## Procedure steps

### 1. Staging and fitness assessment

Cross-sectional and functional imaging are completed, lymph nodes are evaluated by endoscopic or surgical sampling where required, and pulmonary function tests, an electrocardiogram and blood tests are performed. A multidisciplinary team reviews the case to decide whether surgery is the most appropriate next step or whether another treatment should come first.

### 2. Anaesthesia, positioning and access

The operation is performed under general anaesthesia with ventilation that allows the operated lung to be temporarily deflated, and the patient is positioned on their side. The surgeon introduces a thoracoscope and instruments through small incisions between the ribs, or makes a conventional lateral incision when the tumour is large or adhesions are dense; conversion from thoracoscopic to open surgery may be necessary during the procedure for the patient's safety.

### 3. Tumour removal with clear margins

The artery, vein and airway supplying the target portion are identified and securely divided, then the lung tissue is separated and the tumour lifted out with clear margins around it. The specimen is sent to pathology, and margins may be examined during the operation to decide whether a wider resection is required.

### 4. Lymph node sampling and chest inspection

Lymph nodes from the regional and mediastinal stations are sampled for accurate staging, and the pleural surface and lung are inspected for any additional deposits. The closure is tested for air leak and any bleeding is controlled.

### 5. Chest drain, closure and early care

One or more chest drains are placed to evacuate air and fluid and re-expand the lung, and the incisions are closed in layers. The patient is woken and monitored in an intensive care or step-down unit, pain control, breathing exercises and walking usually begin on the first day, and the drain is removed once the air leak has stopped and drainage has fallen.

## About this service

Lung cancer resection is surgery in which the cancerous mass is removed together with a portion of surrounding lung tissue and a clear margin around it, along with sampling of the lymph nodes inside the chest for microscopic examination. Surgery is the primary option for tumours confined to the lung that have not spread, and the decision follows complete staging with cross-sectional and functional imaging plus lymph node evaluation, not a single scan.

The surgeon selects the extent of resection according to tumour location and size and the patient's lung function. Removing a whole lobe is the most common operation; an anatomical segmentectomy or a wedge resection may be sufficient for small peripheral tumours; and removal of an entire lung may be required when the tumour sits centrally on a main airway. The route of access may be video-assisted thoracoscopic surgery through small incisions, robot-assisted surgery, or a traditional open thoracotomy when there are adhesions, large tumours, or a need for airway reconstruction.

Lymph node sampling is not an optional extra but a core part of the operation, because the microscopic result establishes the final stage, determines whether chemotherapy, immunotherapy or radiotherapy is needed after surgery, and shapes the follow-up plan. For this reason the treatment plan after the operation may differ from what was expected beforehand.

Before surgery is agreed, the patient's ability to tolerate it is assessed: lung function testing, cardiac status, anaemia, diabetes and nutritional state, together with stopping smoking beforehand, which genuinely reduces respiratory and wound complications. A chest drain afterwards is expected and temporary, and breathing exercises with early walking are the two most important contributors to a faster recovery.

The limits are important. Surgery is not suitable for every person with lung cancer: metastatic or locally advanced disease, or poor lung function, may make drug treatment or radiotherapy more appropriate. Resection does not remove the need for additional treatment if the oncology team recommends it, it does not repair smoking damage in the rest of the lung, and it does not end the need for regular imaging follow-up. Neither candidacy nor the type of resection can be decided except by clinical examination, imaging results and multidisciplinary team review.

## Preparation

Stop smoking immediately, ideally several weeks before surgery, as this is the single most effective step to reduce respiratory and wound complications, and do not substitute an electronic cigarette. Practise deep breathing exercises and the incentive spirometer beforehand, because you will need them afterwards, and walk daily to improve your fitness. Tell the doctor about all your medicines, particularly blood thinners, antiplatelet drugs, aspirin, diabetes medicines, immunosuppressants, steroids, herbal products and supplements, since some are stopped before the operation for an interval only your doctor can set. Treat any dental or chest infection before the date, and report any fever or increase in cough or sputum. Follow the fasting instructions before anaesthesia exactly, and bring your reports, imaging and laboratory results. Arrange a companion for the hospital stay and the journey home, prepare loose clothing, and organise work and household cover for a recovery that may take several weeks.

## Aftercare

Expect a chest drain for typically one to four days and a hospital stay of about two to six days, usually shorter with thoracoscopic surgery. Use the incentive spirometer every waking hour, breathe deeply and cough effectively while supporting the wound with your hands or a pillow, because suppressing a cough leads to lung collapse and infection. Take pain relief on a regular schedule rather than only when pain peaks, since good analgesia is what allows you to breathe and walk. Walk several times a day from the first day, and move the shoulder and arm on the operated side to prevent stiffness. Do not lift heavy weights or drive before your surgeon allows it, stop smoking permanently, and maintain adequate protein and fluid intake. Attend follow-up appointments, review the pathology result, and follow any additional treatment plan prescribed.

Seek care immediately if there is: \*\*worsening breathlessness or sudden sharp chest pain\*\*; \*\*a temperature of 38C or above, or shivering\*\*; \*\*wound discharge, pus, spreading redness or separation\*\*; \*\*coughing up blood or a sudden increase in purulent sputum\*\*; \*\*leg pain, swelling or redness, or palpitations with dizziness, which requires clot exclusion\*\*; \*\*expanding swelling under the skin with a crackling sensation on touch\*\*; \*\*persistent vomiting or inability to drink\*\*.
