## Procedure steps

### 1. Assessment and defining the dysfunction

It is determined whether the muscles are weak or overactive, and the ability to contract and relax is examined — the wrong exercise for the wrong dysfunction worsens symptoms.

### 2. Screening associated causes

Constipation, chronic cough, weight, activity type, births and pelvic surgery are asked about, with referral for urological or gynaecological assessment where indicated.

### 3. Teaching the correct contraction

The patient is trained to engage the pelvic floor without squeezing the abdomen or buttocks and without holding the breath, with biofeedback used to confirm correct performance.

### 4. A graded exercise programme

Short and long contractions in graded daily doses, integrated with breathing and trunk work, plus training in protective bracing before coughing or lifting.

### 5. Bladder training and habit correction

Timed voiding with gradual urge deferral, fluid and caffeine adjustment, treating constipation, correcting toileting posture and avoiding straining.

## About this service

The pelvic floor is a hammock-like muscle layer across the base of the pelvis that supports the bladder, uterus and rectum and contributes to control of urine, gas and stool and to sexual function. It weakens or becomes dysfunctional after pregnancy and childbirth, with ageing and menopause, with chronic constipation, persistent cough, excess weight and incorrect lifting, and after pelvic or prostate surgery.

Symptoms that often go unspoken: \*\*urine leaking with coughing, laughing, sneezing or exercise\*\*; urgency that cannot be deferred; a sensation of heaviness or descent in the vagina; difficulty emptying; or pelvic pain or pain during intercourse. Many women regard urine leakage after childbirth as “normal and to be accepted” — it is not, and it is among the conditions most responsive to conservative treatment.

But here is a fundamental, neglected point: \*\*not every case is weakness, and not every case needs strengthening.\*\* Some problems arise from \*\*overactive, tight\*\* muscles rather than weak ones (presenting as pelvic pain, pain with intercourse or difficulty emptying), and random strengthening exercises in that situation \*\*make symptoms worse\*\*. Assessment before exercise is therefore not a luxury.

Second point: \*\*most people who say they do Kegel exercises perform them incorrectly\*\* — squeezing the abdomen or buttocks, holding the breath, or trying to stop the urine stream mid-flow (which is harmful). Assessment with a qualified physiotherapist teaches the correct contraction, and biofeedback or electrical stimulation may be used.

The programme also includes bladder training, correcting constipation and toileting posture, teaching pelvic-floor bracing before coughing and lifting, and breathing and trunk exercises — and results require commitment over weeks to months, not days.

## Preparation

Before the visit keep a \*\*three-day bladder diary\*\*: voiding times and estimated volumes, leakage episodes and what was happening then (cough? exercise? urgency?), and fluids and caffeine. This markedly improves the plan’s precision. Report the number and type of births and any tear or difficult delivery, any pelvic or prostate surgery, and menopausal status. Tell the therapist about any \*\*pain\*\* in the pelvis or during intercourse — this directs treatment toward relaxation rather than strengthening. Mention constipation, chronic cough and your medications (some blood-pressure drugs and sedatives affect the bladder). And do not expect results in a week; prepare for a programme of weeks.

## Aftercare

Perform the exercises \*\*at the dose and with the technique prescribed for you\*\* — no more and no less; random increases do not speed results and may fatigue the muscle. Do not stop the urine stream mid-flow as an exercise, and do not squeeze the abdomen or buttocks or hold your breath while contracting. Brace the pelvic floor \*\*before\*\* coughing, sneezing and lifting — this habit alone greatly reduces leakage. Treat constipation with fibre and fluids and do not strain, and sit correctly on the toilet. Avoid incorrect lifting and high-impact exercise until your therapist permits. Maintain a healthy weight and avoid smoking (chronic cough worsens matters). \*\*Expect gradual improvement over six to twelve weeks of daily adherence; stopping is the commonest reason for no result.\*\* Contact your clinician for increasing pain, blood in the urine, burning with fever (urinary infection), a sensation of a descending mass, or sudden new leakage.
