ما يميّزه أنه \*\*علاج عملي محدّد المدة والأهداف، لا حديث مفتوح\*\*: يُبنى على تحديد مشكلة بدقّة، وأهداف قابلة للقياس، وأدوات تُتدرَّب في الجلسة وتُطبَّق بين الجلسات — و\*\*التطبيق بين الجلسات (المهامّ) هو الجزء الفاعل فعلياً\*\*، والجلسة وحدها بلا تطبيق تُنتج فهماً بلا تغيير.

أدلّته قوية في \*\*القلق ونوبات الهلع والوسواس القهري والرهاب الاجتماعي والاكتئاب واضطراب ما بعد الصدمة والأرق\*\*، وله تطبيقات في الألم المزمن وإدارة الغضب والقولون العصبي. ومن أدواته الأساسية: تحديد الأفكار التلقائية وفحص دقّتها بالأدلّة لا بالجدل، وتفعيل السلوك في الاكتئاب (البدء بالفعل قبل انتظار الدافع)، \*\*والتعرّض التدريجي\*\* في القلق والرهاب والوسواس — وهو الأداة الأقوى وتعمل بمواجهة المخيف تدريجياً مع الامتناع عن الطقوس المُطمئنة، لأن التجنّب هو ما يُبقي الخوف حيّاً.

مدّته النموذجية محدودة — غالباً من نحو 8 إلى 20 جلسة بحسب الحالة — وهذا مقصود: الهدف أن يصبح المريض \*\*معالج نفسه\*\*، فيخرج بأدوات يستخدمها بعد انتهاء العلاج، ولهذا أثره يدوم بعده.

ويُجمع مع الدواء أو يُستخدم وحده بحسب الحالة وشدّتها — وليس بديلاً متنافساً معه. وتوقّع أن تسوء الأعراض قليلاً في بداية التعرّض قبل أن تتحسّن، وهي مرحلة متوقّعة لا فشل. أمّا الحالات الشديدة أو وجود أفكار إيذاء النفس فتستوجب تقييماً طبياً عاجلاً لا انتظار جلسات.

## خطوات الإجراء

### 1. التقييم وتحديد المشكلة بدقّة

تُوصَف الأعراض ومدّتها وأثرها، وتُحدَّد المشكلة بصيغة عملية قابلة للعمل عليها، وتُستبعد حالات تحتاج تقييماً طبياً أو دوائياً عاجلاً.

### 2. صياغة الحلقة وشرح النموذج

تُرسَم مع المريض حلقة أفكاره ومشاعره وسلوكه في مواقفه الفعلية، ليرى بنفسه كيف يُثبّت التجنّب أو الطقوس المشكلة — وهذا الفهم يُحرّك التغيير.

### 3. أهداف قابلة للقياس

تُصاغ أهداف محدّدة (سلوك يُفعَل أو موقف يُواجَه) بمقاييس تُعاد في الجلسات لقياس التقدّم بالأرقام لا بالانطباع.

### 4. الأدوات والمهامّ بين الجلسات

تُتدرَّب الأداة في الجلسة (فحص الأفكار، تفعيل السلوك، التعرّض التدريجي، تمارين تهدئة) ثم تُطبَّق بين الجلسات — وهذا التطبيق هو الجزء الفاعل.

### 5. منع النكس والإنهاء المخطَّط

تُبنى خطة مكتوبة لعلامات النكس المبكرة وما يفعله المريض عندها، ويُنهى العلاج تدريجياً ليصبح المريض معالج نفسه.

## Procedure steps

### 1. Assessment and precise problem definition

Symptoms, duration and impact are described, the problem defined in workable practical terms, and conditions needing urgent medical or pharmacological assessment excluded.

### 2. Formulating the loop and explaining the model

The patient’s thought–feeling–behaviour loop is mapped with them in their real situations so they see how avoidance or rituals maintain the problem — an understanding that drives change.

### 3. Measurable goals

Specific goals are set (a behaviour to do or a situation to face) with scales repeated across sessions so progress is measured numerically rather than by impression.

### 4. Tools and between-session homework

The tool is practised in session (thought testing, behavioural activation, graded exposure, relaxation) then applied between sessions — that application is the active ingredient.

### 5. Relapse prevention and planned ending

A written plan sets out early relapse signs and what the patient does then, and treatment is tapered so the patient becomes their own therapist.

## التحضير قبل الإجراء

دوّن قبل الجلسة الأولى: ما المشكلة كما تعيشها، ومتى بدأت، وما يجعلها أسوأ أو أفضل، وما جرّبته سابقاً وماذا نتج، \*\*وما الذي تريد أن يتغيّر فعلياً في حياتك\*\* (هدف ملموس أنفع من «أن أشعر أفضل»). اذكر كل أدويتك وأي علاج نفسي سابق واستخدامك للكافيين أو الكحول أو أي مواد. \*\*اسأل صراحة: هل المعالج مؤهّل ومُدرَّب على CBT تحديداً، وكم عدد الجلسات المتوقّع، وما المهامّ المطلوبة مني بين الجلسات، وكيف سيُقاس التقدّم\*\* — العلاج المعرفي السلوكي منظّم ومحدّد، فهذه أسئلة مشروعة. واستعدّ لأن يُطلب منك عمل بين الجلسات؛ هذا ليس عبئاً إضافياً بل هو العلاج نفسه.

## العناية بعد الإجراء

\*\*أنجز المهامّ بين الجلسات\*\* — هي الفرق بين فهم المشكلة وتغييرها. طبّق الأداة في المواقف الحقيقية لا في الذهن فقط، وسجّل ما جرى بصدق حتى ما لم ينجح، فالفشل معلومة تُعدّل الخطة. في التعرّض التدريجي: \*\*لا تهرب من الموقف قبل أن يهبط القلق، ولا تستخدم طقساً مُطمئناً\*\* — فالخروج المبكر يُعلّم الدماغ أن الهرب هو ما أنقذك ويُقوّي الخوف. توقّع تحسّناً متذبذباً وأن تسوء الأعراض قليلاً في بداية التعرّض. لا توقف دواءً نفسياً بسبب تحسّنك في العلاج النفسي إلا بقرار طبيبك. احتفظ بخطة منع النكس واستخدمها عند أول علامة، فالعودة السريعة للأدوات أسهل من انتظار التدهور. \*\*اطلب مساعدة عاجلة فوراً عند أفكار إيذاء النفس أو الانتحار، أو تدهور شديد في النوم والأكل والوظيفة، أو إن شعرت أنك غير آمن — ولا تنتظر موعد الجلسة القادمة.\*\*

## About this service

Cognitive behavioural therapy (CBT) is a short-term structured psychological treatment based on the idea that thoughts, feelings and behaviour interact in a loop: how we interpret a situation shapes how we feel, feeling directs behaviour, and behaviour reinforces the original interpretation and reproduces it. Treatment works by breaking that loop at its modifiable points.

What distinguishes it is that it is \*\*practical, time-limited and goal-directed rather than open-ended conversation\*\*: it is built on defining the problem precisely, measurable goals, and tools practised in session and applied between sessions — and \*\*the between-session application (homework) is the actually active ingredient\*\*; sessions alone without application produce understanding without change.

Its evidence is strong in \*\*anxiety, panic, obsessive-compulsive disorder, social phobia, depression, post-traumatic stress disorder and insomnia\*\*, with applications in chronic pain, anger management and irritable bowel syndrome. Core tools include identifying automatic thoughts and testing their accuracy against evidence rather than by argument; behavioural activation in depression (acting before waiting for motivation); and \*\*graded exposure\*\* in anxiety, phobia and OCD — the most powerful tool, working by facing the feared thing gradually while refraining from reassuring rituals, because avoidance is what keeps fear alive.

Its typical duration is limited — often around eight to twenty sessions depending on the condition — and that is intentional: the aim is for the patient to become \*\*their own therapist\*\*, leaving with tools they use after treatment ends, which is why its effect persists.

It is combined with medication or used alone depending on the condition and its severity — not a competing alternative. Expect symptoms to worsen slightly at the start of exposure before improving; this is an expected stage, not failure. Severe presentations or thoughts of self-harm require urgent medical assessment rather than waiting for sessions.

## Preparation

Before the first session note: what the problem is as you experience it, when it began, what makes it worse or better, what you have tried and what resulted, \*\*and what you actually want to change in your life\*\* (a concrete goal is more useful than “to feel better”). Report all medications, any previous psychological treatment, and use of caffeine, alcohol or any substances. \*\*Ask explicitly: is the therapist qualified and specifically trained in CBT, how many sessions are expected, what homework is required of me between sessions, and how will progress be measured\*\* — CBT is structured and specific, so these are legitimate questions. And be ready to be asked to work between sessions; this is not an added burden but the treatment itself.

## Aftercare

\*\*Do the between-session homework\*\* — it is the difference between understanding the problem and changing it. Apply the tool in real situations rather than only in your head, and record what happened honestly including what did not work, since failure is information that adjusts the plan. In graded exposure: \*\*do not escape the situation before anxiety subsides, and do not use a reassuring ritual\*\* — leaving early teaches the brain that escape saved you and strengthens the fear. Expect fluctuating improvement and slightly worse symptoms at the start of exposure. Do not stop psychiatric medication because you are improving in therapy except on your physician’s decision. Keep the relapse-prevention plan and use it at the first sign, since returning to the tools quickly is easier than waiting for deterioration. \*\*Seek urgent help immediately for thoughts of self-harm or suicide, severe deterioration in sleep, eating or functioning, or if you feel unsafe — do not wait for the next session.\*\*
