عند تحريك السنّ يتغيّر موضعه داخل العظم، وتحتاج الأربطة المحيطة بجذره والعظم الحاضن له إلى فترة طويلة حتى تعيد ترتيب نفسها حول الوضع الجديد. في هذه الفترة تكون الأسنان مهيّأة للرجوع إلى مكانها الأصلي، ويستمر الفم بالتغيّر مع تقدّم العمر حتى عند من لم يخضع لتقويم أصلاً. \*\*المثبِّت بعد انتهاء التقويم ليس خياراً اختيارياً؛ إهماله يعيد الأسنان إلى مكانها تدريجياً، وهذا أشيع سبب لفشل نتيجة التقويم بعد سنوات من العلاج.\*\*

تنقسم المثبِّتات إلى نوعين. الثابت سلك رقيق يُلصق على السطح الداخلي للأسنان الأمامية ولا يستطيع المريض إزالته، ويناسب من يُخشى عليه من الارتداد أو من لا يُعتمد على التزامه. والمتحرك جهاز شفّاف يغطّي الأسنان أو صفيحة بسلك أمامي، يخلعه المريض عند الأكل وتنظيف الأسنان ويلبسه وفق جدول يحدّده الطبيب. الاختيار بين النوعين أو الجمع بينهما يعتمد على نوع الإزاحة التي عُولجت ودرجة خطر الارتداد وقدرة المريض على الالتزام، ويُحدَّد بالفحص السريري لا بالقراءة.

حدود هذه المرحلة يجب أن تكون واضحة: المثبِّت يحافظ على نتيجة قائمة ولا يُصحّح إزاحة جديدة، ولا يُعيد أسناناً ارتدّت فعلاً إلى مكانها. وإن حدث ارتداد ملحوظ فالحل عادة إعادة تقويم قصيرة وليس مثبِّتاً أقوى أو أطول لبساً. كما أن المثبِّت لا يمنع التسوّس ولا التهاب اللثة؛ بل يزيد الحاجة إلى تنظيف يومي دقيق، خصوصاً حول السلك الملصق حيث يتراكم الجير بسهولة.

## خطوات الإجراء

### 1. تقييم نهاية التقويم

يفحص الطبيب إطباق الأسنان وثباتها ويصوّر الفم شعاعياً عند الحاجة، ليحدّد نوع المثبِّت المناسب وخطر الارتداد قبل إزالة الأجهزة.

### 2. إزالة أجهزة التقويم وتنظيف الأسنان

تُفكّ الحلقات المعدنية أو الخزفية وتُزال بقايا اللاصق عن سطح السنّ، ثم تُصقل الأسنان لتصبح جاهزة لأخذ مقاسات المثبِّت.

### 3. أخذ الطبعة أو المسح الرقمي

تُؤخذ طبعة تقليدية أو مسح ضوئي ثلاثي الأبعاد للفكين في اليوم نفسه، لأن أي تأخير يسمح بحركة طفيفة تجعل المثبِّت غير مطابق.

### 4. تركيب المثبِّت وتسليمه

يُلصق السلك الثابت على السطح الداخلي للأسنان الأمامية إن كان مقرّراً، ويُسلَّم المثبِّت المتحرك بعد ضبطه حتى يستقرّ بلا ضغط مؤلم.

### 5. جدول اللبس والمتابعة طويلة الأمد

يشرح الطبيب عدد ساعات اللبس اليومية وكيف تتقلّص مع الوقت، ويحدّد مواعيد متابعة تمتد سنوات لفحص السلك والمثبِّت وسلامة الإطباق.

## Procedure steps

### 1. End-of-treatment assessment

The orthodontist checks the bite and the stability of the corrected teeth, with radiographs when needed, to decide the retainer type and the relapse risk before appliances are removed.

### 2. Removing braces and polishing

Brackets are debonded and residual adhesive is cleared from the enamel, then the teeth are polished so accurate retainer records can be taken.

### 3. Impression or digital scan

A conventional impression or a three-dimensional scan of both arches is taken the same day, since any delay allows slight movement that would make the retainer fit poorly.

### 4. Fitting and delivery

A fixed wire is bonded behind the front teeth if planned, and the removable retainer is adjusted until it seats fully without painful pressure before it is handed over.

### 5. Wear schedule and long-term follow-up

The clinician explains the daily wear hours and how they taper over time, and sets follow-up visits spanning years to check the wire, the retainer and the stability of the bite.

## التحضير قبل الإجراء

أخبر الطبيب إن كنت تشعر بحركة في أي سنّ أو بتغيّر في إطباقك قبل موعد إزالة التقويم، فذلك يغيّر نوع المثبِّت المطلوب. نظّف أسنانك جيداً قبل الجلسة، والتزم بموعد التنظيف عند اللثة إن طُلب منك، لأن التهاب اللثة يجعل لصق السلك أصعب وأقلّ ثباتاً. أخبر الطبيب عن أي حساسية لمواد اللصق أو المعادن، وعن أي جهاز واقٍ ليلي تستخدمه لصرير الأسنان حتى يُنسَّق تصميم المثبِّت معه. رتّب وقتك بحيث تُؤخذ الطبعة في اليوم نفسه لإزالة الأجهزة لا بعده، وتجنّب تناول أطعمة قاسية قبل الجلسة مباشرة إن كانت أسنانك مؤلمة. أخيراً، احضر ومعك تصوّر واقعي: هذه بداية مرحلة جديدة من الالتزام، لا نهاية العلاج.

## العناية بعد الإجراء

التزم بعدد ساعات اللبس التي حدّدها الطبيب حرفياً، ولا تقرّر بنفسك تقليلها لأن الأسنان تبدو مستقيمة. اخلع المثبِّت المتحرك عند الأكل والشرب الساخن، ونظّفه بفرشاة ناعمة وماء فاتر لا مغلي، واحفظه في علبته دائماً؛ فقدان المثبِّت في منديل ورقي أشيع من كسره. نظّف حول السلك الثابت يومياً بخيط أو فرشاة بين الأسنان لأن الجير يتراكم عليه بسرعة. راجع طبيبك في أقرب وقت إن انفكّ السلك الثابت أو شعرت بطرف حادّ فيه، أو إن أصبح المثبِّت المتحرك لا يستقرّ في مكانه أو صار لبسه يحتاج قوة. \*\*راجع فوراً عند تورّم اللثة أو صديد أو ألم نابض حول سنّ يحمل السلك، أو عند بلع قطعة من المثبِّت، أو ظهور حركة واضحة في سنّ أمامي.\*\* ولا تعتمد على مثبِّت قديم لم تلبسه شهوراً؛ أعِد فحصه قبل استخدامه.

## About this service

Retention is the phase of orthodontic care in which appliances called retainers are used to hold the teeth in their corrected positions after fixed braces are removed or clear aligner wear is completed. It belongs to the treatment plan from day one and is not a separate service added after the braces come off.

When a tooth is moved, its position inside the bone changes, and the ligaments around its root together with the surrounding bone need a long period to reorganise around that new position. During this time the teeth are primed to drift back, and the mouth keeps changing with age even in people who never had orthodontic treatment. \*\*Wearing a retainer after braces is not optional. Skipping it lets the teeth relapse gradually, and this is the most common reason a successful orthodontic result is lost years after treatment ended.\*\*

Retainers fall into two families. A fixed retainer is a thin wire bonded to the inner surface of the front teeth; the patient cannot remove it, which suits cases at higher risk of relapse or patients whose compliance cannot be relied upon. A removable retainer is either a clear tray covering the teeth or a plate with a front wire, taken out for eating and cleaning and worn on a schedule set by the orthodontist. Choosing between them, or using both together, depends on the type of movement that was corrected, the estimated risk of relapse and how reliably the patient can follow instructions. Suitability is decided by clinical examination, not by reading about it.

The limits of retention should be stated plainly. A retainer preserves an existing result; it does not correct a new problem and it cannot pull back teeth that have already relapsed. If noticeable relapse has happened, the usual answer is a short course of re-treatment rather than a stronger retainer or longer wear. A retainer also gives no protection against decay or gum disease; it actually raises the need for careful daily cleaning, especially around a bonded wire where calculus builds up easily.

## Preparation

Tell your orthodontist if you feel any tooth moving or notice a change in your bite before the debonding appointment, because it may change the retainer that is required. Clean your teeth thoroughly before the visit and attend any gum-cleaning session you were asked to have, since inflamed gums make bonding a wire harder and less durable. Mention any known sensitivity to bonding materials or metals, and tell the clinician about any night guard you wear for teeth grinding so the retainer design can be coordinated with it. Arrange your schedule so the impression or scan is taken on the same day the appliances come off rather than later, and avoid hard foods immediately before the visit if your teeth feel tender. Finally, arrive with a realistic expectation: this is the start of a new phase of commitment, not the end of treatment.

## Aftercare

Follow the prescribed wear hours exactly and do not decide on your own to cut them back because the teeth look straight. Remove a removable retainer for eating and for hot drinks, clean it with a soft brush and lukewarm rather than boiling water, and always keep it in its case; retainers are lost in paper napkins more often than they are broken. Clean around a bonded wire every day with floss or an interdental brush, because calculus collects on it quickly. Contact your orthodontist promptly if the fixed wire comes loose or you feel a sharp edge, or if a removable retainer no longer seats properly and needs force to insert. \*\*Seek care immediately for swollen gums, pus or throbbing pain around a tooth carrying the wire, if you swallow a piece of the retainer, or if a front tooth becomes visibly mobile.\*\* Never rely on an old retainer you have not worn for months; have it checked before using it again.
