عند الكبار ترتبط الحالة بزيادة الوزن، وتكرار الحمل، والسعال المزمن، وحمل الأوزان، وتجمّع السوائل في البطن، والعمليات السابقة. الأعراض المعتادة انتفاخ مع شدّ أو ألم خفيف يزداد آخر اليوم أو بعد المجهود، وقد يبدو الجلد فوق الفتق مشدوداً. أما الانتفاخ الصلب المؤلم الذي لا يعود إلى الداخل مع غثيان أو تقيّؤ أو توقّف خروج الريح فهو حالة طارئة تعني احتمال احتباس أو خنق محتويات الفتق.

عند الرُّضّع والأطفال الصغار الوضع مختلف: معظم الفتوق السرّية الصغيرة تُغلق تلقائياً مع نمو الطفل خلال السنوات الأولى، ولذلك يكون التوجّه الأول عادةً المراقبة لا الجراحة، وتُطرح العملية عند بقاء الفتق بعد سنّ معيّنة أو كبر حجمه أو حدوث احتباس. والقرار في كل الأعمار يُحدَّد بالفحص السريري لا بالقراءة.

طريقة الإصلاح تعتمد على حجم الفتحة وحالة الأنسجة: الفتحات الصغيرة قد تُغلق بخياطة الطبقات، والفتحات الأكبر أو المتكرّرة تُدعَّم بشبكة صناعية، وقد يُجرى الإصلاح بشقّ صغير حول السرّة أو بالمنظار. لا توجد طريقة واحدة تناسب الجميع.

حدود الإجراء: إصلاح الفتق السري يغلق الفتحة ويعالج البروز، ولا يشدّ ترهّل جلد البطن، ولا يزيل الدهون، ولا يُنقص الوزن، ولا يعالج بالضرورة تباعد عضلات البطن الطولية الذي يشيع بعد الحمل، ولا يجعل شكل السرّة كما كان تماماً. ومن الخرافات الضارّة ربط قطعة نقود أو شدّ رباط على سرّة الطفل، فهي لا تُغلق الفتق وتسبّب تسلّخ الجلد والتهابه. كما أن استمرار زيادة الوزن أو السعال المزمن أو الرجوع السريع لرفع الأثقال يرفع احتمال عودة الفتق بعد أي إصلاح.

## خطوات الإجراء

### 1. الفحص وتحديد حجم الفتحة

يفحص الجرّاح السرّة وأنت مستلقٍ ثم واقف، ويطلب منك الشدّ أو السعال ليقدّر حجم الفتحة وقابلية الفتق للإرجاع ومحتوياته. قد يُطلب سونار أو تصوير مقطعي في الفتوق الكبيرة أو المتكرّرة أو عند وجود عمليات بطن سابقة.

### 2. تجهيز المريض وضبط العوامل المؤثّرة

تُضبط أمراضك المزمنة، ويُنصح بتخفيف الوزن قدر الممكن قبل الإصلاح المبرمَج، وبعلاج السعال والإمساك ومشاكل التبوّل، لأنها ترفع الضغط داخل البطن وتُجهد الإصلاح. يُوقف مسيّل الدم بتعليمات الطبيب فقط، ويُحدَّد نوع التخدير المناسب.

### 3. الوصول إلى كيس الفتق

يُجرى شقّ صغير في ثنية الجلد قرب السرّة أو تُدخل أدوات المنظار عبر فتحات صغيرة. يُفصل كيس الفتق عن الجلد والأنسجة المحيطة بعناية للحفاظ على شكل السرّة، ثم تُعاد المحتويات إلى داخل البطن ويُفحص النسيج.

### 4. إغلاق الفتحة والتدعيم

تُغلق الفتحة بخياطة قوية إن كانت صغيرة، أو تُدعَّم بشبكة صناعية إن كانت أوسع أو الأنسجة ضعيفة أو الفتق ناكساً. تُوضع الشبكة في الطبقة المناسبة وتُثبَّت بحيث تغطّي حدود الفتحة كلها بلا شدّ زائد.

### 5. الإغلاق والخروج والمتابعة

يُعاد تثبيت السرّة إلى الجدار للحصول على شكل طبيعي قدر الإمكان، ثم يُغلق الجلد. معظم الحالات المبرمَجة تخرج في اليوم نفسه أو بعد ليلة واحدة، مع تعليمات مكتوبة للنشاط ومواعيد مراجعة لتقييم الجرح والتورّم.

## Procedure steps

### 1. Examination and sizing the defect

The surgeon examines the navel with you lying down then standing and asks you to strain or cough, in order to estimate the size of the defect, whether the hernia reduces and what it contains. Ultrasound or cross-sectional imaging may be requested for large or recurrent hernias, or when there have been previous abdominal operations.

### 2. Preparing the patient and controlling risk factors

Chronic conditions are optimised, weight loss is advised where possible before an elective repair, and cough, constipation and urinary problems are treated, since they raise intra-abdominal pressure and strain the repair. Blood thinners are stopped only on medical instruction, and the appropriate anaesthetic is chosen.

### 3. Reaching the hernial sac

A small incision is made in the skin crease near the navel, or laparoscopic instruments are introduced through small ports. The sac is carefully separated from the skin and surrounding tissue to preserve the shape of the navel, the contents are returned into the abdomen, and the tissue is inspected.

### 4. Closing the defect and reinforcement

A small defect is closed with strong sutures; a wider defect, weak tissue or a recurrent hernia is reinforced with a synthetic mesh. The mesh is positioned in the appropriate layer and fixed so that it overlaps all the edges of the defect without excessive tension.

### 5. Closure, discharge and follow-up

The navel is re-anchored to the wall to keep as natural a shape as possible, then the skin is closed. Most elective cases go home the same day or after one night, with written activity instructions and follow-up appointments to assess the wound and swelling.

## التحضير قبل الإجراء

أخبر الطبيب بكل أدويتك، وخصوصاً مسيّلات الدم وأدوية السكري وأدوية الكورتيزون، ولا توقف أي دواء من نفسك. أخبره بأمراضك المزمنة وأمراض الكبد أو أي تجمّع سوائل في البطن وعمليات البطن السابقة والحساسية الدوائية. قبل العملية المبرمَجة: توقّف عن التدخين قدر الإمكان، وعالج الإمساك والسعال، واسأل عن خطة عملية لتخفيف الوزن إن كان مرتفعاً، فذلك يقلّل الشدّ على الإصلاح واحتمال عودة الفتق. نظّف منطقة السرّة جيداً في الحمّام صباح العملية وجفّفها، ولا تحلق شعر المنطقة بالشفرة بنفسك. التزم بالصيام الذي يحدّده الفريق، عادةً من ست إلى ثماني ساعات. أحضر تقاريرك وبطاقة التأمين، وارتدِ ملابس واسعة، ورتّب مَن يعيدك إلى البيت.

## العناية بعد الإجراء

امشِ من اليوم الأول بخطوات قصيرة متكرّرة، واسند بطنك بيدك عند السعال أو العطس أو النهوض. خُذ المسكّنات كما وُصفت، وحافظ على الجرح نظيفاً وجافاً، ولا تنقعه في حوض أو مسبح حتى يسمح لك الطبيب. توقّع تورّماً وكدمة وأحياناً تجمّع سائل بسيط تحت الجلد قد يستمرّ أسابيع ويخفّ تدريجياً، ولا تحاول عصره أو تفريغه بنفسك. تجنّب رفع الأثقال والمجهود العنيف ودفع الأثقال عادةً من أسبوعين إلى ستة أسابيع بحسب حجم الفتق وطريقة الإصلاح وتوجيه جرّاحك، وارجع للرياضة بتدرّج. أكثِر من السوائل والألياف لتجنّب الإمساك والشدّ. حافظ على وزن صحي لأن ذلك أهمّ ما يحمي الإصلاح على المدى الطويل. \*\*راجع فوراً عند: حرارة مرتفعة أو قشعريرة، ألم يزداد بحدّة، احمرار أو سخونة أو صديد أو انفتاح في الجرح، عودة انتفاخ صلب مؤلم لا يرتدّ إلى الداخل، غثيان وتقيّؤ مع توقّف خروج الريح والبراز، انتفاخ بطن متزايد، ضيق نفس أو ألم صدر، ألم وتورّم في الساق.\*\*

## About this service

Umbilical hernia repair is an operation that returns the protruding contents at the navel into the abdomen, then closes and reinforces the defect in the abdominal wall. The hernia itself is a protrusion of fatty tissue, lining or a loop of bowel through the natural weak point where the umbilical cord once passed, producing a swelling around the navel or an outward-pointing belly button that increases with standing, coughing and straining and reduces when lying down.

In adults the condition is linked to excess weight, repeated pregnancies, chronic cough, lifting, fluid accumulation in the abdomen and previous operations. Usual symptoms are a bulge with dragging or mild pain that worsens at the end of the day or after exertion, and the skin over the bulge may look stretched. A firm painful bulge that will not reduce, with nausea, vomiting or no passage of gas, is an emergency, because the contents may be trapped or strangulated.

In infants and small children the picture is different: most small umbilical hernias close on their own as the child grows during the first years, so the usual first approach is observation rather than surgery. An operation is considered if the hernia persists beyond a certain age, becomes large, or a trapping episode occurs. At every age the decision is made by clinical examination, not by reading about it.

The repair technique depends on the size of the defect and the quality of the tissues: small defects may be closed by suturing the layers, while larger or recurrent defects are reinforced with a synthetic mesh, and the repair may be done through a small incision around the navel or laparoscopically. No single technique suits everyone.

Limits of the procedure: umbilical hernia repair closes the defect and treats the protrusion. It does not tighten loose abdominal skin, does not remove fat, does not reduce weight, does not necessarily treat the separation of the vertical abdominal muscles that is common after pregnancy, and does not restore the navel to exactly its former shape. A harmful myth is taping a coin or tying a band over a baby's navel: this does not close the hernia and it causes skin breakdown and infection. Continuing weight gain, an untreated chronic cough or an early return to heavy lifting all increase the chance of recurrence after any repair.

## Preparation

Tell your doctor about all your medicines, especially blood thinners, diabetes medicines and steroids, and stop nothing on your own. Report chronic illnesses, liver disease or any fluid accumulation in the abdomen, previous abdominal operations and drug allergies. Before an elective repair: stop smoking as far as possible, treat constipation and cough, and ask about a practical weight-loss plan if your weight is high, since this reduces strain on the repair and the chance of recurrence. Wash and dry the navel area carefully on the morning of surgery, and do not shave the area yourself with a razor. Follow the fasting instructions given by your team, usually six to eight hours. Bring your reports and insurance card, wear loose clothing and arrange someone to take you home.

## Aftercare

Walk from the first day in short, frequent bouts, and support your abdomen with your hand when coughing, sneezing or getting up. Take pain relief as prescribed, keep the wound clean and dry, and avoid soaking in a bath or pool until your doctor allows it. Expect swelling, bruising and sometimes a small fluid collection under the skin that may take weeks to settle; never try to squeeze or drain it yourself. Avoid heavy lifting, straining and pushing heavy objects for roughly two to six weeks depending on hernia size, repair technique and your surgeon's advice, and return to sport gradually. Take plenty of fluids and fibre to avoid constipation and straining. Maintaining a healthy weight is the single best long-term protection for the repair. \*\*Seek care immediately if you develop: high fever or shivering; sharply increasing pain; redness, warmth, pus or opening of the wound; a firm painful bulge that returns and will not reduce; nausea and vomiting with no passage of gas or stool; increasing abdominal distension; shortness of breath or chest pain; pain and swelling in the leg.\*\*
