يعتمد التقييم على ثلاث ركائز: قصة مرضية مفصّلة تشمل بداية الحرارة ونمطها اليومي، والسفر، والمهنة، والتعرّض للحيوانات، وتناول الحليب أو الأجبان غير المبسترة، والعمليات أو الأجهزة المزروعة في الجسم، والأدوية والمكمّلات؛ ثم فحص سريري شامل من الرأس إلى القدمين يبحث عن مصدر صامت مثل خرّاج في السن أو التهاب في الصمام أو تضخّم في العُقد؛ ثم فحوص متدرّجة تُطلب على مراحل بحسب ما يرجّحه الفحص، وليست قائمة واحدة تُطلب للجميع.

\*\*لا يُعالج المضاد الحيوي الحرارة نفسها، ولا يعالج الفيروسات مثل الرشح والإنفلونزا.\*\* والأخطر أن تناول مضاد حيوي بلا وصفة قبل أخذ العيّنات قد يُخفي البكتيريا المسبّبة ويجعل المزارع سلبية، فيتأخّر التشخيص الصحيح أسابيع. \*\*كما أن إيقاف المضاد الحيوي قبل إتمام المدة الموصوفة يُنتج بكتيريا مقاومة\*\*، وشراؤه بلا وصفة أو من بقايا علبة قديمة ضرر يمتدّ إلى المجتمع كله لأنه يُفقد الأدوية فعاليتها للجميع.

من المهم معرفة حدود هذا المسار: التقييم قد لا ينتهي بتشخيص من الزيارة الأولى، وقد يحتاج فحوصاً متكرّرة أو تصويراً موجّهاً أو أخذ نسيج للفحص، وفي جزء من الحالات تهدأ الحرارة تلقائياً قبل معرفة السبب. كما أن نتيجة تحليل واحدة لا تُفسَّر بمعزل عن الصورة السريرية، ولا تُقرأ الأرقام المخبرية ذاتياً. الترشيح لأي خطوة تشخيصية أو علاجية يُحدَّد بالفحص السريري لدى الطبيب، لا بالقراءة على الإنترنت ولا بمقارنة حالتك بحالة غيرك.

## خطوات الإجراء

### 1. القصة المرضية وخريطة التعرّض

يسألك الطبيب عن تاريخ بداية الحرارة ونمطها خلال اليوم، والأعراض المصاحبة مثل التعرّق الليلي وفقدان الوزن وألم المفاصل، والسفر خلال العام الماضي، والتعرّض للمواشي أو الحليب غير المبستر، والعمليات السابقة والأجهزة المزروعة، وكل دواء أو مكمّل تتناوله، وأي مضاد حيوي أخذته حديثاً ومتى.

### 2. الفحص السريري الشامل

فحص منظّم من الرأس إلى القدمين يشمل الجلد والأسنان والجيوب والحلق والعُقد اللمفية والقلب والرئتين والبطن والمفاصل وموضع أي جراحة سابقة، بحثاً عن مصدر صامت للعدوى أو للالتهاب. هذا الفحص هو ما يوجّه الفحوص لاحقاً ويمنع طلب تحاليل عشوائية بلا فائدة.

### 3. فحوص المرحلة الأولى

تُطلب عادة صورة دم كاملة ودلائل الالتهاب ووظائف الكبد والكلى وتحليل بول، ومزارع دم وبول تُؤخذ قبل أي مضاد حيوي، وصورة للصدر. تُختار الفحوص بحسب ما رجّحه الفحص السريري، وتُقرأ النتائج مجتمعة لا كل رقم على حدة.

### 4. فحوص متقدمة موجّهة

إذا بقي السبب غامضاً تُضاف فحوص مناعية أو مصلية موجّهة لأمراض معيّنة، أو تصوير مقطعي أو تصوير للقلب بالموجات، أو فحص للعُقد أو أخذ نسيج للفحص المجهري. تُطلب هذه الخطوات لسبب محدّد، ولا تُجرى جميعها على كل مريض.

### 5. خطة العلاج والمتابعة

يُبنى العلاج على السبب المُثبت لا على الحرارة وحدها: عدوى بكتيرية تحتاج مضاداً موجّهاً لمدة محدّدة تُكمَل كاملة، وحالة فيروسية تحتاج رعاية داعمة، وسبب التهابي أو دوائي يحتاج مساراً مختلفاً تماماً. تُحدَّد مواعيد متابعة لتقييم الاستجابة وتعديل الخطة.

## Procedure steps

### 1. History and exposure map

Your doctor asks when the fever started and how it behaves through the day, associated symptoms such as night sweats, weight loss or joint pain, travel in the past year, contact with livestock or unpasteurised milk, previous surgery and implanted devices, every medicine or supplement you take, and any antibiotic taken recently and when.

### 2. Full clinical examination

A systematic head-to-toe examination covering skin, teeth, sinuses, throat, lymph nodes, heart, lungs, abdomen, joints and any previous surgical site, searching for a silent source of infection or inflammation. This examination is what directs the later tests and prevents random, unhelpful investigations.

### 3. First-line investigations

This usually includes a complete blood count, inflammatory markers, liver and kidney function, urine analysis, blood and urine cultures taken before any antibiotic, and a chest image. Tests are chosen according to what the examination suggested, and results are read together as a picture rather than number by number.

### 4. Targeted advanced testing

If the cause remains unclear, targeted immune or serological tests for specific diseases may be added, along with cross-sectional imaging, an ultrasound study of the heart, examination of lymph nodes, or a tissue sample for microscopy. Each of these steps is requested for a specific reason and not performed on every patient.

### 5. Treatment plan and follow-up

Treatment is built on the proven cause, not on the fever alone: a bacterial infection needs a targeted antibiotic for a defined course that must be completed, a viral illness needs supportive care, and an inflammatory or drug-related cause needs an entirely different route. Follow-up appointments are set to assess response and adjust the plan.

## العناية بعد الإجراء

Persistent or recurrent fever is a raised body temperature that continues or keeps returning over several weeks without a clear cause after the initial examination and basic tests. It is a symptom rather than a diagnosis, and it needs a structured diagnostic pathway. Fever of this kind may point to a hidden infection, an inflammatory or immune-related condition, a reaction to a medicine, or a non-infectious cause altogether. For that reason the assessment begins with careful history-taking, long before any medicine is considered.

The evaluation rests on three pillars. First, a detailed history: when the fever began, its daily pattern, recent travel, occupation, contact with animals, consumption of unpasteurised milk or cheese, recent surgery or implanted devices, and every medicine or supplement being taken. Second, a head-to-toe clinical examination looking for a silent source such as a dental abscess, a heart valve infection, or enlarged lymph nodes. Third, staged investigations requested in steps according to what the examination suggests, rather than one fixed list of tests ordered for everyone.

\*\*An antibiotic does not treat fever itself, and it does not treat viruses such as the common cold and influenza.\*\* More importantly, taking an antibiotic without a prescription before samples are collected can mask the responsible bacteria and turn cultures negative, delaying the correct diagnosis by weeks. \*\*Stopping an antibiotic before completing the prescribed course breeds resistant bacteria\*\*, and buying antibiotics without a prescription or using leftovers from an old pack causes harm that reaches the whole community, because it strips these medicines of their effectiveness for everyone.

It is important to understand the limits of this pathway. The assessment may not produce a diagnosis at the first visit; it can require repeated tests, targeted imaging, or a tissue sample. In a proportion of cases the fever settles on its own before the cause is identified. A single test result is never interpreted in isolation from the clinical picture, and laboratory numbers should not be read by patients on their own. Eligibility for any diagnostic or treatment step is decided by a clinical examination with your doctor, not by reading online and not by comparing your illness with someone else's.

## Preparation

Keep a temperature diary before your visit: date, time and reading, noting whether the fever rises at a particular time of day. Bring a list of every medicine and supplement you take, and tell your doctor clearly if you have taken any antibiotic in recent weeks and when, because this changes how culture results are interpreted. Gather previous test results, imaging, and reports of any operation or implanted device. Write down where you travelled in the past year and any contact with animals, unpasteurised milk or cheese, or insect bites. Do not start an antibiotic on your own before the visit. Ask in advance whether fasting is needed for some tests, and wear clothing that makes blood sampling and examination easy.

## Aftercare

Keep recording your temperature daily and complete the requested tests in the order your doctor set, because delaying a single sample can send the whole pathway back to the start. Drink enough fluids, rest, and use fever-reducing medicine only as your doctor directed. Do not take an antibiotic on your own, and if one is prescribed, finish the full course even if you feel better quickly. Go to the emergency department immediately if any of the following appears: \*\*fever with a stiff neck, light sensitivity or an unusually severe headache — possible meningitis\*\* · \*\*a rash that does not fade when you press on it\*\* · \*\*rapid breathing, confusion or extreme drowsiness — possible sepsis\*\* · \*\*a high, persistent fever in a young child or in a person with a weakened immune system\*\* · \*\*a seizure, vomiting that prevents drinking, or passing no urine\*\*.
