## Procedure steps

### 1. Identifying and characterising the polyp

The mucosa is inspected carefully and the polyp's size, shape, base and location are documented, sometimes with special light filters to assess its surface pattern. This characterisation dictates which removal technique is safest.

### 2. Submucosal lift when needed

For flat or broad-based lesions, sterile fluid is injected beneath the mucosa so the tissue rises as a cushion separated from the deeper layers of the bowel wall, reducing the risk of injuring the wall during resection.

### 3. Resecting the polyp

A wire snare is closed around the base and the polyp is transected, or very small polyps are grasped with forceps. Resection may be cold, without electrical current, or current-assisted, depending on size and the endoscopist's judgement.

### 4. Securing the site and retrieving tissue

The resection site is examined; if there is bleeding or a risk of delayed bleeding, it is closed with small metal clips or treated thermally. The resected tissue is then retrieved from the bowel and sent to the laboratory.

### 5. Pathology and the surveillance plan

The tissue is examined microscopically, and the type, number, size and degree of change determine the timing of your next examination. Your doctor explains the result and what it means; this step is what turns the procedure into ongoing prevention rather than a one-off event.

## About this service

Polypectomy is the removal of a polyp — a fleshy growth arising from the mucosal lining of the colon or stomach — during an endoscopy session. Fine instruments are passed through a working channel inside the endoscope: a small forceps for tiny polyps, a wire snare that encircles the base of the polyp and cuts it, or an injection of sterile fluid beneath the mucosa to lift the lesion away from the bowel wall before it is resected. Everything is done from inside the bowel, with no external incision.

The main value of the procedure is preventive. Most colorectal cancers do not begin as cancer; they begin as a benign polyp that changes slowly over many years. Removing that polyp now interrupts the sequence before it completes, which is why polyp removal during endoscopy is one of the most effective preventive interventions in gastroenterology. Every polyp removed is sent for pathology, and its type, number and size are what determine when your next examination should be scheduled.

Do not postpone assessment if warning signs appear: rectal bleeding or blood in the stool, unexplained weight loss, a persistent change in bowel habit such as new diarrhoea, new constipation or narrowing of the stool, and iron-deficiency anaemia with no clear cause. These are reasons for prompt review by a gastroenterologist rather than symptoms to watch at home.

The limits matter as much as the benefits. Removing polyps today does not stop new polyps from forming later, so scheduled surveillance is part of the treatment and not an optional extra. Polypectomy does not treat irritable bowel syndrome, bloating or chronic diarrhoea, it does not replace repeating the examination when poor bowel preparation obscured the view, and the appearance of a polyp never replaces the laboratory report.

Which technique is used — forceps, snare, or fluid lift with mucosal resection — is decided during the procedure itself, based on the size, base and location of the polyp. Suitability, and the handling of your medications, particularly blood thinners, are determined by clinical assessment and review of your medical file, not by reading an article.

## Preparation

Preparation is the single biggest determinant of examination quality: an unclean colon hides small polyps and wastes the session. Follow the low-fibre diet you are given before the appointment, avoid legumes, grains, seeds and raw vegetables, then move to clear fluids in the final stage. Drink the cleansing solution in full and at the times your doctor specifies; splitting it into two doses usually gives a better view. Tell your doctor about every medication you take, especially blood thinners and anticoagulants, diabetes medicines and iron tablets, as some need special arrangements beforehand. Also mention pregnancy, a heart valve, a pacemaker, or any drug allergy. Fasting from food and fluids before sedation is mandatory for your safety. Arrange an escort to take you home in advance, because you will not be allowed to drive after sedation.

## Aftercare

Expect bloating, wind and mild cramps that settle with walking and passing gas. Start with fluids, then light food as instructed, and drink well to replace what the preparation removed. Seek emergency care immediately for heavy rectal bleeding or repeated passage of blood, severe or increasing abdominal pain, a distended or rigid abdomen, fever and chills, dizziness or fainting, or repeated vomiting. These can indicate bleeding or perforation: rare complications, but real ones that must never be ignored. Remember that bleeding after polyp removal can be delayed by several days, so an uneventful first day does not end the need for vigilance. Avoid strenuous exertion, heavy lifting and long journeys in the first days, and restart blood thinners on the date your doctor sets rather than on your own judgement. Do not drive or sign important documents on the day of sedation, and do not spend that night alone. Return to collect your pathology result and your next surveillance date.
