# تركيب الأنبوب الغذائي (التغذية المعوية) — Tube Feeding (Enteral Nutrition) > التغذية المعوية هي إيصال تغذية سائلة متكاملة إلى المعدة أو الأمعاء عبر أنبوب، حين يكون البلع غير آمن أو حين لا يكفي الأكل بالفم لتغطية احتياج المريض. تُستخدم مثلاً بعد الجلطة الدماغية، وفي أمراض الأعصاب التقدّمية، وفي أورام الرأس والرقبة والبلعوم، وعند مرضى العناية الحرجة، وفي حالات سوء التغذية الشديد أو الفشل في النموّ عند الأطفال. والأنبوب طريق للتغذية لا علاج للمرض الأساسي، وقرار تركيبه قرار فر… ## معلومات الخدمة - **الاسم (عربي):** تركيب الأنبوب الغذائي (التغذية المعوية) - **الاسم (إنجليزي):** Tube Feeding (Enteral Nutrition) - **التخصص المرتبط:** تغذية علاجية وحميات - **مدة الإجراء:** تركيب الأنبوب الأنفي عادة دقائق قليلة، وعمل فتحة التغذية عبر المنظار عادة من 20 إلى 45 دقيقة مع مراقبة بعدها ساعات، وقد تستلزم البقاء ليلة. تبدأ التغذية عادة في نفس اليوم أو التالي بحسب الطريقة والتعليمات، ويُبدَّل الأنبوب دورياً بحسب نوعه وتوجيه الفريق. - **آخر تحديث:** 2026-07-26 - **البلد:** الأردن - **رابط الصفحة:** https://clinicsjo.com/ar/service/تركيب-الانبوب-الغذايي-التغذية-المعوية ## عن الخدمة التغذية المعوية هي إيصال تغذية سائلة متكاملة إلى المعدة أو الأمعاء عبر أنبوب، حين يكون البلع غير آمن أو حين لا يكفي الأكل بالفم لتغطية احتياج المريض. تُستخدم مثلاً بعد الجلطة الدماغية، وفي أمراض الأعصاب التقدّمية، وفي أورام الرأس والرقبة والبلعوم، وعند مرضى العناية الحرجة، وفي حالات سوء التغذية الشديد أو الفشل في النموّ عند الأطفال. والأنبوب طريق للتغذية لا علاج للمرض الأساسي، وقرار تركيبه قرار فريق: الطبيب المعالج وأخصائي التغذية وأخصائي البلع، ومعهم المريض وعائلته. الطريق يُختار حسب المدة المتوقعة وسلامة المعدة: أنبوب يمرّ من الأنف إلى المعدة يصلح للحاجة القصيرة، وفتحة تغذية تُعمل مباشرة إلى المعدة عبر المنظار أو بالجراحة عندما تطول الحاجة أسابيع وشهوراً، وأنبوب يتجاوز المعدة إلى الأمعاء عند من يعاني ارتجاعاً شديداً أو تفريغاً بطيئاً. يجري التركيب بيد فريق مؤهّل وبتخديرٍ موضعي أو مهدّئ حسب الطريقة، ويُتحقّق من موضع الأنبوب قبل بدء التغذية. حدود الإجراء يجب أن تُقال بصراحة: **الأنبوب لا يمنع الاستنشاق الرئوي منعاً تاماً**، ولا يُغني عن تأهيل البلع، ولا يعني بالضرورة توقف الأكل بالفم للأبد — بعض المرضى يعودون إلى الفم كلياً أو جزئياً بعد تحسّن البلع. وهو أيضاً ليس مجرد قرار تقني: في الأمراض المتقدّمة يُوزن نفعه وأثره على راحة المريض بالحوار معه ومع أهله وفريق العلاج. التركيبات الغذائية الطبية تُوصف بالحساب على وزن المريض ووظائف كليتيه وكبده وسكره، ولا تُشترى بالتخمين ولا تُستبدل بخلطات منزلية مطحونة تختلف كثافتها وتسدّ الأنبوب وقد تتلوّث. وعند المريض الهزيل جداً أو الذي انقطع أكله أياماً تُبدأ التغذية ببطء شديد مع مراقبة أملاح الدم، لتفادي ما يُعرف بمتلازمة إعادة التغذية وهي حالة خطيرة قد تسبّب اضطراب نظم القلب. واحذر مكمّلات «حرق الدهون» أو «فتح الشهية» مجهولة المصدر وحقن التنحيف المُروَّج لها بلا تشخيص، فلا موضع لها هنا إطلاقاً. ولا تُدار التغذية المعوية بحميات الحذف القاسية بلا إشراف، ولا بأنظمة منخفضة النشويات مثل «الكيتو» أو بالصيام الممتدّ — وهذه عند مريض السكري أو الكلى قرار طبي لا اجتهاد أهل. الترشيح للأنبوب ونوعه ومدّته والكمية اليومية تُحدَّد بالفحص السريري وتقييم البلع والتحاليل، لا بالقراءة. وبعد التركيب تستمر المتابعة مع أخصائي التغذية لتعديل الكمية مع تغيّر الوزن والحالة، ومع الطبيب لمعالجة أي مشكلة في الفتحة أو التحمّل. ## خطوات الإجراء [object Object],[object Object],[object Object],[object Object],[object Object] ## التحضير قبل الإجراء أخبر الفريق الطبي بكل أدويتك، وخصوصاً مسيّلات الدم ومضادات التخثّر وأدوية السكري، ولا توقف أيّها من نفسك بل اسأل عن التعليمات المكتوبة. اذكر أي حساسية دواء أو لاصق أو مادة تعقيم، وأمراض القلب والكلى والكبد، والحمل إن وُجد. التزم بساعات الصيام التي يحدّدها الفريق قبل التركيب. أحضِر تقاريرك وتحاليلك وقائمة أدويتك، ورتّب مرافقاً يعيدك إلى البيت إن استُخدم مهدّئ، فلا تقُد سيارة في ذلك اليوم. إن كنت تُعتني بمريض في البيت فاسأل مسبقاً عن مصدر تركيبة التغذية والحقنة والمستلزمات، واطلب تدريباً عملياً على التحضير النظيف والشطف قبل الخروج، ودوِّن رقم الاتصال بالفريق عند وجود مشكلة. ## العناية بعد الإجراء أعطِ التغذية والمريض في وضع نصف جالس وأبقِه كذلك بعدها كما عُلِّمت، واشطف الأنبوب بالماء قبل كل دواء وبعده، ولا تدفع بقوة إن أحسست بمقاومة. نظّف الجلد حول الفتحة يومياً وجفّفه، وراقب الوزن وكمية البول والبراز. لا تستخدم خلطات منزلية أو أعشاباً في الأنبوب ولا تغيّر التركيبة أو الكمية من نفسك. **راجع فوراً عند: سعال أو ضيق نفس أو ازرقاق أثناء التغذية أو بعدها — احتمال استنشاق** · **قيء متكرر أو ألم بطن شديد أو انتفاخ وتوقف خروج الريح** · **حمّى أو احمرار وسخونة أو خروج قيح أو تسرّب حول الفتحة أو نزف** · انزلاق الأنبوب أو خروجه أو انسداده · إسهال شديد أو تجفاف · **دوخة أو إغماء** · **نزول وزن غير مقصود، فهو علامة مرض لا نجاح** · تنميل أو ضعف أو خفقان في أول أيام التغذية. وإن كان رفض الأكل بالفم مرتبطاً **بهوس بالوزن أو تقيّؤ متعمّد أو استخدام مسهّلات، فهذا اضطراب أكل يحتاج تدخّلاً نفسياً وطبياً لا تقييداً أشدّ للطعام**. ## About this service Enteral nutrition means delivering a complete liquid feed into the stomach or intestine through a tube, when swallowing is unsafe or when oral intake cannot cover what the patient needs. It is used after stroke, in progressive neurological disease, in head and neck or oesophageal cancer, in critical care, and in severe malnutrition or faltering growth in children. The tube is a route for nutrition, not a treatment for the underlying disease, and the decision to place one is a team decision involving the treating physician, the dietitian and the swallowing therapist, together with the patient and family. The route depends on how long feeding is expected to be needed and on whether the stomach works. A tube passed through the nose into the stomach suits short-term needs; a feeding stoma created directly into the stomach, endoscopically or surgically, is preferred when the need will last weeks or months; and a tube that reaches beyond the stomach into the small bowel is used in severe reflux or delayed gastric emptying. Placement is carried out by a trained team under local anaesthesia or sedation depending on the technique, and tube position is confirmed before any feed is given. The limits must be stated honestly. **A feeding tube does not completely prevent aspiration into the lungs**, it does not replace swallowing rehabilitation, and it does not necessarily mean the end of eating by mouth: some patients return to oral intake fully or partly once swallowing improves. Nor is it purely a technical decision. In advanced illness, the benefit and the burden are weighed in conversation with the patient, the family and the treating team. Medical feeding formulas are prescribed by calculation against body weight, kidney and liver function and glucose control. They are not bought by guesswork and must not be replaced with blended home mixtures, whose thickness varies, which block tubes and which can be contaminated. In a very wasted patient, or one who has eaten nothing for days, feeding is started very slowly with blood electrolytes monitored, to avoid refeeding syndrome, a dangerous condition that can disturb the heart rhythm. Unregulated fat-burner or appetite-stimulating supplements and marketed slimming injections have no place here at all. Nor is enteral feeding run through severe elimination diets without supervision, or through very low-carbohydrate patterns such as keto or prolonged fasting, which in diabetes or kidney disease are a medical decision rather than a family judgement. Eligibility for a tube, the route chosen, how long it stays and the daily volume are all decided by clinical assessment, swallowing evaluation and laboratory results, never by reading a page. After placement, follow-up continues with the dietitian to adjust volumes as weight and condition change, and with the physician for any problem at the stoma or with tolerance. ## Preparation Tell the medical team about every medicine you take, especially blood thinners, anticoagulants and diabetes medicines, and do not stop any of them yourself; ask for written instructions instead. Mention allergies to medicines, dressings or antiseptics, and any heart, kidney or liver disease, and pregnancy if relevant. Follow exactly the fasting hours the team specifies before placement. Bring your reports, laboratory results and medication list, and arrange someone to take you home if sedation is used, since you must not drive that day. If you will be caring for a patient at home, ask in advance where the feeding formula, syringes and supplies come from, request hands-on training in clean preparation and flushing before discharge, and write down the number to call when a problem arises. ## Aftercare Give feeds with the patient sitting up and keep them upright afterwards as you were taught, flush the tube with water before and after every medicine, and never push against resistance. Clean and dry the skin around the stoma daily, and monitor weight, urine output and bowel habit. Do not put home-blended mixtures or herbal preparations down the tube, and do not change the formula or volume on your own. **Seek medical help immediately for: coughing, breathlessness or a bluish colour during or after feeding, which suggests aspiration** · **repeated vomiting, severe abdominal pain, or distension with no passage of wind** · **fever, redness and warmth, pus or leakage around the stoma, or bleeding** · a tube that has slipped, come out or become blocked · severe diarrhoea or dehydration · **dizziness or fainting** · **unintentional weight loss, which signals illness rather than success** · numbness, weakness or palpitations in the first days of feeding.
