## Procedure steps

### 1. Describing and measuring the outburst

The form of the episode is recorded — screaming, dropping to the floor, kicking, throwing, head banging, breath holding — with its length and weekly frequency, and what precedes it: a refusal, moving away from a preferred activity, hunger, tiredness, crowds or noise.

### 2. Ruling out amplifiers

Sleep, nutrition and ongoing pain such as teeth or ears are reviewed, and language development is assessed: a child who cannot say "I don't want to" explodes instead of speaking. A medical or developmental referral follows if indicators appear.

### 3. Coaching parents through the moment

Parents rehearse safety first, then saying less, staying calmly nearby without bargaining, not granting the demand during the episode, and offering warmth afterwards without a lecture. The response is practised inside the session before it is used at home.

### 4. Prevention and regulation skills

Recurring triggers are adjusted: advance warning of transitions, a visual schedule, fewer simultaneous demands, and protection from hunger and fatigue. The child learns to name the feeling, ask for help, and use safe physical outlets.

### 5. Follow-up and nursery or school coordination

Progress is measured from the log rather than from impressions, and the plan is adjusted around what worked. A practical summary goes to the teacher so responses match, and the assessment is widened if episodes stay severe or if self-harm or harm to others appears.

## About this service

Managing temper tantrums is an assessment and behavioural training programme delivered by a child mental health specialist for a child who explodes into screaming, intense crying, throwing themselves on the floor, kicking or throwing things when refused or frustrated. It starts by defining the shape of the outburst, how long it lasts, what triggers it and who is present, then distinguishes tantrums that are expected at a given stage of development from those that exceed the child's age in intensity or frequency, or that end in injury to the child or someone else.

A tantrum is at heart \*\*a message rather than defiance\*\*: a small brain flooded by a feeling bigger than its capacity to regulate, so control collapses for a while. At the peak of an outburst a child cannot hear reasoning or follow instructions, because the part of the brain responsible for calm thinking is offline in that moment — which is exactly why explaining and bargaining mid-tantrum does not work. \*\*Seeing a specialist is not a stigma and not a parenting failure\*\*; it is training in a skill nobody is born with, and families who learn to manage outbursts early avoid years of daily conflict.

The programme has explicit limits. It does not produce a child who never becomes angry — anger is a normal and necessary emotion, and the goal is to reduce the intensity, length and frequency of episodes and to teach self-regulation, not to abolish the feeling. It also does not replace medical and developmental review where needed, since frequent severe outbursts can be linked to a language delay that blocks expression, sensory sensitivity, ongoing pain, or insufficient sleep. \*\*One widespread practice must be corrected: do not shut a child in a dark room, do not lock them in, and do not answer a tantrum with hitting. Fear ends today's episode, multiplies later explosions, and teaches that whoever shouts loudest wins.\*\* Equally, giving in to the demand during an outburst teaches the child that outbursts are the route that works.

Family and school are a core part of treatment: results depend on the same plan being used at home, at nursery, in the car and in the supermarket — not in the clinic. Assessment is built on information from \*\*more than one source\*\* — parents, teacher, direct observation and the episode log — never a single session. \*\*Be wary of any centre promising a fast cure or a "permanent end" to tantrums, or a programme that does not publish its method and the qualifications of whoever delivers it.\*\*

## Preparation

Before the appointment, keep a log for two weeks if possible: date and time, what happened in the minutes beforehand, the form of the outburst, its length in minutes, what adults did, and how it ended. Add sleep hours and meal times, because outbursts multiply with hunger and tiredness and that link is usually missed. With your family's agreement, and only if it is safe and not humiliating for the child, record a short clip: one minute of video replaces a long description. Bring paediatric reports, any speech or developmental report, and a brief note from the nursery or school teacher. Write down honestly what you have already tried and what happened, including what you now think was a mistake — that information saves weeks and nobody is judging you for it. Tell your child you are visiting someone who helps children when the anger inside them gets too big, and never present the visit as a punishment or use it as a threat.

## Aftercare

Protect safety first in every episode: move away anything that could injure, stay calmly nearby, and cut your words to the minimum, because explaining and bargaining mid-explosion prolong it. Do not grant the demand during the outburst even if it is small; afterwards, comfort the child without a lecture or a reminder of the wrongdoing, and name what happened: "your anger was very big and now it has passed." Praise every time the child asks for help or says what is bothering them instead of exploding. Log episodes weekly so you follow the trend rather than a single bad day, and keep the response identical across parents, carers and school. Do not use isolation in a dark room, hitting, or threats of abandonment. \*\*Seek help immediately, not an appointment weeks away, if the child talks about hurting themselves or wanting to die, injures themselves during an episode by banging their head or scratching their body, withdraws severely and stops eating and sleeping, shows a sudden collapse in school and social functioning, or if outbursts come with loss of consciousness, convulsion or blue discolouration.\*\* \*\*Any sign that the child is being abused or subjected to violence requires immediate protective action, not waiting for the next session.\*\*
