## Procedure steps

### 1. Handover and reading the discharge plan

The nurse begins with the discharge sheet and operative report: the type of surgery, the state of the wound, the presence of tubes or drains, the prescribed medicines and their timing, restrictions on movement and bathing, and the follow-up appointments. A baseline of vital signs and pain score is recorded so any later change can be measured against it.

### 2. Daily assessment and complication surveillance

Vital signs are measured and pain, consciousness, breathing and the wound are assessed. The legs are checked for the pain, swelling or warmth that suggest a venous clot, and urination, bowel movement and tolerance of food and fluids are tracked. Any deviation from the baseline is documented and reported to the responsible physician without waiting for the next visit.

### 3. Wound, tube and drain care

The wound is redressed using aseptic technique at the frequency the physician ordered, with careful description of any discharge, its colour and odour, and the condition of the wound edges. Where a drain or catheter is present, output is measured and the tube kept secured and its site clean, because sterile technique here separates quiet healing from an infection that reaches the blood.

### 4. Mobility, breathing and nutrition

The patient is encouraged to get up and take short frequent walks within the limits the physician allows, is taught deep-breathing and supported-cough exercises to reduce pneumonia, and has fluid and protein intake monitored, since poor nutrition slows wound healing. Where mobility is limited, a repositioning schedule and daily skin inspection are put in place.

### 5. Documentation, communication and handing care back

Every assessment, medicine and dressing change is documented in a record that stays with the patient, and the surgeon is contacted at any worrying sign. As recovery progresses, the patient and family are taught to take over: how to watch for warning signs, how to protect the wound, and when to go to the emergency department instead of waiting for the next visit.

## About this service

Post-operative care is a structured nursing service delivered at home or in a clinic after hospital discharge. Its purpose is to follow the wound, the vital signs, mobility, nutrition and prescribed medicines, and to catch complications early, before they escalate. The first couple of weeks after any operation are not a period of passive rest but a period of surveillance: infection, fluid collections, venous clots, bowel and urinary problems most often surface in this window, and the difference between a complication settled in clinic and one that ends in readmission is usually the number of days it went unnoticed.

The service is built on \*\*a physician's order and a written care plan\*\* drawn from the discharge sheet: the type of operation, instructions for the wound, tubes and drains, medication times, and what is permitted or forbidden in terms of movement, lifting and bathing. A licensed nurse follows that plan, assesses daily, documents and escalates at the first deviation, but \*\*does not diagnose, does not change any dose, does not stop an antibiotic and does not adjust pain medication\*\* on their own authority.

The limits of the service deserve stating plainly. Home nursing does not replace attending your physician's follow-up appointments, nor the blood tests and imaging they request, nor emergency care if you deteriorate. It also does not speed up healing in itself; what it does is reduce the chance of complications and keep recovery on its normal course. Early walking and breathing exercises remain among the strongest measures against post-operative clots and pneumonia, and no medicine substitutes for them.

If the patient is bedbound or has limited mobility after surgery, pressure injury is a genuine risk, and \*\*prevention is the treatment\*\*: regular repositioning, daily skin inspection, moisture control and adequate nutrition. Wherever a catheter or drain is present, \*\*hygiene and sterile technique are what prevent the most dangerous complication, bloodstream infection or sepsis\*\*. Because Jordan's home-care market includes unqualified providers, \*\*verify the licence of the nurse and of the centre or company\*\* before contracting, and insist on a written record of every visit.

## Preparation

Before the first visit, gather the discharge sheet, the operative report and all your medicines in their original packaging, and write down your doctors' names and contact numbers. Prepare a clean, well-lit space at home for dressing changes, a bed reachable from both sides, and a clear path to the bathroom free of obstacles and slippery rugs to reduce the risk of falls. Tell the nurse about diabetes, heart or kidney disease, blood thinners, steroids and drug allergies, about any tube, catheter or drain you were sent home with, and when you last passed urine or opened your bowels. Ask your physician explicitly what is allowed and what is not: when you may shower, how much you may lift, when you may return to work and driving. \*\*Do not sign a home-care contract before verifying the nurse's licence and the licence of the centre or company\*\*, and ask for the care plan in writing and for every intervention to be recorded at each visit.

## Aftercare

Take your medicines exactly as written: do not stop an antibiotic because you feel better and do not increase a painkiller because you hurt; call your physician instead. Move little and often rather than sitting for long stretches, keep up the deep-breathing exercises, and take enough fluid and protein to help the wound knit. Keep the wound clean and dry as your surgeon instructed, apply no household substances to it and do not pick off crusts. Avoid heavy lifting and straining until your physician allows it, and stop smoking, which slows wound healing. Seek care immediately if you develop: \*\*high fever or chills\*\*, \*\*difficulty breathing or chest pain\*\*, \*\*a change in level of consciousness or confusion\*\*, \*\*bleeding from the wound or from a catheter or tube site\*\*, \*\*discharge, pus or separation of the wound\*\*, \*\*pain or swelling in one leg\*\*, \*\*no urine output, or abdominal distension with vomiting\*\*, \*\*a pressure sore that is widening or draining pus\*\*. These signs mean \*\*urgent contact or emergency care, not waiting for the next visit\*\*.
