## Procedure steps

### 1. Assessment before starting

The operation report, ECG, echocardiogram, kidney function and medication list are reviewed, the wound, sternal stability and lungs are examined, and blood pressure, pulse and oxygen saturation are recorded. Exercise capacity is then measured with a walk test or a stress test as appropriate, so that a safe training intensity and target heart rate can be set.

### 2. Phase one in hospital

This starts on the first or second day with deep-breathing exercises and an incentive spirometer, coughing supported by a pillow held to the chest, sitting on the edge of the bed, then standing and short repeated walks. The patient is taught to stand without pulling on the arms, the limits on lifting, and the signs that mean stopping activity.

### 3. Prescribing the monitored exercise dose

Each session is prescribed with a warm-up and a cool-down, aerobic work on a treadmill or bicycle at an intensity defined by heart rate and perceived exertion, and light resistance work added at the appropriate time to protect the breastbone. Symptoms, ECG, blood pressure and oxygen saturation are monitored throughout, and intensity is progressed weekly according to response.

### 4. Education, risk factors and psychological support

Dedicated sessions cover understanding each medicine and why it continues, controlling blood pressure, blood sugar and lipids, diet, salt and weight, stopping smoking, sleep, and returning to work, driving and sexual activity. Mood and anxiety are screened with simple tools, those who need psychological support are referred, and the family is included in the plan.

### 5. Moving to long-term maintenance

At the end of the outpatient phase capacity is remeasured and compared with baseline, and a home or community programme is written with a defined intensity, duration and weekly frequency, along with training in monitoring pulse and symptoms. Cardiology follow-up and lipid and glucose testing are scheduled, with emphasis that consistency, not seasonal effort, preserves the gains.

## About this service

Cardiac rehabilitation after heart surgery is a supervised medical programme that begins in hospital after the operation and continues for weeks in the outpatient clinic. It combines graded, monitored exercise with patient education, risk-factor modification, psychological support and a review of medication adherence. It is not general fitness training: the exercise is a calculated dose prescribed after your cardiac capacity has been assessed, in the same way a medicine is prescribed.

The programme is used after coronary artery bypass grafting, valve repair or replacement, chest and aortic surgery, after some catheter procedures, and in chronic heart disease. It runs in three phases. Phase one, in hospital, focuses on deep breathing, coughing with a pillow held against the chest, sitting up and early walking, and protecting the breastbone. Phase two is monitored outpatient rehabilitation with ECG and blood pressure supervision, typically two to three sessions a week for six to twelve weeks. Phase three is lifelong maintenance of safe, regular activity.

The content is wider than exercise alone: graded aerobic work at a defined intensity, light resistance training introduced at the right time, breathing and lung-expansion exercises, and education on wound care and sternal precautions during the early weeks, such as not lifting above the permitted limit and not pulling up on the arms to stand. Blood pressure, blood sugar and lipids are brought under control, smoking is stopped completely, diet and salt are adjusted, sleep problems are treated, and returning to work, driving and sexual activity is discussed openly. Depression and anxiety are actively looked for, as both are common after heart surgery and both reduce adherence to treatment.

The limits are clear. Rehabilitation improves exercise capacity and quality of life and reduces readmission, but it does not open a new blockage, does not substitute for medication or cardiology follow-up, and does not restore pumping strength to muscle that was damaged before surgery. It is also not identical for every patient. Exercising alone after heart surgery without assessment and supervision can do harm. Eligibility and exercise intensity are decided by clinical examination and capacity testing, never by reading.

## Preparation

Bring your operation report, echocardiogram report, any recent ECG and a complete list of your medicines with their generic names and timings, plus your home blood pressure and blood sugar records if you keep them. Tell the team about any new chest pain, breathlessness, dizziness or palpitations since discharge, about any wound problem or a sensation of movement in the breastbone, and about any joint or nerve pain that limits you. Report a pacemaker or implanted defibrillator and bring its card. Take your medicines at their usual times and do not stop anything before a session on your own initiative, avoid a large meal in the two hours beforehand, and come in closed sports shoes and loose cotton clothing with a bottle of water. If you have diabetes, bring a light snack in case your blood sugar drops.

## Aftercare

Respect the sternal precautions given to you during the early weeks: do not lift above the permitted weight, do not push or pull heavy doors, do not stand up by pulling on your arms, and support your chest with a pillow when you cough. Keep the wound clean and dry and inspect it daily, walk every day for the prescribed duration split into intervals, and sleep on your back or as advised. Record your morning weight, blood pressure and pulse, continue your medicines without interruption, avoid cigarettes and waterpipe completely, and do not resume driving or work before your doctor allows it.

\*\*Stop activity and seek help immediately if there is: chest pain or tightness during exertion or at rest; increasing breathlessness; fainting or severe dizziness; sustained palpitations or a markedly irregular pulse; a weight gain of one to two kilograms in two days with leg swelling or breathlessness lying flat; fever, redness, discharge or leakage from the chest wound, or a clicking sensation in the breastbone; or pain, swelling and warmth in one leg.\*\* None of these should wait for the next scheduled visit.
