## Procedure steps

### 1. Multidisciplinary assessment

BMI calculation and assessment of comorbidities, gastroscopy, comprehensive laboratory tests, and nutritional and psychological evaluation — to determine whether bypass suits the case better than sleeve.

### 2. Pre-operative preparation

A specified pre-operative diet to shrink the liver and ease laparoscopic access, control of diabetes and blood pressure, smoking cessation, and a clot-prevention plan.

### 3. Creating the gastric pouch

Under general anaesthesia through small ports, a small upper portion of the stomach is separated to become the functioning pouch that receives food.

### 4. Intestinal reconstruction

The pouch is joined to a downstream segment of small intestine at a calculated length, and the digestive route is reconstructed so bile and enzymes flow into their new position.

### 5. Lifelong follow-up and supplementation

A graded diet from fluids to solids, permanent vitamin and mineral supplementation, and periodic tests for iron, B12, calcium and vitamin D to detect any deficiency early.

## About this service

Laparoscopic gastric bypass is an obesity operation performed through small incisions, in which the functioning stomach volume is reduced to a small pouch, and that pouch is then connected directly to a downstream segment of small intestine — reducing the volume of food that can be eaten and altering its route so less is absorbed.

What sets it apart from sleeve gastrectomy is that it does not work by volume restriction alone but produces substantial gut-hormone changes. It is therefore among the strongest options for \*\*improving type 2 diabetes\*\*, and is often preferred in people with severe gastro-oesophageal reflux — because sleeve gastrectomy may worsen reflux whereas bypass usually improves it.

In return it carries a permanent commitment that tolerates no slack: the altered absorption route makes deficiency of vitamins and minerals — iron, vitamin B12, calcium and vitamin D — a realistic long-term possibility. \*\*Taking supplements and attending regular blood tests are therefore not optional but part of lifelong treatment\*\*, and neglecting them can lead to anaemia, neurological problems and bone thinning.

It must also be understood that surgery is a tool, not a magic solution: results depend on changing eating patterns and activity, with nutritional and psychological follow-up. Patients are advised to avoid pregnancy in the first months after surgery until weight and nutrition stabilise. ## Preparation
