بعد العملية تُفحص القطعة المستأصلة نسيجياً، وهذا التقرير — لا شكل العملية — هو ما يحدّد المرحلة النهائية والحاجة إلى علاج كيميائي مساند. لذلك يُدار المرض ضمن فريق يشترك فيه جرّاح القولون والمستقيم وطبيب الأورام والأشعة والمختبر، ويُناقَش القرار غالباً في اجتماع فريق متعدّد التخصصات قبل الجراحة وبعدها.

حدود العملية: هي تُزيل الورم من موضعه، ولا تُغني بذاتها عن العلاج المساند إن أوصى به التقرير النسيجي، ولا تمنع ظهور ورم جديد لاحقاً في بقية القولون — ولهذا تستمر المتابعة بالمناظير والتحاليل والتصوير سنوات بعدها. ومن الشائع أن يظنّ المريض أن كل عملية قولون تعني كيس فغر دائم، والصحيح أن أكثر الحالات تُوصل فيها الأمعاء مباشرة، وأن الفغر يُلجأ إليه في حالات محدّدة وكثيراً ما يكون مؤقتاً لحماية الوصلة الجديدة ثم يُغلق في عملية أخرى.

الترشيح للعملية ونوعها وطريقها يُحدَّدان بالفحص السريري ونتائج المنظار والتصوير وتقييم القلب والرئة، ولا يمكن تقديرها من قراءة صفحة أو من تجربة قريب أو صديق.

## خطوات الإجراء

### 1. التشخيص وتحديد المرحلة

يُثبَّت التشخيص بمنظار وخزعة، ثم يُحدَّد الامتداد بتصوير مقطعي وتحاليل، وتُقيَّم صلاحيتك للتخدير والجراحة من ناحية القلب والرئة والتغذية. تُناقَش الحالة عادةً في اجتماع فريق متعدّد التخصصات ليُختار الترتيب الأنسب بين الجراحة والعلاجات الأخرى.

### 2. التحضير قبل العملية

تُعدَّل الأدوية المؤثّرة على التخثّر والسكري بإرشاد الطبيب، ويُطلب تحضير الأمعاء في بعض العمليات لا كلها، وتُؤخَذ تدابير الوقاية من التخثّر الوريدي ويُعطى مضاد حيوي وقائي قبل الشق. يُشرح لك احتمال الحاجة إلى فغر مؤقت ومكانه إن كان ذلك وارداً.

### 3. استئصال القطعة الحاملة للورم

يُدخل الجرّاح عبر فتحات المنظار أو الشق المفتوح، ويُحرَّر القولون، ثم تُستأصل القطعة التي فيها الورم مع هامش سليم من الجانبين ومع المساريقا والعقد الليمفية التابعة لها كوحدة واحدة، مع تفادي إصابة الأعضاء والأعصاب المجاورة.

### 4. إعادة توصيل الأمعاء

تُوصل نهايتا الأمعاء السليمتان بالخِيَط أو بالدبّاسة الجراحية، ويُتحقّق من سلامة الوصلة وتروية أطرافها. في حالات مختارة — كالجراحة الطارئة أو الأنسجة الملتهبة أو الوصلة المنخفضة — يُعمل فغر مؤقت يحمي الوصلة ويُغلق لاحقاً.

### 5. التعافي المبكر والتقرير النسيجي

يُتبع برنامج تعافٍ معجّل: شرب وحركة مبكران، تمارين تنفّس، تسكين ألم منظّم، وسحب القسطرة والمغذّيات مبكراً ما أمكن. ثم يُراجع تقرير المختبر معك لتحديد المرحلة النهائية والحاجة إلى علاج مساند وجدول المتابعة.

## Procedure steps

### 1. Diagnosis and staging

The diagnosis is confirmed by colonoscopy and biopsy, the extent is defined by CT imaging and blood tests, and your fitness for anaesthesia and surgery is assessed from a cardiac, respiratory and nutritional standpoint. The case is usually reviewed in a multidisciplinary meeting so the best sequence between surgery and other treatments is chosen.

### 2. Preoperative preparation

Medicines affecting clotting and diabetes control are adjusted under medical guidance, bowel preparation is requested for some operations but not all, venous thrombosis prophylaxis is arranged and a prophylactic antibiotic is given before the incision. If a temporary stoma is a possibility, this is explained and its site marked in advance.

### 3. Resecting the tumour-bearing segment

The surgeon enters through laparoscopic ports or an open incision, mobilises the colon, then removes the tumour-bearing segment with a healthy margin on each side together with its mesentery and draining lymph nodes as a single block, while protecting neighbouring organs and nerves.

### 4. Restoring bowel continuity

The two healthy bowel ends are joined with sutures or a surgical stapler, and the join is checked for integrity and good blood supply. In selected situations — emergency surgery, inflamed tissue or a low join — a temporary stoma is created to protect the anastomosis and closed at a later date.

### 5. Early recovery and the pathology report

An enhanced recovery programme is followed: early drinking and mobilisation, breathing exercises, structured pain control, and early removal of catheters and drips where possible. The pathology report is then reviewed with you to set the final stage, the need for adjuvant treatment and the surveillance schedule.

## العناية بعد الإجراء

تحرّك مبكراً وبتدرّج كما يُطلب منك، فالمشي القصير المتكرّر يقلّل خطر التخثّر والالتهاب الرئوي ويُسرّع عودة حركة الأمعاء. مارس تمارين التنفّس العميق، واستخدم المسكّنات المنتظمة حتى تتمكّن من الحركة والسعال بلا خوف. ابدأ السوائل ثم الطعام بالتدرّج الذي يوصفه الفريق، واشرب ماءً كافياً. اعتنِ بالجرح وحافظ على جفافه ونظافته، وتجنّب حمل الأثقال والجهد الشديد نحو أربعة إلى ستة أسابيع. توقّع تغيّراً مؤقتاً في نمط التبرّز — تكراراً أو إلحاحاً أو غازات — يتحسّن غالباً تدريجياً. التزم بمواعيد مراجعة تقرير المختبر ومتابعة الأورام إن أُوصي بها. \*\*راجع فوراً عند ارتفاع الحرارة أو الرجفة، أو ألم بطن متزايد مع انتفاخ وتوقّف الغازات، أو قيء متكرّر، أو احمرار وتسريب من الجرح أو خروج مادة برائحة الأمعاء منه، أو دوخة وتسارع نبض، أو ضيق نفس وألم صدر، أو ألم وتورّم في الساق.\*\*

## About this service

Colon cancer resection is an operation in which the segment of colon containing the tumour is removed together with a margin of healthy bowel on either side, along with the mesentery and lymph nodes that drain that segment; the two healthy ends are then joined in a connection called an anastomosis. The lymph nodes are removed because this tumour typically spreads through them first, and examining them in the laboratory is what reveals the true stage of the disease.

Several diagnostic steps come first: a colonoscopy with biopsy to confirm the tumour type, CT imaging of the chest, abdomen and pelvis for staging, and blood tests. The resection is named according to tumour location — right, left, transverse or sigmoid — and it may be performed laparoscopically through small incisions or as open surgery. That choice follows the position and size of the tumour, previous abdominal operations and the patient overall condition, not preference alone.

After surgery the removed specimen is examined by a pathologist, and it is that report, not the appearance of the operation, which sets the final stage and decides whether adjuvant chemotherapy is advised. For this reason the disease is managed by a team that includes the colorectal surgeon, medical oncologist, radiologist and pathologist, and the plan is usually discussed in a multidisciplinary meeting both before and after the operation.

The limits matter. Resection removes the tumour from its site; it does not replace adjuvant treatment when the pathology report calls for it, and it does not prevent a new tumour from arising later elsewhere in the colon, which is why surveillance with colonoscopy, blood tests and imaging continues for years. Many patients also assume that any colon operation means a permanent bag. In fact the bowel is usually joined directly, and a stoma is reserved for specific situations, often as a temporary measure to protect a fresh join before being closed at a second operation.

Whether you are a candidate, and which operation and approach suit you, is decided by clinical examination together with colonoscopy, imaging and heart and lung assessment — never from reading a page or from a relative experience.

## Preparation

Tell your doctor everything you take, especially blood thinners, aspirin, diabetes medication, glucose or weight-control injections, steroids and herbal supplements: some are stopped a set time before surgery and others are adjusted rather than stopped, and never on your own initiative. Mention your chronic conditions, any previous abdominal operations and all drug allergies. Stop smoking as early as you can, since this reduces both lung and wound complications. Ask whether bowel preparation is required for your particular operation, how and when to take it, and how to manage clear fluids afterwards. Confirm your fasting time and arrival time, and bring your blood tests, ECG and medicines in their original boxes. Arrange an escort, a hospital stay of several days and time off work. Ask directly about the possibility of a temporary stoma and how it would be managed, because understanding it beforehand makes recovery considerably easier.

## Aftercare

Get moving early and progressively as instructed: repeated short walks lower the risk of clots and chest infection and speed the return of bowel function. Do deep breathing exercises and take regular analgesia so you can move and cough without fear. Start fluids and then food at the pace your team sets, and drink enough water. Keep the wound clean and dry, and avoid heavy lifting and strenuous effort for roughly four to six weeks. Expect a temporary change in bowel habit — frequency, urgency or wind — which usually settles gradually. Keep the appointments for reviewing the pathology report and for oncology follow-up if advised. \*\*Seek care immediately if you develop fever or chills, increasing abdominal pain with distension and no passage of wind, repeated vomiting, redness or leakage from the wound or bowel-smelling discharge from it, dizziness with a fast pulse, breathlessness or chest pain, or pain and swelling in the leg.\*\*
