Radioactive Iodine Therapy

## Procedure steps

### 1. Assessment and confirming the indication

The doctor reviews the diagnosis, thyroid tests and imaging, discusses the alternatives of medication or surgery, confirms that you are not pregnant or breastfeeding, and asks about medicines, supplements and any recent contrast scan.

### 2. Dietary and medication preparation

A low-iodine diet is usually requested for a short period beforehand, and thyroid medicines may be adjusted or briefly stopped, but only on your doctor's instruction, because correct preparation is what allows the gland to take up the treatment effectively.

### 3. Administering the dose

The capsule or liquid is given in the nuclear medicine unit and swallowed with water. There is no pain and no anaesthesia. Written radiation-protection instructions are explained to you before you leave the unit.

### 4. Temporary distancing and post-dose instructions

A short stay in a dedicated room may be required for higher activities, followed by temporary distancing from children and pregnant women, sleeping alone, not sharing utensils, and drinking plenty of fluids with frequent voiding for the period the team specifies.

### 5. Follow-up and hormone adjustment

Thyroid tests are repeated after some weeks and then at regular intervals to track response, detect an underactive gland early and adjust replacement hormone; follow-up scans may be requested in cancer cases.

## About this service

Radioactive iodine therapy is a treatment given by mouth as a capsule or a liquid containing radioactive iodine. Thyroid cells are the only cells in the body that actively take up and store iodine, so the treatment concentrates there and acts locally, with limited exposure to other tissues. It is used for an overactive thyroid, whether caused by Graves' disease or by one or more overactive nodules, and also after thyroid surgery in selected cases of differentiated thyroid cancer to clear remaining thyroid tissue.

The decision is taken after a full assessment that includes clinical examination, thyroid function tests and imaging. The activity given is chosen as the smallest amount expected to be effective for your particular situation, and your doctor balances the expected benefit against radiation exposure. This is a justified, tightly regulated treatment delivered by nuclear medicine units, but it is not a routine test: it comes with before and after instructions that must be followed exactly so that it works and remains safe for the people around you.

\*\*Tell your doctor immediately if you are pregnant, breastfeeding, or think you might be pregnant. Pregnancy and breastfeeding are contraindications to radioactive iodine, a pregnancy test is usually required before the dose is given, and you will be asked to avoid conceiving for a period your doctor defines afterwards.\*\* You must also declare every medicine or supplement containing iodine, and any recent scan that used contrast dye, because these saturate the gland with iodine and weaken the result.

The limits are clear. It does not work instantly: the gland responds over weeks to months and a second treatment is sometimes needed. Many patients later become underactive and need regular thyroid hormone replacement; this is an expected outcome managed by follow-up, not a surprise. Radioactive iodine does not help thyroid cancers that do not take up iodine, and it does not treat the eye changes of Graves' disease and may worsen them in some patients, which is why this is assessed in advance. Eligibility and dose are decided by medical assessment, not by reading a page.

## Preparation

Tell your doctor if you are pregnant, breastfeeding or planning pregnancy, and never conceal a suspicion of it. List every medicine and supplement you take, especially anything containing iodine or sea-derived herbal products, any contrast dye used for a scan in recent weeks, and any kidney disease or previous thyroid surgery. Follow the low-iodine diet for exactly the period your team specifies and do not improvise it. Do not stop thyroid medication except on your doctor's instruction. Organise your home before the appointment: who will care for the children during the distancing period, a separate bedroom, and your own towel and utensils. Bring previous reports and scans, and ask how long the precautions last in your specific case and when you can return to work.

## Aftercare

Drink plenty of fluids and empty your bladder frequently in the first days to speed the clearance of what the gland did not take up, flush the toilet and wash your hands thoroughly after every use, and wash your clothes and utensils separately for the period prescribed. Follow the temporary distancing from children and pregnant women, sleep alone and avoid long periods in crowded places as instructed by the nuclear medicine unit, and do not breastfeed after treatment. Sucking sour sweets and drinking water reduce dry mouth and salivary gland tenderness, and mild neck discomfort or a temporary change in taste can occur. \*\*Seek care immediately for painful neck swelling, difficulty swallowing or breathing, severe palpitations with fever and confusion, repeated vomiting that prevents you from drinking, or a red, painful, swollen eye with changed vision.\*\*
