## خطوات الإجراء

### 1. تقييم القدرة والمخاطر

يقيّم الممرض ما يستطيع المريض فعله بنفسه وما يحتاج مساعدة فيه، ويحدّد خطر قرح الفراش وخطر السقوط وحالة الجلد والفم والإخراج والتغذية. هذا التقييم يُنتج خطة مكتوبة تُحدّد جدول تغيير الوضعية ومواعيد النظافة والاحتياطات الخاصة، وتُراجع دورياً مع تغيّر الحالة.

### 2. الاستحمام والعناية بالجلد

يُستحمّ المريض في السرير أو الحمّام بحسب قدرته، بماء دافئ وصابون لطيف، مع تغطية ما لا يُغسل حفاظاً على الخصوصية والدفء. يُجفَّف الجلد بالتنشيف خصوصاً بين الثنيات، ويُرطَّب الجلد الجاف. تُتجنَّب الكحول والبودرة والفرك القوي، ويُفحص الجلد أثناء الاستحمام عند العجز والكعبين والوركين.

### 3. العناية بالفم والشعر والأظافر

تُنظَّف الأسنان واللسان مرّتين يومياً على الأقل، وتُعتنى بالفم أكثر عند من يتغذّى بأنبوب أو يتنفّس بفمه لأن جفاف الفم يُسبّب التهاباً وقد يُسهم في التهاب رئوي. يُغسل الشعر بشكل دوري، وتُقلَّم الأظافر بحذر شديد عند مرضى السكري أو ضعف التروية، أو تُترك لمختصّ.

### 4. المساعدة في الإخراج والحفاظ على الكرامة

يُساعد المريض في الوصول إلى الحمّام أو استخدام كرسي المرحاض أو الحفاظات بحسب حالته، مع تنظيف فوري بعد كل تبلّل لأن البلل المستمر يُرقّق الجلد ويُهيّئ للقرح. تُحترم الخصوصية بإغلاق الباب والتغطية والشرح قبل كل خطوة، ويُتابع نمط التبوّل والتبرّز ويُوثَّق أي تغيّر.

### 5. تغيير الوضعية والتوثيق وتدريب الأسرة

تُغيَّر وضعية المريض على فترات منتظمة بحسب الخطة، مع تفريغ الضغط عن الكعبين ووسائد بين البروزات العظمية وتجنّب سحب الجسم على الفراش. يُوثَّق كل ما يُقدَّم وحالة الجلد وكمّية الأكل والشرب، وتُدرَّب الأسرة على الجدول وعلى علامات الخطر التي تستدعي تواصلاً عاجلاً.

## Procedure steps

### 1. Assessing ability and risk

The nurse assesses what the patient can still do alone and where help is needed, and grades the risk of pressure injury and of falls alongside the state of the skin, mouth, continence and nutrition. This assessment produces a written plan setting the repositioning schedule, hygiene times and specific precautions, and it is reviewed as the patient's condition changes.

### 2. Bathing and skin care

The patient is washed in bed or in the bathroom according to ability, with warm water and mild soap, keeping unwashed areas covered for privacy and warmth. Skin is patted dry, particularly in skin folds, and dry areas are moisturised. Alcohol, powder and vigorous rubbing are avoided, and during washing the skin over the tailbone, heels and hips is inspected.

### 3. Mouth, hair and nail care

Teeth and tongue are cleaned at least twice daily, with extra mouth care for anyone fed by tube or breathing through the mouth, since a dry mouth becomes inflamed and can contribute to pneumonia. Hair is washed regularly, and nails are trimmed with great caution in people with diabetes or poor circulation, or left to a specialist.

### 4. Help with toileting and preserving dignity

The patient is helped to reach the toilet, use a commode or manage continence products as their condition requires, with prompt cleaning after every episode of wetness, since constant moisture weakens skin and prepares the ground for sores. Privacy is respected by closing the door, covering the patient and explaining each step, while bowel and bladder patterns are tracked and any change documented.

### 5. Repositioning, documentation and family training

The patient is repositioned at regular intervals according to the plan, with pressure taken off the heels, pillows placed between bony prominences, and dragging the body across the sheets avoided. Everything delivered is documented alongside skin condition and food and fluid intake, and the family is trained on the schedule and on the warning signs that call for urgent contact.

## التحضير قبل الإجراء

قبل بدء الخدمة، جهّز قائمة أدوية المريض بعبواتها الأصلية وتقاريره الطبية الأخيرة، وأخبر الممرض بأمراضه المزمنة وحساسيته الدوائية ومستوى وعيه وقدرته على البلع والحركة. رتّب البيت لتقليل الأذى: سرير يمكن الوصول إليه من جانبيه بارتفاع مناسب، إضاءة ليلية في الطريق إلى الحمّام، إزالة السجّاد المنزلق والأسلاك، ومقاعد ثابتة ومسند في الحمّام. جهّز مستلزمات النظافة والمناشف ووسائد إضافية لتفريغ الضغط، واسأل الطبيب عن سطح دعم مناسب للسرير إن كان المريض قليل الحركة. أخبر الممرض عن أي جرح أو احمرار أو قسطرة أو أنبوب موجود، وعن آخر مرّة تبرّز فيها المريض. \*\*ولا تتعاقد مع أحد قبل التحقّق من ترخيصه ومن ترخيص المركز أو الشركة\*\*، واطلب خطة رعاية مكتوبة وسجلاً يُدوَّن فيه كل ما يُقدَّم في كل زيارة.

## العناية بعد الإجراء

بين الزيارات، حافظ على جدول تغيير الوضعية كما كُتب، وافحص جلد المريض يومياً عند العجز والكعبين والوركين والكتفين والأذنين، وصوّر أي منطقة محمرّة لتقارنها لاحقاً. غيّر الحفاظات فور تبلّلها، وجفّف الجلد ولا تفركه، ولا تضع كحولاً ولا بودرة ولا تدلّك المنطقة المحمرّة فوق العظم. اِحرص على شرب سوائل كافية وتغذية غنيّة بالبروتين، ونظّف الفم مرّتين يومياً على الأقل. اجعل المريض يجلس ويتحرّك قدر ما يسمح الطبيب، فالحركة أفضل واقٍ من القرح. راجع فوراً عند: \*\*تغيّر مستوى الوعي أو خمول غير معتاد\*\*، \*\*صعوبة تنفس\*\*، \*\*حرارة عالية أو قشعريرة\*\*، \*\*قرحة فراش تتوسّع أو تُصرّف قيحاً أو تتحوّل حافتها إلى سوداء\*\*، \*\*نزيف من موضع قسطرة أو أنبوب\*\*، \*\*بول عكر ذو رائحة قوية أو انقطاع البول\*\*، \*\*قيء متكرّر أو رفض الأكل والشرب\*\*، \*\*سقوط أو ألم مفصل جديد\*\*. هذه كلها تعني \*\*تواصلاً عاجلاً أو طوارئ لا انتظاراً للزيارة القادمة\*\*.

## About this service

Personal care nursing supports people who can no longer manage their daily needs alone: bathing and hygiene, mouth, hair and nail care, dressing, help with toileting, and repositioning in bed. It is far from marginal work. Most of what harms a bedbound patient at home begins with neglect of exactly these details: a pressure sore, a urinary infection, an inflamed mouth, a fall, or a silent decline in nutrition.

When delivered at home the service runs \*\*on a physician's order with a written care plan\*\* that sets out the level of dependence, the repositioning schedule, what movement is permitted, and the precautions specific to that patient's condition. A licensed nurse assesses, delivers, documents and reports any change, but \*\*does not diagnose and does not alter a medication dose\*\*, and does not decide alone to add a cream, ointment or nutritional supplement.

In this service above all, \*\*prevention is the treatment\*\* where pressure sores are concerned: repositioning at regular intervals, inspecting the skin daily over bony prominences, keeping skin clean and dry and away from constant moisture, adequate protein and fluid intake, and a suitable support surface on the bed. Several common mistakes need correcting: a reddened area over a bone must not be massaged, because massage adds injury; alcohol and powder do not belong on at-risk skin; and ring or doughnut cushions should not be used, because they restrict blood flow. An established, deep sore is a condition that needs \*\*a team\*\*, including the physician, a wound specialist, nutrition and physiotherapy, not simply a dressing that gets changed.

Where a urinary catheter or feeding tube is present, \*\*hygiene and sterile technique are what prevent the most dangerous complication, bloodstream infection or sepsis\*\*. Personal care is also about dignity: privacy, keeping the patient covered, asking before touching, and involving them in decisions are all part of the quality of the service. ## Preparation

Before the service starts, gather the patient's medicines in their original packaging and their recent medical reports, and tell the nurse about chronic conditions, drug allergies, level of awareness and ability to swallow and move. Arrange the home to reduce harm: a bed reachable from both sides at a workable height, night lighting on the route to the bathroom, slippery rugs and trailing cables removed, and a stable seat and grab rail in the bathroom. Prepare hygiene supplies, towels and extra pillows for offloading pressure, and ask the physician about a suitable mattress or support surface if the patient moves very little. Tell the nurse about any wound, reddened area, catheter or tube already in place, and when the patient last opened their bowels. \*\*Do not contract with anyone before verifying their licence and that of the centre or company\*\*, and ask for a written care plan and a log recording everything delivered at each visit.

## Aftercare

Between visits, keep to the repositioning schedule as written, inspect the skin daily over the tailbone, heels, hips, shoulders and ears, and photograph any reddened area so you can compare it later. Change continence products as soon as they are wet, pat the skin dry rather than rubbing, and use no alcohol or powder and no massage over a reddened bony area. Ensure adequate fluids and protein-rich nutrition, and clean the mouth at least twice a day. Have the patient sit up and move as much as the physician allows, because movement is the best protection against sores. Seek care immediately for: \*\*a change in level of consciousness or unusual drowsiness\*\*, \*\*difficulty breathing\*\*, \*\*high fever or chills\*\*, \*\*a pressure sore that is widening, draining pus or turning black at its edge\*\*, \*\*bleeding from a catheter or tube site\*\*, \*\*cloudy strong-smelling urine or no urine at all\*\*, \*\*repeated vomiting or refusal of food and drink\*\*, \*\*a fall or new joint pain\*\*. All of these mean \*\*urgent contact or emergency care, not waiting for the next visit\*\*.
