Joint replacement rehabilitation

## Procedure steps

### 1. Post-operative assessment and first-phase plan

The therapist reviews the type of operation, the surgical approach and the surgeon's weight-bearing instructions, then examines the wound, swelling, range of motion, thigh and gluteal strength, and your ability to transfer and walk. What is allowed and what is restricted in the first weeks is explained, and the right walking aid is selected: a frame or two crutches.

### 2. Complication prevention and early mobility

Ankle pumping and deep breathing reduce the risk of clots and chest problems, while frequent short walks from the first day, as the condition allows, restore mobility. You are trained to sit and rise from a chair, toilet and bed in a way that protects the joint. Cooling and elevation control the swelling that otherwise blocks movement.

### 3. Restoring full range of motion

In the knee, full extension alongside progressive flexion is the absolute priority; in the hip, range is regained within the prescribed precautions. Active, active-assisted and gentle manual techniques are used, and angles are tracked by measurement rather than impression. Range lost in the first weeks is much harder to recover later, so daily repetition is non-negotiable.

### 4. Strength, balance and normal gait

Graded strengthening of the quadriceps, hamstrings, hip and gluteal muscles, plus balance and weight-shift work, gait retraining to eliminate a limp, and stair training up and down. The walking aid is weaned by the therapist's decision rather than by enthusiasm, because dropping a crutch too early locks in a limp that is hard to unlearn.

### 5. Return to daily life and reassessment

Endurance is built with walking and a stationary bike, work and household tasks are rehearsed, and a long-term activity plan protects the joint: low-impact activity, weight control, and continued weekly strength work after discharge. No session count and no specific outcome are promised; progress is measured and reassessed at each review.

## About this service

Rehabilitation after joint replacement is a graded physiotherapy programme that usually begins on the day of surgery or the day after and continues for weeks after discharge, covering total or partial knee and hip replacement. Surgery replaces the worn surface and removes the mechanical source of pain, but it does not restore strength, range of motion or balance. Those are regained through exercise alone, which is why rehabilitation is half the result of the operation rather than an optional extra afterwards.

The programme moves through phases. The early phase aims at walking with an aid, preventing clots and chest complications, and recovering full bending and especially full straightening of the joint. The middle phase strengthens the thigh and gluteal muscles and works on balance, stairs and walking without a limp. The later phase builds endurance and returns you to work, household activity and longer walks. After hip replacement, positional precautions are followed for a period defined by the surgeon according to the surgical approach.

The biggest factor in the outcome is not the number of clinic sessions but your adherence to the home programme several times a day. Physiotherapy works cumulatively, and a week of neglected exercise costs range that is hard to win back, particularly full knee extension. This is not massage and it is not a machine: electrical stimulation, therapeutic ultrasound and cooling may help settle a given phase, but they do not build muscle and they do not open a stiff joint. The idea that exercise only works if it hurts is also wrong here; what is needed is a measured load that advances step by step, not training into severe pain.

The limits should be stated plainly. A replaced joint does not become a natural joint again, numbness around the scar or occasional clicking may persist, and contact sport, long-distance running and deep squatting are generally not advised. Rehabilitation cannot correct a fixation problem, an infection or a leg-length difference; those are referred back to the surgeon. Programme length is estimated initially in weeks to months and reassessed at every review, and suitability for each phase is decided by clinical examination and the surgeon's report rather than by reading.

## Preparation

Bring your operation or discharge report and your imaging, and tell the therapist which joint was replaced, the surgical approach and the surgeon's instructions on weight-bearing and permitted range, because these define your plan. Declare your medications, especially blood thinners, painkillers and diabetes or blood-pressure drugs, and any dressing allergy or wound problem. Wear loose clothing that allows the joint to be exposed and firm closed shoes with a low heel rather than slippers, and bring the frame or crutches you are using. Prepare your home before you return: remove loose rugs and cables, and arrange a raised toilet seat and a shower chair if your therapist recommends them. Take strong painkillers before a session only as your doctor prescribed.

## Aftercare

Do your home programme several times a day in short doses, because it is what really decides the outcome, and work on full knee extension every single day. Cool the joint and elevate the limb after exercise if it swells, take frequent short walks rather than one long one, and avoid sitting for more than an hour at a stretch in the early weeks. Follow your hip precautions if they were given, and do not abandon your crutch before you are cleared. Expect soreness that settles within a day; pain that increases day after day should be reported. Seek medical care immediately if you have: \*\*fever with redness, swelling or discharge from the wound\*\*, \*\*persistent, worsening night pain after an initial improvement\*\*, \*\*swelling and pain in one calf or sudden breathlessness — an emergency, as this may be a clot\*\*, \*\*an audible pop with sudden inability to bear weight or a deformed limb position\*\*, or increasing numbness and weakness below the joint.
