## خطوات الإجراء

### 1. التقييم والترشيح

## Procedure steps

### 1. Assessment and eligibility

A clinical interview documents your symptoms and previous treatments, alongside detailed questions about implants or metal in the head and neck, cardiac pacemakers, any history of seizures or head injury, and pregnancy. This step determines whether the procedure is appropriate for you at all.

### 2. Motor threshold measurement and targeting

At the first visit the coil is placed over defined areas until a small movement response is observed in the hand. Your individual stimulation intensity is calculated from this, and the coil position is marked precisely so it can be reproduced at every session. This calibration takes longer than subsequent visits.

### 3. The treatment session

You sit awake wearing earplugs to protect your hearing while pulses are delivered in repeated trains with short rest intervals. You hear clicking and feel tapping, and slight twitching of facial or jaw muscles is common. No anaesthesia is needed and you do not require someone to take you home.

### 4. The treatment course and monitoring

Sessions are usually given five days a week over several weeks. Your clinician reviews symptoms with standard rating scales and checks for side effects, and may adjust intensity, target or the number of sessions according to your response rather than following one fixed schedule for everyone.

### 5. Maintaining gains after the course

## About this service

Transcranial magnetic stimulation (TMS) is a non-surgical procedure in which repeated magnetic pulses are delivered to a specific area of the brain's cortex to modulate the activity of neural circuits involved in mood regulation. It is carried out while you are fully awake, with no anaesthesia and no hospital admission.

It is usually considered for a person living with depression that has persisted despite an adequate treatment plan, or for someone who cannot tolerate the side effects of medication. Protocols differ in target site and intensity for other conditions such as obsessive-compulsive disorder. Suitability is decided by clinical assessment and a review of your medical history and any metal implants in the head — never by reading an article or taking an online test.

The session itself is straightforward. You sit in a chair, the coil is positioned over the scalp after your motor threshold is measured so the intensity can be set individually, and you hear a repeated clicking sound while feeling a tapping sensation on the head. You can return to work and drive the same day. The most common complaints are mild scalp discomfort or a headache that eases during the first week of the course.

Improvement is gradual and is tracked with symptom rating scales at follow-up visits rather than by how you feel immediately after a single session, so the course is completed even if the difference is not obvious at the start.

It is equally important to understand what TMS does not do. It is not a replacement for psychotherapy or for a medication plan, it does not work from the first session, it does not erase memories or alter personality, and it is not electroconvulsive therapy — there is no anaesthesia and no intentionally induced seizure. It also cannot guarantee that symptoms will never return, which is why a clear follow-up plan is agreed once the course ends.

## Preparation

Before your first session, tell the clinician about any metal or implant in the head or neck — plates, vascular clips, cochlear implants, neurostimulators — about a cardiac pacemaker, about any previous seizure, head injury or brain surgery, and about pregnancy or the possibility of it. Give a complete list of everything you take, including supplements and herbal products, and do not stop any medication on your own before starting. Sleep properly the night before and reduce caffeine on the day, since sleep loss and excess stimulants increase neural excitability. No fasting, anaesthesia or escort is required. Remove earrings, pins and metal clips, tell the team if you use a hearing aid, wear comfortable clothing, and arrive with time to spare because the first visit is longer due to calibration.

## Aftercare

You can return to work and drive the same day.
