## Procedure steps

### 1. Assessing the wound and its cause

The wound is described precisely: site, dimensions, depth, the colour of its base, the amount and odour of discharge, and the condition of surrounding skin, alongside questions about diabetes, arterial and venous disease, nutrition, smoking and medicines that slow healing. The wound is photographed for comparison and its cause established, because treatment starts from the cause and not from the dressing.

### 2. Cleaning with correct technique

Hands are cleaned and gloves worn, and the wound is irrigated with a solution suited to its nature to lift debris and dried exudate without damaging new tissue. Harsh agents are kept off the wound bed, no swab or solution is shared between patients, and the surrounding skin is cleaned and dried, since moisture there is a frequent cause of irritation and of the wound enlarging.

### 3. Removing dead tissue when indicated

Dead tissue blocks healing and feeds bacteria, so its removal is a clinical decision taken by someone with the competence and authorisation to do it, by a method suited to the wound and the patient. Tissue is not cut at home without a clear order, and dry eschar on a diabetic foot is not disturbed while the blood supply is unassessed, because doing so can open a far larger wound.

### 4. Choosing the dressing and change interval

The dressing is chosen according to the amount of exudate, the depth of the wound and whether it needs moisture or absorption, the aim being a moist wound bed with dry, intact edges. The change interval is set by exudate rather than by habit: unnecessarily frequent changes damage new tissue, while leaving a saturated dressing too long invites infection.

### 5. Follow-up, measuring progress and referral

The wound is measured and documented at every visit so its direction is known: shrinking or spreading. The family is taught pressure offloading, nutrition and warning signs. If progress stalls, or signs of deep infection or poor blood supply appear, the patient is referred to the physician, surgeon or vascular service without delay, because timely referral saves limbs.

## About this service

Wound care is a specialised nursing service covering assessment of the wound, cleaning, selection of an appropriate dressing, the interval between changes, and monitoring whether healing is progressing or has stalled. It spans acute wounds such as surgical incisions, superficial burns and traumatic injuries, and chronic wounds such as pressure sores, diabetic foot ulcers and venous leg ulcers, which are the hardest of all, because they do not heal until their cause is treated.

The fact that changes outcomes is this: \*\*a dressing does not treat the cause of a wound\*\*. A diabetic foot ulcer needs glucose control, assessment of the arterial blood supply, offloading of pressure from the ulcer site and testing of sensation. A venous leg ulcer needs properly prescribed compression once the arteries have been shown to be adequate. A pressure sore needs pressure relief and nutrition. Choosing the dressing is the last step, not the first, which is why the wound is measured and described precisely at each visit: its dimensions, depth, the colour of its base, the nature and amount of discharge, any odour, and the state of the surrounding skin.

With pressure sores in particular, \*\*prevention is the treatment\*\*: repositioning at regular intervals, daily skin inspection, moisture control, adequate protein and fluid intake, and a suitable support surface. An advanced, deep sore is a condition that needs \*\*a team\*\* — physician, surgeon, vascular assessment, nutrition and physiotherapy — not merely a dressing that gets changed. A wound showing no improvement after weeks of correct care is a signal to reassess, not to repeat the same dressing.

Some common household practices are dangerous and must stop: \*\*do not put coffee grounds, toothpaste, oils, herbs or ash on a wound\*\*. They introduce contamination, obscure assessment and can end in deep infection. Avoid pouring alcohol or harsh agents over newly growing tissue as well. \*\*Cleanliness and sterile technique are what prevent the most dangerous complication, bloodstream infection or sepsis.\*\* The service is delivered \*\*on a physician's order with a written care plan\*\*, and a licensed nurse \*\*neither diagnoses nor changes a dose\*\*. Because Jordan's home-care market includes unqualified providers, \*\*verify the licence of the nurse and of the centre or company\*\*.

## Preparation

Tell the nurse how the wound started, when it began and what has been applied to it so far, and bring your medical reports and recent laboratory results if available, along with your medicines in their original packaging. Mention diabetes, arterial, venous or kidney disease, and any use of steroids, blood thinners or chemotherapy, plus any allergy to medical adhesive or antiseptics. \*\*Do not apply household substances such as coffee grounds, toothpaste, oils or herbs to the wound\*\*, do not pull off crusts, and do not soak it in hot water. If the wound is on your foot, do not walk on it unprotected and do not cut your own nails or skin. Prepare a clean table, good lighting and a comfortable place at home where the wound can be exposed, and have the dressings your physician prescribed ready in their sealed packaging. \*\*Verify the licence of the nurse and of the centre or company before contracting\*\*, and ask for the wound to be measured, photographed and documented at every visit.

## Aftercare

Keep the dressing clean and dry and do not open it without reason, and bathe in the way the nurse showed you so the wound stays protected. Follow the ordered change interval and never leave a dressing saturated with exudate in place. Most important, treat the cause: control your glucose; wear the prescribed footwear and offload pressure from the site if the ulcer is on your foot; wear the compression your physician prescribed for a venous leg ulcer; and reposition yourself or the patient regularly for a pressure sore. Increase protein and fluids, and stop smoking, which reduces blood flow to the wound. Seek care immediately for: \*\*spreading redness, hot swelling or a red streak travelling upwards\*\*, \*\*purulent discharge or a new foul odour\*\*, \*\*high fever or chills\*\*, \*\*pain that increases sharply, or sensation that disappears entirely\*\*, \*\*blackening of the wound edge or surrounding tissue\*\*, \*\*a pressure sore that is widening or draining pus\*\*, \*\*bleeding from the wound or from a catheter or tube site\*\*, \*\*a change in level of consciousness or difficulty breathing\*\*. These mean \*\*urgent contact or emergency care, not waiting for the next visit\*\*.
