## Procedure steps

### 1. Assessing the infant before the procedure

The doctor examines the penis and the position of the urinary opening to exclude hypospadias, curvature or a buried penis, and checks the testes and for hernia. The infant's health is reviewed: weight, jaundice, any fever or infection, and the family history of bleeding or clotting disorders.

### 2. Informed consent and analgesia

The technique, alternatives and possible complications are explained to the parents and consent is signed. The infant is settled and given age-appropriate analgesia and local anaesthesia as decided by the doctor, then gently positioned on a warm clean surface while the area is disinfected.

### 3. Separating the foreskin and placing the ring

Adhesions between the foreskin and glans are gently separated, then a ring of the correct size for the penis is selected and positioned precisely over the glans. Correct sizing is fundamental to safety, since a ring that is too tight or too loose both cause problems.

### 4. Tying the ligature and trimming the excess

A strong ligature is tied around the foreskin within the groove of the ring at a calculated tension that stops perfusion and prevents bleeding, then the protruding foreskin is trimmed. The doctor confirms there is no bleeding and that the glans and urinary opening are intact before finishing.

### 5. Observation, urination check and instructions

The infant is observed for a short period, fed and comforted, and it is preferable to confirm that he has passed urine before discharge. Parents are taught home care, the warning signs and when the ring is expected to separate, and a review appointment is arranged to confirm healing and ring separation.

## About this service

Plastibell circumcision is a common technique for infant circumcision that uses a small plastic ring. The doctor separates the foreskin from the glans, places a correctly sized ring over the glans, and ties a ligature around the foreskin into the groove of the ring, which cuts off the blood supply to the excess tissue and prevents bleeding. The protruding tissue is trimmed, and the ring with its ligature then falls away on its own after several days up to about two weeks. The procedure needs no sutures and no cautery in most cases, and is performed with analgesia or local anaesthesia appropriate for the infant's age.

The reassuring nickname used for it in Arabic deserves correction: \*\*it is a popular label, not a medical claim that the procedure is painless or free of risk.\*\* The infant does feel pain and deserves adequate analgesia, and like any surgical procedure it carries possible complications: bleeding, infection, adhesions, an unsuitable ring size, or delayed separation of the ring. It should therefore be performed in an equipped setting by a qualified doctor, never at home or by an untrained person.

Its limits matter before you consent. The ring technique generally suits young infants and does not suit every age; older children and adults usually require a different surgical technique. \*\*Circumcision must be avoided or postponed if there is hypospadias, meaning the urinary opening is not in its normal position, or penile curvature, a buried penis, or any anomaly of the urethra, because the foreskin itself may be needed for later surgical repair, and removing it makes that operation harder for the child.\*\* It is also postponed in significant jaundice, infection or fever, prematurity and low weight, or when there is a family history of a bleeding disorder or any unexplained bleeding.

Afterwards the home care phase begins, and it is the most important part: cleanliness, a loose nappy, and watching for urination. \*\*Never pull the ring, and never try to remove it or cut the ligature yourself no matter how long it takes\*\*; it normally falls off unaided. If it has not separated within the expected time, or if it slips back onto the shaft of the penis, seek medical review immediately. Do not apply powders, kohl, oils or any traditional substance to the wound, and use only what the doctor has prescribed.

Whether to proceed, when, and with which technique are decided by clinical examination of the infant by a qualified doctor, not by a social date on the calendar or by other families' experiences.

## Preparation

Feed your baby before the appointment unless you were told otherwise, and bring the birth summary and vaccination card. Tell the doctor about any jaundice, fever or current infection, about the baby's weight and age, about any preterm birth, and about any bleeding or clotting disorder in the family or medicines you take if you are breastfeeding. Mention as well if you have noticed that the urinary opening is not at the tip of the penis, or that the urine stream is weak or deviated, because this information can change the decision entirely. Do not apply any cream, ointment or substance to the area beforehand. Bring spare nappies, loose clothing and a blanket. Ask the doctor in advance which analgesia suits your baby's age, how it is given and by whom, and never give your baby any medicine on your own. Arrange an early review and stay reachable by phone during the first days.

## Aftercare

Change the nappy frequently and keep the area clean and dry, cleaning with lukewarm water without rubbing or harsh soap, then patting gently dry. Keep the nappy loose, never tight, and place nothing compressive over it. Use only the ointment the doctor prescribed, and apply no powders, kohl, oils or traditional substances. Mild swelling, redness and a yellowish crust around the ring are normal during healing. \*\*Do not pull the ring and do not cut the ligature; let it fall off on its own\*\*, and watch your baby's urination, feeding and activity, noting the day the ring separates. \*\*Seek emergency care immediately for bleeding that soaks the nappy or does not stop with gentle pressure, failure to pass urine within hours of the procedure or painful dribbling, increasing swelling or spreading redness or purulent discharge or foul smell, any fever, lethargy or refusal to feed in an infant, a glans that turns blue or black, or a ring that slips back onto the shaft of the penis or remains in place beyond about two weeks.\*\*
