## Procedure steps

### 1. Assessment and heart team decision

Angiography, echocardiography, left ventricular function and kidney and lung status are reviewed, and surgery is compared with stenting and medical therapy at a joint team meeting. The options and their individual risks are then explained to you before consent.

### 2. Pre-operative preparation

Blood tests, a chest radiograph and, where indicated, carotid and dental assessment are completed, cardiac and diabetes medication is optimised, the timing of antiplatelet and anticoagulant drugs is set by the treating doctors, and breathing exercises and smoking cessation begin.

### 3. Anaesthesia and conduit harvesting

After full general anaesthesia and close monitoring, the internal mammary artery is harvested from inside the chest wall, and a leg vein or forearm artery is taken where needed through incisions or endoscopically. Conduit choice depends on the target vessel and the state of the patient's vessels.

### 4. Constructing the bypass grafts

The breastbone is opened and the patient is usually placed on cardiopulmonary bypass. Each graft is then sewn to the coronary artery beyond the narrowing and to its blood source. Flow through every graft is verified before the heart resumes work and the sternum is closed with wires.

### 5. Intensive care and cardiac rehabilitation

A day or two in intensive care with chest drains and monitoring of rhythm and fluid balance, then several days on the ward with graded walking and breathing exercises. After discharge a supervised rehabilitation programme begins, alongside control of blood pressure, sugar and lipids, and wound review.

## About this service

Coronary artery bypass surgery creates an alternative route for blood to reach the heart muscle beyond a narrowing in a coronary artery, using a conduit taken from the patient: the internal mammary artery from inside the chest, a saphenous vein from the leg, or the radial artery from the forearm. The conduit is sewn above and below the blockage so the muscle is perfused again. The central idea matters: the operation does not remove or dilate the blockage, it bypasses it.

It is considered when angiography has shown disease spread across more than one artery, a significant narrowing of the left main segment, anatomy too complex for catheter treatment, or coronary disease combined with a weakened heart muscle or with diabetes. The decision is usually taken by a joint heart team of cardiologist and cardiac surgeon together with the patient, after weighing it against stenting and intensive medical therapy. Suitability is established by examination, echocardiography and angiography, not by reading about it or comparing with other people's cases.

The operation is most often performed through an incision in the breastbone with cardiopulmonary bypass, a machine that temporarily takes over the circulation so the surgeon can work on a still heart. In selected patients it can be done with the heart beating or through smaller incisions. The number of grafts is decided by what is seen and measured during the operation itself, and valves are addressed at the same time if they are diseased.

The limits must be clear. Bypass surgery does not stop or cure atherosclerosis. The original arteries and the grafts themselves can narrow over the years unless blood pressure, blood sugar and cholesterol are controlled and smoking stops. It is not a treatment for obesity and not a replacement for physical activity and medication. It cannot repair heart muscle already damaged by a previous large heart attack, and it does not remove the possibility of rhythm disturbance. The benefit is preserved by long-term adherence, not by the operation alone.

Recovery has two phases: usually a day or two in intensive care and then several days on the ward, followed by months of graduated cardiac rehabilitation. The breastbone requires adherence to movement restrictions for several weeks. Psychological support is part of care, because mood swings, disturbed sleep and reduced concentration are common after cardiac surgery and improve gradually.

## Preparation

\*\*Do not stop aspirin, any antiplatelet drug or any anticoagulant on your own before surgery; whether and when to stop it is a joint decision of your cardiologist and surgeon.\*\* Give the team a complete list of your medicines, supplements and herbal products, and report diabetes, kidney or lung disease, sleep apnoea, and any drug allergy or previous bleeding problem. Complete blood tests, echocardiography, a chest radiograph and dental assessment if requested, since dental infection raises the risk of post-operative infection. Stop smoking immediately, as every smoke-free day before surgery reduces lung complications. Practise deep breathing, use of an incentive spirometer, and coughing while supporting your chest with a pillow. Fast according to the anaesthetist's instructions, shower with the prescribed antiseptic, and bring your medication list and a companion.

## Aftercare

Protect your breastbone for several weeks as your surgeon directs: do not lift, push or pull heavy loads, do not push up on your arms to rise from a bed or chair, brace your chest with a pillow or your arms when coughing or sneezing, and do not drive until you are cleared. Walk daily, increasing the distance gradually, and weigh yourself every morning on the same scale. Inspect and clean the wound exactly as instructed, and do not apply creams or herbal preparations of your own. Take every prescribed medicine on time and stop none of them yourself, attend cardiac rehabilitation, and keep blood sugar, blood pressure and lipids controlled. Do not return to smoking. Mood swings and disturbed sleep are common and improve. \*\*Go to emergency care immediately for new chest pain or sudden breathlessness, fluid or pus draining from the chest wound or a sensation of movement or clicking in the breastbone, persistent high fever, a fast irregular heartbeat with dizziness or fainting, painful swelling of one leg, rapid weight gain with swollen feet and breathlessness when lying flat, or coughing up blood.\*\*
