## Procedure steps

### 1. Verifying the prescription and the medicine

The nurse matches the patient's identity with the prescription and checks the drug name, the intended route, the expiry date, the integrity of the packaging, the clarity of the solution, and that the order is current and valid. Any ambiguity, or any mismatch between the order and what was brought, halts the procedure until the prescribing physician clarifies it.

### 2. Allergy screening and patient assessment

The patient is asked about previous drug allergies and exactly what reaction occurred, about anticoagulants or clotting problems, and about pregnancy, breastfeeding and chronic illness. Vital signs are taken where indicated, and the nurse asks whether the same medicine has been given before and how well it was tolerated.

### 3. Aseptic preparation and site selection

Hands are washed and gloves worn, the medicine is drawn up with a sterile single-use syringe and needle, and the correct anatomical site is chosen: the outer hip muscle or the upper arm for intramuscular injection, the abdomen or thigh for subcutaneous injection, rotating sites when doses are repeated. The skin is disinfected and allowed to dry before the needle goes in.

### 4. Giving the injection correctly

The needle is inserted at the angle and depth appropriate to the route, the medicine is delivered at the rate it requires, then the needle is withdrawn and gentle pressure applied without rubbing. The patient is asked to report at once any severe burning pain, numbness, shortness of breath or itching, since these signs stop the procedure and require assessment.

### 5. Observation, safe disposal and documentation

The patient stays under observation long enough afterwards to detect an allergic reaction, and that period is longer with the first dose of any new medicine. The needle is discarded into a rigid sharps container without unsafe recapping, and the medicine, route, site, time and any symptom that appeared are documented, along with follow-up instructions.

## About this service

Administering injections is a nursing service in which a prescribed medicine is delivered through a needle into muscle, under the skin, into a vein or into the skin itself, depending on what the drug requires and how quickly it needs to act. It is not a nursing decision: everything begins with \*\*a physician's prescription and a written order\*\* naming the medicine, the route, the timing and the length of the course. A licensed nurse carries out that order, monitors the patient and documents the care, but \*\*does not diagnose, does not change a dose and does not substitute one medicine for another\*\*, even when the prescribed one is unavailable.

A common belief needs correcting: an injection is not automatically stronger than a tablet. Some medicines are injected because they are destroyed in the stomach, are poorly absorbed, or because the patient cannot swallow, or the situation calls for a faster onset. In many other cases an oral tablet is entirely equivalent and carries less risk. Likewise, vitamin or so-called energy injections do not treat fatigue whose cause has never been investigated, do not build immunity, and are no substitute for the tests a physician requests.

What distinguishes a competent nurse is what happens before and after the needle: verifying patient identity, the medicine, its expiry and the intended route; asking about drug allergies, blood-thinning treatment and clotting disorders; selecting the correct anatomical site so that nerves and vessels are avoided; and then observing the patient long enough afterwards to catch an allergic reaction.

\*\*Hygiene and sterile technique here are not a formality.\*\* Needles and syringes are strictly single use, no medicine or syringe is ever shared between two people, and sharps go into a dedicated rigid container. These measures are what prevent injection-site abscess, bloodborne infection and \*\*bloodstream infection, or sepsis\*\*, which can begin at one small contaminated site. Because Jordan's home-care market includes unqualified providers, \*\*verify the licence of the nurse and of the centre or company employing them\*\* before allowing anyone to give an injection in your home, and ask that every dose given be documented.

## Preparation

Bring the original prescription and the medicine in its original packaging, and store it exactly as the label instructs, since some products spoil outside refrigeration. Tell the nurse everything you take, including supplements and herbal products, and describe any previous drug allergy and precisely what happened to you at the time. Mention blood-thinning treatment, any clotting disorder or low platelets. Say whether you are pregnant or breastfeeding, and whether you live with a chronic condition such as diabetes, heart or kidney disease. All of this belongs before the needle, not after it. Sit or lie comfortably so the injection site is exposed, never use a medicine prescribed for someone else, and do not buy injectable medicines without a prescription. \*\*Do not let anyone give an injection in your home before you verify their licence and the licence of the centre or company they work for\*\*, and ask that every dose be entered in a written record you keep a copy of.

## Aftercare

Stay where you are, under observation, for as long as the nurse specifies, especially after the first dose of a new medicine, and do not drive yourself away immediately if the drug can cause dizziness or drowsiness. Mild pain, slight redness or a small firm lump at the site is common and settles within a day or two; gently moving the limb and a brief cold pack help. Do not rub the site hard and do not apply household substances to it. Keep a record of your dose appointments, and never double up a missed dose on your own initiative: ask the physician. Seek care immediately if you develop: \*\*difficulty breathing, swelling of the face or lips, a spreading rash or severe dizziness after the injection\*\*, \*\*high fever or chills\*\*, \*\*hot tender swelling, increasing hardness or pus at the injection site\*\*, \*\*numbness, weakness or burning pain spreading down the limb\*\*, \*\*any change in level of consciousness\*\*. In home care, \*\*altered consciousness, breathing difficulty, high fever, or bleeding from a catheter or tube site means urgent contact or emergency care, not waiting for the next visit\*\*.
