## Procedure steps

### 1. Risk assessment, sensation and pulses

The specialist examines both feet completely, including between the toes and under the heel, tests sensation with simple instruments that detect nerve damage before any complaint appears, and palpates pulses while assessing skin colour and temperature. The foot is assigned a risk grade that sets the follow-up interval, and findings are documented for comparison at the next visit.

### 2. Hands-on skin and nail care

Hardened skin is thinned and calluses and cracks are treated with sterile instruments, nails are trimmed straight across, and ingrown or fungal nails are managed. Any wound is cleaned and its depth assessed, and a swab or an X-ray may be requested if there is any suspicion that infection has reached the bone.

### 3. Footwear and pressure redistribution

Shoes are measured and checked from the inside, and pressure points are corrected with padding or medical insoles that shift load away from the ulcer site. The patient is shown how to choose closed shoes with a wide toe box, and told that new shoes should be worn for only a few hours on the first day, with the foot inspected immediately afterwards.

### 4. Wound and infection management

The wound is cleaned and dressed appropriately for its type, load is taken off the area, and treatment for infection is prescribed by the doctor when present. The wound is reassessed at short intervals, because a single day's delay can change the outcome, and the diabetes plan itself is reviewed, since wounds do not heal while blood sugar is high.

### 5. Education and onward referral

The patient and a family member are taught to inspect the feet daily with a mirror and to recognise the signs that demand urgent review. If pulses are weak, if walking causes pain, or if a wound is not improving, the patient is referred for vascular assessment, and a surgeon or infection specialist may join the same care team.

## About this service

Diabetic foot care is an organised medical programme that protects the feet of a person with diabetes from ulceration and infection. It combines periodic testing of sensation and circulation, treatment of any wound, crack or callus, correction of footwear and walking pattern, and teaching the patient to examine their own feet every single day. The programme exists because diabetes damages peripheral nerves and narrows small arteries, so the skin becomes both less able to feel and less able to heal at the same time.

The real danger here is not pain but its absence. We are used to pain being what drives us to the doctor, but in the diabetic foot the absence of pain is false reassurance. You may walk all day on a nail, a pebble or an open wound without feeling it, and a wound may burrow beneath a layer of hardened skin and reach bone before any symptom appears. Many ulcers begin with a tight new shoe, with hot water whose temperature was not felt, with incorrect nail cutting, or with walking barefoot indoors.

What foot care does is clear, and what it does not do must be equally clear. The programme prevents ulceration, speeds the healing of what can heal, and reduces the need for major intervention, but it does not restore sensation already lost in damaged nerves. It does not remove the need for glucose control, because a wound will not heal in a body with high blood sugar, and it does not fix arterial narrowing, which may need separate vascular assessment. Foot care is also not massage or pampering: it is examination and treatment with sterile instruments by a trained professional.

Several common household practices cause real harm: using a blade or scissors to remove hardened skin, buying caustic corn-removal ointments, applying hot compresses or hot water bottles for warmth, soaking the feet for long periods, and walking barefoot on hot sand, mosque floors or hot tiles. Each of these can create a painless wound that becomes a deep infection. Herbal and topical folk remedies do not replace medical treatment and may mask an infection until it has spread.

An annual foot examination by a professional is not optional, because loss of sensation and weak pulses both begin without any complaint and cannot be felt by the patient. Suitability for any treatment, and how often follow-up is needed, are determined by clinical examination of the foot, sensation and pulse testing, and the degree of glucose control, not by reading online.

## Preparation

Come to the appointment with clean, dry feet, and do not apply any cream, ointment or nail polish on the same day, because these hide skin detail and colour. Bring the shoes you actually wear every day rather than a new pair for the visit, along with your usual socks and any medical insoles you own. Tell the specialist when the first wound or crack appeared, whether you feel numbness, burning or night pain in the legs, and whether walking a certain distance brings on leg pain that eases with rest. Mention everything you have used on your feet yourself: a blade, scissors, a file, corn-removal ointment, herbal remedies, hot water or prolonged soaking. Bring your medication list, your glucose readings and your latest laboratory result, because glucose control is part of wound treatment and not a separate matter. Report smoking, since it narrows arteries and delays healing, and any heart or kidney disease. If you have an open wound, swelling, redness or fever, do not wait for a routine appointment: ask to be seen the same day.

## Aftercare

Inspect your feet every day at a fixed time, looking between the toes and under the sole with a mirror or asking someone to look for you, and search for redness, swelling, a wound, a dark patch or any colour change. Wash your feet in lukewarm water and dry them thoroughly, especially between the toes, then moisturise dry skin while avoiding the spaces between the toes. Never walk barefoot, even indoors, and check inside your shoes with your hand before putting them on. Test water temperature with your hand, your elbow or a thermometer, never with your foot's sensation. Do not use a blade, scissors or caustic corn ointment on calluses, do not apply a hot water bottle, and do not soak your feet for long periods. \*\*See a doctor the same day if you have: any wound, blister or crack even if small and painless; redness, swelling or local heat; discharge or bad odour; a blue or black colour change; a red streak spreading up the leg; fever, shivering, or an unexplained sudden rise in blood sugar.\*\* Also remember: \*\*severe low blood sugar is an emergency. Trembling, sweating, palpitations and confusion mean fast-acting sugar immediately; if consciousness is lost, call an ambulance and give nothing by mouth.\*\*
