## Procedure steps

### 1. Clinical assessment and screening

The therapist examines range of motion, swelling, ligament stability and the strength of the thigh and hip muscles, watches your walking and stair descent, and performs meniscal and patellar tests. The most important part of this step is screening out what exercise cannot treat: a locked joint, infection, a fracture after trauma, or red flags that require a doctor and imaging first.

### 2. Symptom relief and load modification

Activities that flare the pain are eased temporarily without complete rest, since prolonged rest weakens the muscle and worsens pain later. Cooling, heat, taping or manual therapy are used according to the case, and lower-load techniques for stairs, standing up and squatting are taught. The purpose of this phase is to make exercise possible, not to settle there.

### 3. Restoring range and flexibility

Exercises restore full flexion and extension, with stretching for the hamstrings, hip flexors and calf, and patellar mobilisation where there is stiffness. Full extension is a priority because losing it alters gait and increases load on the quadriceps. Progress is measured in degrees at each review rather than judged by feel alone.

### 4. Progressive strengthening and neuromuscular control

Strengthening begins without load and progresses to resistance that is increased weekly: quadriceps, hamstrings, glutes and hip abductors, then functional work such as controlled sit-to-stand and step up and down. Balance and position-sense training is added to keep the knee tracking correctly, and this is what protects against recurrence.

### 5. Endurance, return to activity and long-term plan

Endurance is built with walking, a stationary bike or the pool, and return to work or sport is graded against strength and balance criteria compared with the sound leg. You leave with a weekly maintenance plan, because the gains fade if exercise stops. Duration and intensity are estimated initially and reassessed, with no promise of a session count or a guaranteed result.

## About this service

Knee rehabilitation is a physiotherapy programme built on clinical assessment to address knee pain, weakness and restricted movement, whether in osteoarthritis, patellar pain, after a sprain, ligament or meniscal injury, or following arthroscopic surgery. Its core is not temporary pain relief but rebuilding capacity: strength in the thigh and hip muscles, neuromuscular control that keeps the knee tracking correctly, and enough endurance to walk, climb stairs and stand without flaring up.

The programme includes an assessment that screens out what exercise cannot treat, then exercise graded by resistance and repetition, range and flexibility work, and load management: temporary activity modification rather than complete rest, along with advice on body weight and daily movement patterns such as descending stairs and squatting. Manual therapy, taping or a brace may be added to ease symptoms in one phase; these are temporary aids that make exercise possible and do not take its place.

Results accumulate, and the biggest factor is your adherence to home exercise: two weeks of consistency shows more than ten scattered clinic visits, and a break sets you back. Two myths hold many patients up. The first is that physiotherapy is just massage, when it is in fact prescribed therapeutic exercise with a dose, a resistance and a planned progression. The second is that pain proves benefit; in reality mild soreness that settles by the next day is acceptable, while sharp pain or renewed swelling means the dose was too high and needs adjusting rather than enduring. Devices such as electrical stimulation, ultrasound, laser and shockwave help settle symptoms in a supporting role and do not replace exercise.

The limits are explicit. Exercise does not grow cartilage and does not reverse the wear visible on a scan, it does not knit a completely torn ligament, and it cannot fix a locked knee caused by a displaced meniscal tear. A brace does not strengthen a muscle. What exercise genuinely does is reduce pain, improve function, and in many cases delay or remove the need for intervention. Duration is estimated initially in weeks and reassessed periodically, and suitability for each exercise is decided by clinical examination rather than by reading.

## Preparation

Tell the therapist when the pain began, what aggravates it and what relieves it, and about any previous knee injury, trauma or surgery. Bring X-rays or MRI scans and any doctor's reports, along with a list of your medications including any joint injections and when they were given. Mention diabetes, heart or blood-pressure disease, rheumatoid arthritis or osteoporosis, as these shape the type and intensity of exercise. Wear shorts or trousers that roll above the knee and closed sports shoes, and bring your brace or walking stick if you use one. Avoid strong painkillers before the session unless needed, because blocking pain completely hides the safe limits of loading. If your knee is locked and will not straighten, or has suddenly swollen and feels hot, see a doctor before your physiotherapy session.

## Aftercare

Do the home exercises at the prescribed dose daily or as scheduled, because progress comes from repetition rather than from the session alone, and do not stop when the pain disappears, since the muscle needs weeks beyond that point to reach protective strength. Expect mild soreness that settles within a day; if the knee swells or the pain rises on two consecutive days, step back on the resistance and tell your therapist so the plan can be adjusted. Cool the joint after exercise if it swells, limit stair descent and deep squatting in the early phase, and manage your weight, since every kilogram is felt at the knee. Seek medical care immediately if you have: \*\*fever with a swollen, red, hot knee — possible joint infection, which is an emergency\*\*, \*\*persistent night pain that does not settle with rest, or unexplained swelling with weight loss\*\*, \*\*a locked joint or sudden inability to bear weight\*\*, \*\*pain after significant trauma before imaging has been done\*\*, or increasing numbness and weakness in the leg.
