين والأمعاء عند النساء.

### 4. استئصال الزائدة وتنظيف التجويف

تُربَط قاعدة الزائدة وتُقطَع مساريقها الحاملة للأوعية، ثم تُخرَج الزائدة داخل كيس لمنع تلويث الشقّ. يُشفَط القيح ويُغسَل التجويف عند الحاجة، ويُوضَع تصريف في الحالات المعقّدة، وتُرسَل الزائدة للفحص النسيجي.

### 5. التعافي والخروج

يُشجَّع المشي المبكر في اليوم نفسه، وتبدأ السوائل ثم الطعام الخفيف مع عودة حركة الأمعاء. البقاء عادة من يوم إلى يومين في الحالات البسيطة وأطول عند الانثقاب أو الخُراج مع مضادات وريدية. تُحدَّد مراجعة لمتابعة الجروح ونتيجة الفحص النسيجي.

## Procedure steps

### 1. Emergency assessment and diagnosis

The abdomen is examined carefully with pulse and temperature recorded, a blood count, urine test and a pregnancy test for every woman of childbearing age are requested, plus ultrasound or a CT scan when needed. The aim is to distinguish appendicitis from other causes of right lower abdominal pain before deciding on surgery.

### 2. Urgent preparation

You are kept nil by mouth, given intravenous fluids, safe analgesia and a prophylactic antibiotic before the incision as decided by the team, and a rapid anaesthetic assessment reviews your medications, blood thinners and chronic conditions. The operation and the possibility of conversion to open surgery are explained and consent is taken.

### 3. Laparoscopic entry and abdominal inspection

After general anaesthesia three small incisions are made, the abdomen is inflated with carbon dioxide and the camera introduced. The cavity is inspected to confirm the diagnosis, judge the severity of inflammation and look for perforation or pus, and to check neighbouring organs, particularly the ovaries and bowel in women.

### 4. Removing the appendix and cleaning the cavity

The base of the appendix is secured and its vascular mesentery divided, then the appendix is removed inside a retrieval bag to avoid contaminating the incision. Pus is aspirated and the cavity irrigated when needed, a drain is placed in complicated cases, and the appendix is sent for histological examination.

### 5. Recovery and discharge

Early walking on the same day is encouraged, and fluids then light food are started as bowel activity returns. Hospital stay is usually one to two days in simple cases and longer with perforation or an abscess, when intravenous antibiotics are needed. A review is arranged to check the wounds and the histology result.

## التحضير قبل الإجراء

إن كان لديك ألم في أسفل يمين البطن يزداد مع فقدان شهية وغثيان وحرارة فاذهب إلى الطوارئ ولا تنتظر، ولا تأكل ولا تشرب حتى يفحصك الطبيب لأن الجراحة قد تكون في الساعات القريبة. لا تأخذ مليّناً ولا حقنة شرجية ولا تضع كمّادة ساخنة على البطن، ولا تكتم الألم بمسكّنات قوية من عندك ثم تنم عليه، فهذا يؤخّر التشخيص. أخبر الفريق بموعد آخر أكل وشرب بدقّة، وبكل أدويتك ومنها مسيّلات الدم والأسبرين وأدوية السكري، وبحساسيتك الدوائية، وبأي تخدير سابق ومشاكله، وبعملياتك السابقة على البطن، وبأمراضك المزمنة. أخبريه إن كان هناك احتمال حمل أو تأخّر في الدورة. أزل المجوهرات وطلاء الأظافر، ورتّب من يبقى معك بعد الخروج.

## العناية بعد الإجراء

انزل من السرير وامشِ لمسافات قصيرة متكرّرة من اليوم الأول لتقليل خطر التجلّط والالتهاب الرئوي وتنشيط الأمعاء. ابدأ بالسوائل ثم الطعام الخفيف مع عودة الريح وحركة الأمعاء، واشرب سوائل كافية وتناول أليافاً لتفادي الإمساك، وخذ المسكّن الموصوف بانتظام أول أيام. حافظ على الشقوق نظيفة وجافة واتبع تعليمات الاستحمام، ولا تفتح اللاصق قبل الموعد المحدّد. تجنّب رفع الأثقال والرياضة العنيفة وشدّ عضلات البطن لمدة يحدّدها جراحك عادة من أسبوعين إلى أربعة، وتُستأنف الأعمال المكتبية عادة أسرع. أكمل المضاد الحيوي الموصوف كامل المدّة وراجع لسماع نتيجة الفحص النسيجي.

## About this service

Laparoscopic appendectomy is removal of the inflamed appendix through three small abdominal incisions with a camera and fine instruments under general anaesthesia, and it is the mainstay of treatment for acute appendicitis. The appendix is a small blind-ended tube attached to the start of the colon; when its lumen becomes blocked, bacteria multiply inside, it swells and becomes inflamed, and if left it may perforate and spread infection through the abdominal cavity.

Diagnosis is clinical first. Pain begins around the navel or the upper abdomen and moves over hours to settle in the lower right abdomen, with loss of appetite, nausea, sometimes vomiting and a low-grade fever, and it worsens with movement, coughing and pressure. Blood and urine tests support the picture, with ultrasound or a CT scan when needed, and a pregnancy test is requested for every woman of childbearing age because ectopic pregnancy and ovarian conditions mimic the same symptoms. Time matters: delay increases the chance of perforation.

Laparoscopy is usually the preferred route because there is less pain afterwards, smaller wounds and faster return to activity, and it allows inspection of the whole abdominal cavity, an important advantage in women and where the diagnosis is uncertain. The base of the appendix is secured, its blood supply divided, and the appendix removed inside a bag to avoid contaminating the wound. Any pus is aspirated and the cavity irrigated, and a drain may be left in complicated cases. With severe inflammation, an abscess or dense adhesions, the surgeon may convert to open surgery, which is a safety decision.

Another pathway exists for selected uncomplicated cases: treatment with antibiotics alone without surgery. However, the chance of recurrent inflammation afterwards is higher and some patients need surgery later. Choosing between the pathways is a medical decision based on severity, imaging and the patient's condition. Where there is a large abscess, initial drainage with antibiotics followed by later surgery if needed may be the safer sequence.

The limits are clear. Appendectomy treats appendicitis and nothing else; it does not treat other causes of abdominal pain such as irritable bowel syndrome, kidney stones, ovarian conditions or inflammatory bowel disease, and removing the appendix does not affect digestion. A dangerous myth is that appendix pain can be managed with a painkiller, rest and sleeping it off, or with a laxative, an enema or a hot compress; these delay diagnosis and may increase the risk of perforation. Whether and when to operate is decided by clinical examination and imaging, not by reading.

## Preparation

If you have right lower abdominal pain that is worsening with loss of appetite, nausea and fever, go to the emergency department and do not wait, and do not eat or drink until you have been assessed, because surgery may be needed within hours. Do not take a laxative or an enema, do not apply a hot compress to the abdomen, and do not mask the pain with strong painkillers of your own and sleep it off, as this delays diagnosis. Tell the team exactly when you last ate and drank, list all your medications including blood thinners, aspirin and diabetes medicines, report drug allergies, any previous anaesthetic and problems with it, previous abdominal operations, and chronic conditions. Mention any possibility of pregnancy or a missed period. Remove jewellery and nail polish, and arrange for someone to be with you after discharge.

## Aftercare

Get out of bed and take short frequent walks from the first day to reduce the risk of clots and chest infection and to stimulate the bowel. Start with fluids then light food as gas and bowel movements return, drink enough fluids and include fibre to avoid constipation, and take the prescribed painkiller regularly for the first few days. Keep the incisions clean and dry, follow the showering instructions, and do not remove the dressings before the time you were told. Avoid heavy lifting, vigorous sport and abdominal straining for the period your surgeon sets, usually two to four weeks, while desk work is normally resumed sooner. Complete any prescribed antibiotic course in full and attend for the histology result.

Seek care immediately if you develop: \*\*a temperature of 38C or above with shivering, abdominal pain that increases after the second day or spreading pain with a rigid abdomen, persistent vomiting that prevents drinking, distension with no passage of gas or stool, redness, warmth or pus from an incision or a wound that opens, severe watery diarrhoea, or breathlessness, chest pain, or pain and swelling in one leg, all of which are emergencies\*\*.
