تُظهر الأشعة السينية بكفاءة: الكسور والخلوع وتغيّرات المفاصل التنكسية، والتهاب الرئة والسوائل حول الرئة وحجم القلب ومواضع الأنابيب والأجهزة داخل الصدر، وانسداد الأمعاء أو الهواء الحر في البطن، والأجسام الغريبة المعدنية، وتسوّس الأسنان والجذور والعظم المحيط بها، ومتابعة التحام الكسور بعد الجبس أو الجراحة.

وما لا تُظهره لا يقلّ أهمية: الأشعة السينية لا تُصوّر الأربطة والأوتار والغضاريف والحبل الشوكي بتفصيل، فهي لا تُشخّص الانزلاق الغضروفي ولا قطع الرباط الصليبي ولا تمزّق الكفة المدوّرة. وصورة الصدر الطبيعية لا تنفي جلطة رئوية ولا نوبة قلبية ولا ورماً مبكراً. وبعض الكسور — كالكسر في العظم الزورقي بالرسغ وكسور الإجهاد — لا تظهر في الصور الأولى وقد تحتاج إعادة تصوير بعد أيام أو فحصاً مختلفاً. والصورة لا تقيس الألم ولا شدّته، ولا تُثبت أن التغيّر العظمي الظاهر فيها هو سبب شكواك.

ولهذا يجب أن تُفهم القاعدة الأهم: \*\*الأشعة أداة تشخيصية وليست تشخيصاً بذاته.\*\* التغيّرات التنكسية في الفقرات والمفاصل شائعة عند أشخاص بلا أي ألم، وفي المقابل قد تكون الصورة طبيعية عند مريض حالته خطيرة. التقرير يُقرأ إلى جانب شكواك والفحص السريري والتحاليل، والطبيب المُحوِّل هو من يجمعها؛ فلا تبنِ قراراً علاجياً على تقرير قرأته وحدك.

أما الإشعاع فجرعة الصورة السينية البسيطة منخفضة، لكن المبدأ المهني أن يكون لكل صورة داعٍ سريري واضح وأن تُستخدم أقل جرعة كافية، وأن تُحمى الأعضاء الحسّاسة عند الأطفال، وألّا تُكرَّر الصور بلا ضرورة. والأشعة السينية الرقمية هي المبدأ نفسه لكن بكاشف رقمي بدل الفيلم، مع صور تُعالج وتُشارك إلكترونياً. وقبل أي صورة: أخبري فريق الأشعة إن كنتِ حاملاً أو يحتمل أن تكوني حاملاً.

## خطوات الإجراء

### 1. الإحالة وتحديد الداعي السريري

يحدّد الطبيب المُحوِّل المنطقة المطلوبة وعدد الزوايا والسبب السريري، وهذا يمنع صوراً غير ضرورية ويوجّه فنّي الأشعة إلى الوضعيات الصحيحة؛ فالصورة الخاطئة الزاوية قد تُخفي كسراً موجوداً.

### 2. التحقّق من الحمل وإزالة المعادن

تُسأل المريضة في سنّ الإنجاب عن احتمال الحمل قبل التصوير، ويُطلب خلع الحلي والساعات والأحزمة والملابس ذات المعدن وارتداء رداء الفحص، لأن المعدن يظهر في الصورة ويحجب ما تحته.

### 3. التوضيع والتصوير

يضع الفنّي المنطقة المطلوبة في وضعية محدّدة أمام الكاشف، ويُطلب منك الثبات وربما حبس النفَس ثوانٍ؛ والتصوير نفسه لا يستغرق أكثر من أجزاء من الثانية ولا تشعر به إطلاقاً.

### 4. التحقّق من جودة الصورة

تُراجَع الصورة فوراً للتأكد من وضوحها وتغطيتها المنطقة كاملة، وقد تُعاد صورة أو تُضاف زاوية أخرى، وهذا ليس خطأً بل جزء من الحصول على تشخيص صحيح.

### 5. تقرير الطبيب المختص وربطه بالفحص

يقرأ طبيب الأشعة الصور ويكتب تقريراً وصفياً، ثم يعود التقرير إلى طبيبك المُحوِّل ليجمعه مع الفحص السريري والتحاليل؛ وهنا فقط تتحوّل الصورة إلى تشخيص وقرار علاجي.

## Procedure steps

### 1. Referral and clinical justification

The referring doctor specifies the region, the number of views and the clinical reason. This prevents unnecessary exposures and guides the radiographer to the correct positioning — a film taken at the wrong angle can hide a real fracture.

### 2. Pregnancy check and removing metal

Women of childbearing age are asked about possible pregnancy before exposure, and jewellery, watches, belts and clothing with metal are removed in favour of a gown, because metal appears on the image and obscures what lies beneath it.

### 3. Positioning and exposure

The radiographer places the region in a defined position against the detector and asks you to keep still and perhaps hold your breath for a few seconds. The exposure itself lasts a fraction of a second and is not felt at all.

### 4. Checking image quality

The image is reviewed immediately to confirm that it is sharp and covers the whole region, and a view may be repeated or an extra angle added. This is not an error but part of reaching a correct diagnosis.

### 5. Radiologist report and clinical correlation

A radiologist reads the images and issues a descriptive report, which returns to your referring doctor to be combined with examination findings and laboratory results. Only at this point does an image become a diagnosis and a treatment decision.

أبلغي فريق الأشعة إن كنتِ حاملاً أو يحتمل أن تكوني حاملاً قبل التصوير، ولا تفترضي أنهم يعرفون. لا يلزم صيام قبل الأشعة السينية البسيطة ولا تُستخدم فيها صبغة، لكن بعض فحوص الصبغة المنفصلة لها تعليمات أخرى يُخبرك بها المركز. اخلع الحلي والساعة والحزام والقطع المعدنية ودبابيس الشعر وأي ملابس فيها معدن، وأخبر الفريق إن كنت تحمل جهازاً طبياً مزروعاً. أحضر صورك وتقاريرك السابقة لنفس المنطقة إن وُجدت، فالمقارنة بالقديم تكشف تغيّرات لا تُرى في صورة واحدة. أخبر الفنّي إن كنت لا تستطيع الوقوف أو الثبات أو حبس النفَس، أو إن كان لديك ألم شديد يمنع وضعية معيّنة، فهناك بدائل للوضعيات. وللأطفال: أخبر الفريق بالعمر والوزن ليُضبط الجهاز على أقل جرعة كافية.

## العناية بعد الإجراء

لا يوجد تعافٍ بعد الأشعة السينية البسيطة، وتعود إلى نشاطك وطعامك فوراً. وهنا خرافة تستحقّ التصحيح: لا يبقى إشعاع في جسمك بعد الصورة العادية، ولا تصبح مصدر إشعاع لأطفالك أو للحاضنة أو لمن حولك، ولا تحتاج شرب حليب أو سوائل أو أطعمة معيّنة «لطرد الإشعاع». احتفظ بالصور والتقرير — نسخة رقمية أفضل — واحملها معك إلى طبيبك المُحوِّل وإلى أي طبيب آخر لاحقاً، فهذا يوفّر عليك تصويراً مكرّراً. لا تبدأ علاجاً ولا توقف دواءً بناءً على قراءتك للتقرير وحدك، فعبارات مثل «تغيّرات تنكسية» أو «بروز قرص» شائعة جداً عند أشخاص بلا أعراض. \*\*راجع الطوارئ فوراً — ولا تنتظر تقريراً أو موعداً — عند: ألم صدر أو ضيق نفَس أو نفث دم، تفاقم الألم بعد إصابة أو ظهور تشوّه في الطرف، شحوب أو برودة أو تنميل في الأصابع داخل الجبس، ضعف في طرف أو فقد إحساس أو فقد تحكّم في البول أو الغائط بعد إصابة الظهر، أو ارتفاع حرارة مع ألم موضعي شديد.\*\* وتذكّر أن الصورة الطبيعية لا تُطمئنك إن كانت أعراضك تسوء — راجع طبيبك مرة أخرى.

## About this service

An X-ray examination passes a beam of ionising radiation through the body onto a digital detector or film, producing a two-dimensional image in which tissues appear according to their density: bone absorbs more of the beam and appears white, air appears black, and soft tissues fall between them in shades of grey. It is the fastest, most widely available and least expensive imaging test — and, in return, what it is able to show is strictly limited.

X-rays demonstrate well: fractures, dislocations and degenerative joint change; pneumonia, fluid around the lung, heart size and the position of tubes and devices inside the chest; bowel obstruction or free air in the abdomen; metallic foreign bodies; dental decay, roots and the surrounding bone; and healing of fractures after casting or surgery.

What an X-ray does not show matters just as much. It does not depict ligaments, tendons, cartilage or the spinal cord in detail, so it does not diagnose a herniated disc, a torn cruciate ligament or a rotator cuff tear. A normal chest film does not rule out a pulmonary embolism, a heart attack or an early tumour. Some fractures — such as a scaphoid fracture in the wrist and stress fractures — are invisible on the first images and may need repeat films after several days or a different test altogether. And an image cannot measure pain or its severity, nor prove that the bony change visible on it is the cause of your symptoms.

This leads to the single most important principle: \*\*imaging is a diagnostic tool, not a diagnosis in itself.\*\* Degenerative changes in the spine and joints are common in people with no pain at all, while conversely the film can look normal in someone who is seriously unwell. A report is read alongside your symptoms, physical examination and blood tests, and it is the referring doctor who brings these together — so do not act on a report you have read on your own.

As for radiation, the dose from a plain X-ray is low, but the professional principle is that every image must have a clear clinical justification, the lowest sufficient dose is used, sensitive organs are shielded in children, and films are not repeated without need. Digital radiography uses the same principle with a digital detector instead of film, allowing images to be processed and shared electronically. Before any image: tell the radiography team if you are pregnant or might be.

## Preparation

Tell the radiography team if you are pregnant or might be, before any exposure — do not assume they already know. Fasting is not required for a plain X-ray and no contrast is used, although separate contrast studies have their own instructions which the centre will give you. Remove jewellery, watches, belts, metal objects, hair clips and any clothing containing metal, and mention any implanted medical device. Bring previous images and reports of the same region if you have them, because comparison with older studies reveals changes that a single film cannot show. Tell the radiographer if you cannot stand, keep still or hold your breath, or if pain prevents a particular position — alternative projections exist. For children, give the team the age and weight so the equipment can be set to the lowest sufficient dose.

## Aftercare

There is no recovery period after a plain X-ray; you return to normal activity and eating immediately. One myth deserves correction here: no radiation remains in your body after a standard film, you do not become a source of radiation to your children or anyone near you, and you do not need milk, extra fluids or special foods to 'flush radiation out'. Keep the images and the report — a digital copy is best — and take them to your referring doctor and to any doctor you see later, which spares you repeated imaging. Do not start or stop treatment based on your own reading of the report: phrases such as 'degenerative changes' or 'disc bulge' are extremely common in people without symptoms. \*\*Go to emergency care immediately — without waiting for a report or an appointment — if you have: chest pain, breathlessness or coughing blood; worsening pain after an injury or visible deformity of a limb; pale, cold or numb fingers inside a cast; weakness, loss of sensation or loss of bladder or bowel control after a back injury; or fever with severe localised pain.\*\* Remember that a normal image is not reassurance if your symptoms are getting worse — go back to your doctor.
