## Procedure steps

### 1. Initial interview and setting priorities

The therapist gathers developmental history, medical and diagnostic reports, asks about current communication, routines, sleep and eating, and listens to the family priorities. Safety and functional communication goals come before others, and medical causes of behaviour such as pain, a hearing problem or seizures are excluded before the behaviour is attributed to learning alone.

### 2. Functional behaviour assessment and baseline

Behavioural incidents are recorded with what preceded and followed them, their duration and frequency, using direct observation, interviews and rating scales, and controlled situations may be arranged to test the function of the behaviour. Current language, play and independence skills are also measured, and baseline figures are documented so that progress can later be measured against them.

### 3. Writing the plan and its measurable goals

A positive behaviour support plan is written: modifying what precedes the behaviour, teaching a communication alternative that serves the same function, building replacement skills such as waiting, choosing and tolerating frustration, and agreeing one consistent adult response. Goals are written in measurable, time-bound terms, explained to the family, and consented to before implementation.

### 4. Implementation and family training

Sessions combine structured teaching with instruction embedded in play and routines, using immediate reinforcement, graded steps and systematic fading of prompts. Parents and caregivers are trained to apply the same methods at home, because consistency between home and session is the strongest driver of behavioural improvement and the fastest route to generalisation.

### 5. Data review, plan revision and transition

Data are recorded at every session and reviewed regularly: goals that are progressing are generalised to home, school and other people, while goals that are not progressing have their method changed rather than repeated unchanged. Targets are usually updated every 3 to 6 months, with a plan for school inclusion and a gradual reduction of hours as goals are achieved.

## About this service

Applied behaviour analysis (ABA) is a therapeutic approach based on the science of behaviour. It examines the relationship between a behaviour, what happens in the environment before it, and the consequences that follow, and then uses that relationship to teach new skills and reduce behaviours that interfere with learning or safety. It is used with children on the autism spectrum and with developmental delay, learning difficulties and attention and behaviour disorders; it is not limited to a single diagnosis, and it is also applied to independence skills in adolescents and adults.

The cornerstone of a programme is functional behaviour assessment: the question is not only what the behaviour is, but why it happens. The same behaviour, such as screaming, hitting or escaping from a request, may function to obtain something, to escape a difficult task, to gain attention, or to meet a sensory need. Identifying the function drives the plan, because a behaviour maintained by escape is handled quite differently from one maintained by requesting. For that reason a sound plan begins by teaching a communication alternative that achieves the same function in an acceptable way, rather than merely suppressing the behaviour.

A programme consists of written measurable goals, structured sessions alongside teaching embedded in play and daily routines, planned reinforcement, training for the family and caregivers, and data collected at every session and compared with baseline so the plan can be adjusted. Attention to generalisation is essential: a skill that appears only in the therapy room and not at home or school is treated as an unfinished goal.

The limits of the approach should be clear before starting. Applied behaviour analysis is not a medicine and does not cure autism; it does not treat co-occurring medical problems such as epilepsy, sleep disorders, reflux or dental pain; and it does not replace speech therapy, occupational therapy or hearing assessment when those are needed. Outcomes vary between children according to age, goals, consistency and family involvement, and nobody should promise you a specific result or a fixed timetable for progress.

One widely held misconception deserves correcting: difficult behaviour is not stubbornness or poor parenting but a message with a function, and suppressing it without teaching a communication alternative usually makes it worse. A professional programme is also not built on punishment, pain or withholding a child basic needs; always ask for a written plan with understandable goals, intervention methods you consent to, and a clear measure of progress.

## Preparation

Bring diagnostic, psychological and educational assessment reports, hearing and vision test results, speech and occupational therapy reports if available, and a list of current medicines. If you can, record a short video of the behaviour that worries you in its natural setting, and keep a one-week log of when the behaviour happens, how long it lasts, what was going on beforehand and how adults responded afterwards. Rule out medical causes first: dental, ear or stomach pain, itching, constipation and poor sleep, because in a child with little speech pain shows up as behaviour rather than as a complaint. Tell the therapist about sleep problems, food allergies and any previous seizures. Before the session, write down the three goals that matter most in your child daily life, because those are the ones that will be targeted first.

## Aftercare

Apply the same plan at home consistently and by everyone who deals with the child, because inconsistency between adults lengthens treatment. Record the simple data you are asked for honestly, including on bad days, since it is the basis for revising the plan. Do not introduce physical punishment or withhold food or sleep as a behavioural method, do not stop medicines prescribed by the paediatrician or neurologist on your own, and do not let a skill stay tied to one person or one room. Contact the team before the routine appointment if you notice: loss of skills the child had mastered, \*\*self-injury or aggression that threatens the child or others\*\*, or \*\*refusal of food or fluids or a sudden marked change in behaviour\*\*, and think of pain or illness first. If a \*\*seizure, loss of consciousness, or sudden weakness of movement or speech occurs, call emergency services on 911 at once\*\*.
