## Procedure steps

### 1. Timing and excluding pregnancy

The appointment is set for the first half of the cycle, after bleeding has stopped and before ovulation. You are asked to avoid unprotected intercourse from the start of the cycle, or to have a pregnancy test beforehand, because pregnancy is an absolute contraindication.

### 2. Preparation before the study

Iodine contrast allergy, thyroid disease, asthma and previous pelvic infection are reviewed. A painkiller before the study or prophylactic antibiotics may be prescribed according to protocol and your history, and you are told that period-like cramping is expected.

### 3. Placing the cannula in the cervix

You lie on the X-ray table in the gynaecological position, a speculum is inserted and the cervix is cleaned, then a fine cannula or small catheter is passed into the cervical canal. This is the most uncomfortable moment and it lasts only seconds.

### 4. Contrast injection and sequential imaging

Contrast is injected slowly and sequential images show the uterine cavity filling, then the passage of contrast along both tubes and its spill into the pelvis. You may be asked to change position slightly for clearer angles; cramping peaks at the moment of injection and then eases.

### 5. Reporting and the next plan

A report describes the shape of the uterine cavity and the behaviour of both tubes, then is read together with your history, your results and your partner's assessment to decide the next step: surveillance, hysteroscopy or laparoscopy, or an assisted conception plan. The decision is never based on the image alone.

## About this service

Hysterosalpingography (HSG) is an X-ray examination in which contrast material is injected through the cervix so that the inside of the uterine cavity and the course of both fallopian tubes become visible. The doctor watches on screen for contrast spilling from the end of the tube into the pelvic cavity, which indicates that the tube is patent. It is a short diagnostic study performed in a radiology department or an equipped clinic and does not normally require general anaesthesia.

Its commonest indications are the assessment of delayed conception to determine whether the tubes are open; assessment of the shape of the uterine cavity in recurrent miscarriage or when a uterine septum, a duplicated uterus or intrauterine adhesions are suspected; evaluation of a fibroid or polyp indenting the cavity; and follow-up after tubal surgery or intrauterine procedures.

Its limits are fundamental and should be understood before the test. HSG shows whether a tube is open or blocked, but it \*\*does not assess tubal function\*\* — the movement of the cilia and the tube's ability to pick up and transport an egg — so a patent tube is not necessarily a working tube. It does not detect adhesions around the pelvis or endometriosis, for which laparoscopy is far more accurate. It does not assess the ovaries or ovarian reserve and does not prove that ovulation occurs; those need hormone tests and ultrasound. And it does not examine semen at all — assessing delayed conception is a shared pathway for both partners, not a single test performed on the woman. In addition, apparent blockage of one tube may represent temporary spasm rather than true obstruction, and may need re-evaluation or a further test before any decision is built on it.

Timing and safety are the points settled in advance. The study is usually performed in the first half of the cycle, after menstrual bleeding has stopped and before ovulation — commonly around days five to twelve — mainly to avoid an early pregnancy being present. It is not performed in pregnancy or suspected pregnancy, in active pelvic infection or abnormal discharge, or during heavy vaginal bleeding. Tell the team about any previous reaction to iodine-based contrast and about thyroid disease and asthma. Your doctor may advise a painkiller beforehand or prophylactic antibiotics in selected cases, according to the centre's protocol and your history — never on your own initiative.

Finally, this examination is a tool read alongside your history, your laboratory results and your partner's assessment. It does not by itself diagnose the cause of delayed conception, and it is not a treatment for infertility. Whether and when you should have it is decided by clinical assessment and your doctor's follow-up, not by reading a page.

## Preparation

Note the first day of your period and tell the centre so the appointment falls correctly in the first half of your cycle. Avoid unprotected intercourse from the start of that cycle until the test, or have a pregnancy test as instructed, since pregnancy is an absolute contraindication. Tell the team about any previous reaction to iodine contrast or to medicines, about thyroid disease or asthma, about previous pelvic infection or any current discharge, itching or unusual odour, and about all your medications including blood thinners. Ask your doctor whether to take a painkiller beforehand and how long before, and do not take anything on your own initiative. Fasting is not usually required unless you are told otherwise. Bring previous reports and images, hormone results and your partner's semen analysis if available, bring a sanitary pad because some contrast leakage and spotting are expected, and arrange for someone to drive you if you expect troublesome cramping.

## Aftercare

Expect period-like cramping for a few hours that gradually settles, along with some contrast leakage or spotting over a day or two. Use a sanitary pad rather than a tampon, and follow your doctor's advice on intercourse, swimming and bathing during the first days. You can usually return to work the same or the next day depending on how you feel, and may take a simple painkiller if your doctor allows. Do not rely on your own reading of the report: the word 'blockage' in one tube may represent temporary spasm, a patent tube is not necessarily a functioning one, and the next treatment step is decided by your doctor after both partners' results are together. \*\*Seek care promptly if you develop: fever or chills; pelvic pain that increases after the first 24 hours or severe pain not relieved by painkillers; foul-smelling discharge; bleeding heavier than a normal period; dizziness or fainting; or rash, itching, facial swelling or breathlessness after the contrast.\*\*
