الأعراض المعتادة انتفاخ مع شدّ أو ثقل أو ألم خفيف يزداد آخر النهار، وقد يمتدّ الإحساس إلى الخصية عند الرجال. عوامل مثل السعال المزمن، الإجهاد عند التبوّل أو التبرّز، حمل الأوزان، والعمل الواقف الطويل تزيد الأعراض. أما الانتفاخ الصلب المؤلم الذي لا يعود إلى الداخل ويصحبه غثيان وتقيّؤ وتوقّف خروج الريح، فهو حالة طارئة تعني احتمال احتباس محتويات الفتق أو خنقها وتحتاج طوارئ فوراً.

الإصلاح يُجرى بطريقتين رئيسيتين: إصلاح مفتوح بشقّ في المنطقة الإربية، أو إصلاح بالمنظار عبر فتحات صغيرة. في معظم حالات الكبار تُستخدم شبكة صناعية لتدعيم الجدار وتوزيع الشدّ، وقد يُفضّل الإصلاح بالخياطة وحدها في حالات مختارة مثل الفتق الصغير جداً أو الأطفال. اختيار الطريقة ليس مسألة تفضيل عام، بل يعتمد على حجم الفتق وكونه ثنائياً أو ناكساً، وعمل المريض ووزنه وأمراضه، والترشيح يُحدَّد بالفحص السريري.

حدود الإجراء: الفتق لا يُشفى بالتمارين ولا بشدّ عضلات البطن ولا بالمساج ولا بالأعشاب، والحزام الطبي يخفّف الإحساس مؤقتاً فقط ولا يغلق الفتحة أبداً، وقد يعطي طمأنينة زائفة تؤخّر العلاج. ولا يوجد «إصلاح فتق بالليزر»: الليزر لا يبني جدار البطن، وأي طريقة تُسمّى بذلك تعني في الواقع إصلاحاً جراحياً بأدوات مختلفة.

كما أن العملية لا تعالج آلام أسفل الظهر ولا الشدّ العضلي الرياضي ولا مشاكل الخصية، ولا تمنع ظهور فتق جديد في موضع آخر أو رجوع الفتق نفسه، خصوصاً مع استمرار السعال المزمن أو زيادة الوزن أو رفع الأثقال بعد العملية بلا تدرّج. التنميل أو الألم المزمن في المنطقة احتمال معروف بعد أي إصلاح ويجب مناقشته مع الجرّاح قبل القرار.

## خطوات الإجراء

### 1. الفحص السريري وتأكيد التشخيص

يفحص الجرّاح المنطقة وأنت واقف وأنت مستلقٍ، ويطلب منك السعال لتحديد موضع الفتق وحجمه وقابليته للإرجاع. أكثر الحالات تُشخَّص بالفحص وحده، ويُطلب سونار أو تصوير مقطعي عند الشك أو عند وجود ألم بلا انتفاخ واضح.

### 2. التخطيط للعملية واختيار الطريقة

يُناقش معك الإصلاح المفتوح أو بالمنظار، ونوع التدعيم، ونوع التخدير الممكن (عام أو نصفي أو موضعي مع تهدئة في حالات مختارة). تُضبط أمراضك المزمنة قبل الموعد، ويُنصح بالإقلاع عن التدخين ومعالجة السعال والإمساك لتقليل الشدّ على الإصلاح.

### 3. الوصول إلى الفتق وإعادة محتوياته

بعد التخدير يُفتح المسار إلى كيس الفتق، سواء بشقّ في المنطقة الإربية أو من داخل البطن بالمنظار. يُفصل الكيس بعناية عن الحبل المنوي والأوعية والأعصاب، وتُعاد محتوياته إلى البطن، ويُفحص النسيج للتأكد من سلامته.

### 4. تقوية جدار البطن

تُوضع شبكة مسطّحة تغطّي منطقة الضعف كلها وتُثبَّت بخياطة أو مثبّتات، أو يُخاط الجدار مباشرة في حالات مختارة. الهدف تدعيم بلا شدّ زائد، لأن الشدّ هو أهمّ سبب لعودة الفتق وللألم المزمن.

### 5. الإغلاق والتعافي والعودة للنشاط

تُغلق الطبقات والجلد، وتُفيق من التخدير، وكثير من الحالات المبرمَجة تخرج في اليوم نفسه. تبدأ المشي في اليوم الأول، ويُسمح بالعودة التدريجية للعمل المكتبي في أيام، أما الأعمال الشاقّة ورفع الأثقال فتحتاج تدرّجاً وفق تعليمات جرّاحك.

## Procedure steps

### 1. Clinical examination and confirming the diagnosis

The surgeon examines the groin with you standing and lying down and asks you to cough, to establish the site and size of the hernia and whether it reduces. Most cases are diagnosed by examination alone; ultrasound or cross-sectional imaging is requested when the diagnosis is unclear or there is pain without an obvious bulge.

### 2. Planning the operation and choosing the technique

Open versus laparoscopic repair is discussed with you, along with the type of reinforcement and the anaesthetic options: general, spinal, or local with sedation in selected cases. Chronic conditions are optimised before the date, and stopping smoking and treating cough or constipation are advised to reduce strain on the repair.

### 3. Reaching the hernia and reducing its contents

After anaesthesia the surgeon exposes the hernial sac, either through a groin incision or from inside the abdomen laparoscopically. The sac is carefully separated from the spermatic cord, vessels and nerves, its contents are returned to the abdomen, and the tissue is inspected to confirm it is healthy.

### 4. Reinforcing the abdominal wall

A flat mesh is placed to cover the entire weak area and fixed with sutures or tacks, or the wall is sutured directly in selected cases. The aim is support without excessive tension, since tension is the main driver of both recurrence and chronic pain.

### 5. Closure, recovery and return to activity

The layers and skin are closed, you wake from anaesthesia, and many elective cases go home the same day. Walking starts on day one, gradual return to desk work is usually possible within days, while heavy work and lifting require a staged return according to your surgeon's instructions.

## التحضير قبل الإجراء

أخبر الطبيب بكل أدويتك، وخصوصاً مسيّلات الدم ومضادات التجمّع الصفيحي وأدوية السكري، ولا توقف أي دواء إلا بتعليمات مكتوبة منه. أخبره بأمراض القلب والتنفّس والحساسية الدوائية وأي عملية سابقة في المنطقة الإربية أو أسفل البطن. عالج ما يزيد الضغط على جدار البطن قبل العملية: توقّف عن التدخين قدر الإمكان لأنه يزيد السعال ويؤثّر على الشفاء، وعالج الإمساك، وراجع الطبيب إن كان لديك إجهاد عند التبوّل. التزم بالصيام الذي يحدّده الفريق، عادةً من ست إلى ثماني ساعات. استحمّ في الصباح ولا تحلق شعر المنطقة بنفسك بالشفرة لأن ذلك يزيد خطر العدوى، ودع الفريق يفعل ذلك إن لزم. أحضر تقاريرك وبطاقة التأمين، وارتدِ ملابس واسعة، ورتّب مَن يعيدك إلى البيت.

## العناية بعد الإجراء

امشِ من اليوم الأول بخطوات قصيرة متكرّرة، فذلك يقلّل خطر التجلّط ويسرّع التعافي، وتجنّب الاستلقاء الطويل. اسند البطن بيدك عند السعال أو العطس أو النهوض من السرير في الأيام الأولى. خُذ المسكّنات كما وُصفت، وحافظ على الجرح نظيفاً وجافاً، ولا تنقعه في حوض أو مسبح حتى يسمح الطبيب. توقّع تورّماً وكدمة وأحياناً تجمّع سائل بسيط في المنطقة أو الخصية، وقد يستمرّ أسابيع ويخفّ تدريجياً. تجنّب رفع الأثقال والمجهود العنيف عادةً من أسبوعين إلى ستة أسابيع بحسب نوع العمل وطريقة الإصلاح وتوجيه جرّاحك، وارجع للرياضة بتدرّج. حافظ على وزن صحي وعالج الإمساك والسعال لتقليل احتمال عودة الفتق. \*\*راجع فوراً عند: حرارة مرتفعة أو قشعريرة، ألم يزداد بحدّة، احمرار أو صديد أو انفتاح في الجرح، انتفاخ صلب مؤلم لا يعود إلى الداخل، غثيان وتقيّؤ مع توقّف خروج الريح، صعوبة أو عدم قدرة على التبوّل، تورّم وألم شديد في الخصية، ألم وتورّم في الساق أو ضيق نفس.\*\*

## About this service

Inguinal hernia repair is an operation that returns the protruding abdominal contents to their place and reinforces the weak point in the lower abdominal wall at the groin. The hernia itself is a bulge of fatty tissue or a loop of bowel through that weak point, appearing as a swelling in the groin that increases on standing, coughing or lifting and reduces or disappears when lying down.

Usual symptoms are a bulge with dragging, heaviness or mild ache that worsens towards the end of the day, and in men the sensation may extend into the scrotum. Chronic cough, straining to pass urine or stool, lifting weights and long hours of standing all make symptoms worse. A firm, painful bulge that will not go back in, accompanied by nausea, vomiting and no passage of gas, is an emergency: it may mean the hernia contents are trapped or strangulated, and it needs emergency care immediately.

Repair is carried out in two main ways: open repair through a groin incision, or laparoscopic repair through small ports. In most adults a synthetic mesh is used to support the wall and spread tension, while suture-only repair may be preferred in selected situations such as a very small defect or in children. The choice is not a matter of general preference; it depends on the size of the hernia, whether it is bilateral or recurrent, and on the patient's occupation, weight and medical conditions. Suitability is determined by clinical examination.

Limits of the procedure: a hernia is not cured by exercise, abdominal muscle work, massage or herbal remedies. A truss or support belt only eases the sensation temporarily and never closes the defect; it can also give false reassurance that delays treatment. There is no such thing as laser hernia repair: a laser does not rebuild the abdominal wall, and anything marketed under that name is in reality surgical repair with different instruments.

The operation also does not treat low back pain, sports-related muscle strain or testicular problems, and it cannot guarantee that no new hernia will appear elsewhere or that this one will never recur, particularly if a chronic cough continues, weight increases or heavy lifting resumes too soon. Numbness or persistent groin discomfort is a recognised possibility after any repair and should be discussed with your surgeon before deciding.

## Preparation

Tell your doctor about every medicine you take, especially blood thinners, antiplatelet drugs and diabetes medicines, and stop nothing without written instructions. Report heart and lung conditions, drug allergies and any previous operation in the groin or lower abdomen. Deal with anything that raises pressure on the abdominal wall before surgery: stop smoking as far as you can, since it worsens cough and impairs healing, treat constipation, and mention any straining to pass urine. Follow the fasting instructions given by your team, usually six to eight hours. Shower on the morning of surgery and do not shave the area yourself with a razor, as this increases infection risk; let the team do it if needed. Bring your reports and insurance card, wear loose clothing and arrange someone to take you home.

## Aftercare

Walk from the first day in short, frequent bouts, as this reduces clot risk and speeds recovery; avoid long spells lying down. Support your abdomen with your hand when you cough, sneeze or get out of bed during the first days. Take pain relief as prescribed, keep the wound clean and dry, and do not soak it in a bath or pool until your doctor allows it. Expect swelling, bruising and sometimes a small fluid collection in the groin or scrotum, which may take weeks to settle. Avoid heavy lifting and strenuous effort for roughly two to six weeks depending on your work, the repair technique and your surgeon's advice, and return to sport gradually. Keep a healthy weight and treat constipation and cough to reduce the chance of recurrence. \*\*Seek care immediately if you develop: high fever or shivering; sharply increasing pain; redness, pus or opening of the wound; a firm painful bulge that will not go back in; nausea and vomiting with no passage of gas; difficulty or inability to pass urine; severe testicular swelling and pain; leg pain and swelling or shortness of breath.\*\*
