ومن المهم معرفة حدود هذا الفحص بوضوح: تخطيط طبيعي لا ينفي الصرع، لأن التسجيل يلتقط شريحة زمنية قصيرة قد لا تظهر فيها الاضطرابات الكهربائية؛ ولهذا قد يُطلب تخطيط بعد قلة نوم أو تخطيط مطوّل بالفيديو. وفي المقابل، وجود تغيّرات بسيطة في التخطيط لا يعني بالضرورة صرعاً ولا يُعالَج وحده. والتخطيط لا يقيس الذكاء ولا القدرات العقلية ولا يكشف الشخصية، ولا يُظهر الأورام أو الجلطات أو التصلّب اللويحي — هذه يحدّدها التصوير بالرنين المغناطيسي أو المقطعي — ولا يشخّص الصداع النصفي ولا التوتر ولا الدوار ولا فرط الحركة وتشتّت الانتباه.

ترشيح المريض لنوع التخطيط ومدّته يُحدَّد بالفحص السريري وبقصة الحادثة كما يرويها من شاهدها، لا بقراءة صفحة على الإنترنت. وقيمة التخطيط الحقيقية تظهر حين يُقرأ إلى جانب هذه القصة السريرية، لا كورقة نتيجة معزولة.

## خطوات الإجراء

### 1. التقييم السريري وتحديد نوع التخطيط

يستمع الطبيب إلى وصف الحادثة منك ومن شاهدها، ويسأل عن مدّتها وما قبلها وما بعدها، والأدوية، وقلة النوم، والحرارة، وتاريخ العائلة. على هذا الأساس يُحدَّد نوع التخطيط: روتيني في اليقظة، أو بعد قلة نوم، أو مع نوم، أو مطوّل بالفيديو لعدّة ساعات أو أيام.

### 2. تحضير فروة الرأس وتركيب الأقطاب

يُقاس الرأس وتُعلَّم مواضع الأقطاب بنمط قياسي، ثم يُنظَّف الجلد بمعجون مقشّر لطيف وتُثبَّت الأقطاب بمعجون موصل أو غطاء مرن. لا يخترق شيء الجلد ولا يؤلم التركيب، وقد تشعر ببرودة المعجون أو ضغط خفيف. يستغرق التركيب عادة من عشر إلى عشرين دقيقة.

### 3. التسجيل في حالة الراحة

تستلقي أو تجلس في غرفة هادئة معتمة، مسترخياً وعينيك مغمضتين، والفنّي يراقب الشاشة. يُطلب منك تجنّب الحركة والكلام ومضغ العلك لأنها تُحدث تشويشاً في التسجيل. إن ظهرت حادثة أثناء الفحص فذلك مفيد لا مقلق، والفنّي مدرَّب على التعامل معها.

### 4. مناورات التنشيط

لإبراز الاضطرابات الكهربائية يُطلب فتح العينين وإغماضهما، ثم التنفّس العميق السريع لدقائق قليلة (قد يسبّب تنميلاً أو دوخة عابرة)، ثم التحفيز بضوء متقطّع. وإن كان مطلوباً نوم فقد تُتاح لك فترة نوم قصيرة. هذه المناورات آمنة وتُوقَف فوراً عند أي إزعاج.

### 5. قراءة التخطيط وربطه بالسياق السريري

يُراجع طبيب الأعصاب التسجيل كاملاً بحثاً عن نمط الخلفية والتغيّرات البؤرية أو المعمّمة، ويستبعد ما هو تشويش من العضلات أو العين أو التعرّق. ثم يُصاغ التقرير مقروناً بقصتك السريرية، ويُشرح لك المعنى العملي والخطوة التالية: مراقبة، أو صورة دماغ، أو تخطيط أطول، أو خطة دوائية.

## Procedure steps

### 1. Clinical assessment and choice of EEG type

The physician listens to your description of the event and that of any witness, asking about its duration, what came before and after, medications, sleep loss, fever and family history. On that basis the type of recording is chosen: a routine awake study, a sleep-deprived study, a sleep recording, or prolonged video EEG over hours or days.

### 2. Scalp preparation and electrode placement

The head is measured and electrode positions are marked in a standard pattern. The skin is cleaned with a mild abrasive paste and electrodes are fixed with conductive gel or a flexible cap. Nothing penetrates the skin and placement is not painful, though you may feel the cool gel or light pressure. Setup usually takes ten to twenty minutes.

### 3. Baseline recording at rest

You lie or sit in a quiet, dimly lit room, relaxed with your eyes closed, while the technologist watches the screen. You are asked to avoid moving, talking or chewing gum, since these create artefact on the tracing. If an event happens during the study it is useful rather than alarming, and the technologist is trained to manage it.

### 4. Activation manoeuvres

To bring out abnormal activity you are asked to open and close your eyes, then to breathe deeply and rapidly for a few minutes (which may cause brief tingling or light-headedness), followed by intermittent photic stimulation. If sleep is required, a short nap period may be allowed. These manoeuvres are safe and are stopped immediately if you feel unwell.

### 5. Interpretation in clinical context

The neurologist reviews the entire recording, assessing the background rhythm and any focal or generalised abnormalities, and excluding artefact from muscle, eye movement or sweat. The report is then written alongside your clinical story, and the practical meaning and next step are explained to you: observation, brain imaging, a longer recording, or a medication plan.

## التحضير قبل الإجراء

أخبر الطبيب بكل أدويتك ومكمّلاتك، ولا توقف دواءً مضاداً للنوبات ولا تعدّل جرعته من نفسك قبل الفحص إلا بتعليم صريح من طبيبك. اغسل شعرك في الليلة السابقة بالشامبو وحده، وامتنع عن الزيت والكريم والجل والسبراي وأي مثبّت أو صبغة لأنها تمنع التصاق الأقطاب. تناول طعامك في موعده حتى لا ينخفض سكر الدم، وتجنّب القهوة والشاي ومشروبات الطاقة في يوم الفحص. إن طُلب منك تخطيط بعد قلة نوم فالتزم بعدد ساعات النوم الذي حدّده الطبيب بالضبط، ورتّب شخصاً يقود بك السيارة ذهاباً وعودة. للأطفال: أحضر لعبة مفضّلة أو زجاجة الحليب، واشرح للطفل مسبقاً أن الفحص لا يؤلم. أحضر تقارير فحوصك وصورك السابقة، ووصفاً مكتوباً لما حدث في النوبة من شخص شاهدها، وسجّل مقطع فيديو للحادثة إن أمكن ذلك بأمان.

## العناية بعد الإجراء

بعد الفحص يُزال المعجون بالماء والشامبو، وقد يبقى أثر لزج في الشعر يزول بغسلة أو غسلتين. تعود إلى طعامك وأدويتك ونشاطك المعتاد فوراً، إلا إن كان الفحص بعد قلة نوم فتجنّب قيادة السيارة والأعمال الخطرة حتى تنام جيداً. لا يترك التخطيط أي إشعاع ولا آثاراً جانبية. النتيجة تُقرأ عادة خلال أيام قليلة، فحدّد موعد المراجعة ولا تفسّر التقرير بنفسك. \*\*راجع الطوارئ فوراً إذا استمرّت النوبة أكثر من خمس دقائق، أو تكرّرت نوبتان أو أكثر بلا استعادة كاملة للوعي بينهما، أو ظهر ضيق نفس أو ازرقاق، أو حدثت إصابة في الرأس أو الرقبة أثناء النوبة، أو كانت هذه أول نوبة في حياتك، أو صحبتها حرارة وتشوّش أو صداع شديد مفاجئ أو ضعف في وجه أو طرف، أو كنتِ حاملاً.\*\* وأثناء أي نوبة: أَمِل المريض على جانبه، وأزِل ما حوله من أشياء صلبة، وضع شيئاً طرياً تحت رأسه، وسجّل وقت البداية — و\*\*لا تضع أبداً شيئاً في فمه ولا تحاول تثبيت أطرافه بالقوة، فهذه خرافة تسبّب كسر أسنان وإصابات في الفم والكتف\*\*.

## About this service

An electroencephalogram (EEG) is a test that records the electrical activity of brain cells through small electrodes attached to the scalp, converting that activity into waveforms a neurologist interprets. It is non-invasive, sends no electrical current into the head, involves no radiation, and can be safely repeated in children, adults and during pregnancy.

The main reason an EEG is ordered is to evaluate seizures: whether the event was an epileptic seizure or something that mimics one such as fainting, a panic attack, a sleep-related movement or a non-epileptic psychogenic event; what type of seizure it was; and which part of the brain it appears to arise from. It is also used to assess unexplained confusion or altered consciousness, to follow a patient with epilepsy whose seizure pattern has changed or who is being considered for withdrawal of medication, to assess selected children with developmental delay or loss of acquired skills, to monitor critically ill patients, and to help evaluate certain sleep disorders alongside other studies.

The limits of the test matter just as much as its uses. A normal EEG does not rule out epilepsy, because the recording captures only a short window of time in which abnormal electrical activity may simply not appear; this is why a sleep-deprived or prolonged video EEG is sometimes requested. Conversely, minor variations on a tracing do not automatically mean epilepsy and are not treated in isolation. An EEG does not measure intelligence or personality, and it does not show tumours, strokes or multiple sclerosis, which require MRI or CT imaging instead. It does not diagnose migraine, anxiety, vertigo, or attention deficit hyperactivity disorder.

Which type of EEG is appropriate, and for how long, is decided by clinical examination and by a careful account of the event from someone who witnessed it, not by reading a web page. The test delivers its real value when it is interpreted together with that clinical story rather than as a stand-alone result sheet.

## Preparation

Tell the physician about every medication and supplement you take, and do not stop or change the dose of any anti-seizure medicine on your own before the test unless your doctor explicitly instructs you to. Wash your hair the night before with shampoo only, and avoid oil, cream, gel, spray, hair fixative and dye, since these prevent the electrodes from adhering. Eat your meals as usual so your blood sugar does not drop, and avoid coffee, tea and energy drinks on the day of the test. If a sleep-deprived recording has been requested, follow exactly the number of sleep hours your doctor specified, and arrange for someone to drive you both ways. For children, bring a favourite toy or bottle and explain in advance that the test does not hurt. Bring previous reports and imaging, a written account of the event from someone who witnessed it, and a video clip of the episode if it can be recorded safely.

## Aftercare

After the test the gel is removed with water and shampoo; a sticky residue may remain and clears after one or two washes. You may return to your usual food, medication and activity immediately, except after a sleep-deprived study, when you should avoid driving and hazardous work until you have slept properly. The recording leaves no radiation and has no side effects. Results are usually reported within a few days, so book your follow-up visit and do not interpret the report yourself. \*\*Go to the emergency department immediately if a seizure lasts more than five minutes, if two or more seizures occur without full recovery of consciousness in between, if there is breathing difficulty or blue discolouration, if the head or neck was injured during the seizure, if this was your first ever seizure, or if it came with fever and confusion, a sudden severe headache, weakness of the face or a limb, or if you are pregnant.\*\* During any seizure, turn the person on their side, clear hard objects away, cushion the head and note the start time — and \*\*never put anything in the mouth or forcibly restrain the limbs, a widespread myth that causes broken teeth and injuries to the mouth and shoulder\*\*.
