## Procedure steps

### 1. Assessment and four-limb blood pressure

Blood pressure is measured in both arms and both legs and peripheral pulses are examined, with echocardiography of the heart and aortic valve and a CT or MR study mapping the shape and length of the narrowing and its relation to the branches. Kidney function and blood tests are reviewed before the plan is set.

### 2. Anaesthesia and femoral arterial access

General anaesthesia is used in children and local anaesthesia with sedation in many adults. A sheath is placed in an artery in the upper thigh — an artery rather than a vein in this case — and anticoagulation is given at the doctor's discretion.

### 3. Gradient measurement and aortography

The catheter is passed across the narrowing and pressure is recorded above and below it to establish its true severity, then contrast is injected to image the aorta and measure the diameter of the narrowed segment and of the healthy aorta on either side. These measurements define the balloon size precisely.

### 4. Graded balloon inflation

A stiff wire is anchored across the narrowing, the balloon is positioned and inflated briefly, sometimes starting with a smaller size and increasing in steps. Blood pressure is monitored, the gradient is re-measured after each inflation, and the site is imaged to exclude a tear in the vessel wall.

### 5. Monitoring and blood-pressure control afterwards

Haemostasis is secured at the arterial site and you are monitored in an equipped unit: blood pressure, foot pulses, leg colour and warmth, and the access site. Blood pressure may rise temporarily after repair and is controlled with medication; echocardiography and four-limb pressures are repeated before discharge.

## About this service

Balloon angioplasty of aortic coarctation treats a narrowing of the aorta at the isthmus — the segment beyond the branches to the head and left arm — from inside the vessel itself. A catheter is introduced through an artery in the upper thigh and guided to the narrowed segment, the pressure difference above and below it is measured, and a balloon of calculated size is then inflated to widen the narrow portion, without opening the chest and without stopping the heart.

Coarctation is a congenital defect that obstructs blood reaching the lower half of the body, so blood pressure in the arms rises while the leg pulses weaken. Patients may report headaches, nosebleeds, or leg pain and coldness on exertion, and the condition is often found while investigating high blood pressure at a young age. In a newborn with critical coarctation the picture is rapid circulatory deterioration requiring urgent management. Balloon dilatation is the usual first choice for \*\*recurrent narrowing after previous surgery or intervention\*\*; for native coarctation in older patients, doctors often prefer to implant a stent to support the wall after dilatation, and the choice between balloon alone and a stent depends on age, weight and the shape of the narrowing on CT imaging.

The limits must be stated plainly: \*\*dilatation relieves the mechanical narrowing but does not necessarily cure high blood pressure.\*\* Many patients still need antihypertensive medication and lifelong follow-up, because the heart and vessels adapted to the narrowing before it was repaired, and because hypertension can appear later even with a well-opened aorta. The procedure also \*\*does not treat associated abnormalities\*\* such as a bicuspid aortic valve or narrowing below the valve, and it does not remove the need for periodic aortic imaging, since the dilated segment can re-narrow or develop a localised bulge over time.

Suitability is determined by clinical examination, blood pressure measured in all four limbs, echocardiography and CT or MR imaging — not by reading. \*\*Seek emergency care immediately for: sudden severe pain in the chest, between the shoulder blades or in the upper abdomen; cold, blue or pulseless legs; sudden weakness of a limb or slurred speech; or severely raised blood pressure with headache and confusion.\*\*

## Preparation

Tell your doctor about every medicine you take, especially antihypertensives, anticoagulants and antiplatelet drugs — some are continued on the morning of the procedure and some adjusted, and that decision is the doctor's alone. Never stop a medicine yourself, and do not stop your blood-pressure medicine before the appointment. Report your kidney function if you have a recent result, and any previous reaction to X-ray contrast or iodine, since the procedure relies on contrast to measure the aorta. Report any previous aortic surgery or intervention and bring those reports and images, as they substantially change the plan. Follow the fasting period exactly, especially for children. Say if you are or might be pregnant, and arrange a companion and a stay of one night or more.

## Aftercare

Care for the groin access site: keep the dressing clean and dry, avoid lifting, deep bending and driving in the first days, and do not let a child run or jump until cleared. Drink enough fluid to help the kidneys clear the contrast unless your intake is restricted. \*\*Go to the emergency department immediately for: sudden severe pain in the chest, between the shoulder blades or in the upper abdomen; cold, blue, pulseless or severely painful legs; sudden weakness of a limb, facial weakness or slurred speech; increasing swelling, bleeding or severe pain at the access site; or severely raised blood pressure with headache, confusion or vomiting.\*\* Take your blood-pressure medicines exactly as prescribed, measure and record your pressure at home, and never stop them yourself — hypertension can persist after a successful repair. Keep long-term follow-up and scheduled aortic imaging to monitor the dilated segment for life. \*\*If a stent was implanted in your aorta, never stop or change the dose of the antiplatelet medicine prescribed to you on your own, not even for a tooth extraction or minor surgery; only the doctor who performed the procedure may decide, and every dentist and surgeon must know you have a stent.\*\*
