Ankle fusion (Ankle arthrodesis)
- **Procedure duration:** Surgery usually takes one and a half to three hours depending on deformity and fixation type, with an overnight stay or longer. Non-weight-bearing lasts for weeks set by your surgeon, followed by staged loading, while bony union needs three to six months and longer in smokers and people with diabetes. Full return to activity can extend towards a year.

## Procedure steps

### 1. Assessment and localising the problem

The surgeon checks ankle range of motion, alignment and ligament stability, and orders weight-bearing X-rays, since standing films show wear and deformity differently from films taken lying down. A CT or MRI may be added to assess bone quality and the neighbouring joints. Pulses, skin condition and foot perfusion are evaluated, and you are asked about smoking and diabetic control because both determine the chance of union.

### 2. Preparation and anaesthesia

Your medicines are reviewed and blood thinners are stopped only on written instruction from your doctor, never on your own. Blood tests and, where needed, cardiac assessment are arranged. Surgery is performed under general or regional anaesthesia, usually with a nerve block around the leg to reduce pain in the first days, and a prophylactic antibiotic is given before the incision.

### 3. Joint preparation and alignment

The joint is exposed from the front or the side, or addressed arthroscopically in selected cases, then the remaining damaged cartilage is removed and the bone surfaces are prepared so that they can knit together. The critical step is positioning the foot in correct alignment: an error of a few degrees changes gait and overloads the neighbouring joints, so alignment is verified with imaging during the operation.

### 4. Fixation and bone grafting

The joint is held in position with screws, a plate or an intramedullary nail, and in certain situations with an external fixator. Bone graft taken from the patient or a bone substitute may be added to fill gaps and improve the chance of union. The aim is rigid stability that prevents any micromotion between the prepared surfaces, since repeated micromotion is what causes union to fail.

### 5. Casting, staged weight-bearing and follow-up

The foot is placed in a cast or boot and kept non-weight-bearing for a period set by the surgeon, after which increasing partial weight-bearing is allowed according to follow-up imaging. X-rays are repeated at intervals to confirm that union is progressing, and physiotherapy begins to strengthen the leg and retrain balance and gait. Your compliance with the non-weight-bearing phase is the factor in the outcome that is genuinely under your control.

## About this service

Ankle fusion, or ankle arthrodesis, is an operation in which the shin bone and the bone beneath it are joined into a single block of bone, so that the joint disappears and with it the pain caused by its worn-out cartilage. The logic is direct: in an arthritic ankle, bone rubs on bone with every step, the cartilage cannot be regrown, and abolishing movement at that joint abolishes the mechanical pain. It is normally considered only after conservative measures have failed, including pain control, physiotherapy, stiff-soled footwear, bracing and selected injections.

The commonest cause in Jordan is post-traumatic ankle arthritis. The ankle carries very high loads over a small surface area, so any fracture that alters the shape of its joint surface predisposes it to early wear, often years later. Other causes include rheumatoid and other inflammatory joint diseases, severe deformity of the ankle axis, death of part of the bone, and failure of a previous ankle replacement.

The limits of this operation must be understood before consenting. Fusion permanently abolishes movement at the ankle itself, so up-and-down bending no longer occurs at that joint. Walking on level ground remains possible and is usually easier once the pain is gone, because the neighbouring joints of the foot take over part of the motion, but going down stairs and slopes, running and wearing heels change permanently. Fusion also does not treat arthritis in the neighbouring joints, and load on them may increase over the years so that they can become arthritic later.

The main alternative is ankle replacement, which preserves some movement. The choice between the two is decided by clinical examination and imaging together with your age, activity level, weight, bone quality and the state of the surrounding soft tissue, not by preference alone. The most important condition for successful union is strict compliance with the period of non-weight-bearing, along with stopping smoking, because smoking and poorly controlled diabetes both raise the risk of the bone failing to unite, which is the most significant complication of this surgery.

## Preparation

Tell your surgeon about every medicine you take, particularly blood thinners, aspirin, steroids, biologic drugs for rheumatoid disease and diabetes medications, and never stop any drug on your own initiative. Follow the fasting instructions exactly, and tell the anaesthetic team about any allergy or previous problem with anaesthesia. If you smoke, stopping before surgery is not general advice but a condition that improves the odds of the bone uniting, and if you have diabetes work on tightening your glucose control. Prepare your home in advance: a sleeping place on the ground floor if possible, a shower chair, loose rugs removed, and practise with crutches or a walker before the operation, because learning to use them afterwards is much harder. Arrange transport and help for the first days, and specifically ask your surgeon how long you must avoid weight-bearing and how long you will be off work before you fix the date.

## Aftercare

Keep the foot elevated above heart level for most of the first days, as this is the single most effective measure against swelling and pain. Move your toes, knee and hip as permitted, drink fluids and move in bed to reduce the risk of clots. Do not put weight on the foot before your surgeon explicitly allows it, even if the pain has gone, and keep the cast or boot dry, never pushing anything inside it to relieve itching. \*\*Seek help immediately for severe pain not relieved by your prescribed painkillers, new numbness or weakness in the foot, pale or bluish toes, or a painful warm swollen calf, and treat it as an emergency if it comes with shortness of breath or chest pain.\*\* \*\*Also seek review for fever or shivering, foul-smelling discharge, spreading redness around the wound, or if you hear a sound or feel sudden movement at the fusion site.\*\* Do not return to smoking during the healing period.
