## Procedure steps

### 1. Assessing need and cause

The doctor confirms there is a genuine indication, examines the lower abdomen to gauge bladder fullness, uses ultrasound to measure residual urine, and identifies the likely cause of the retention or obstruction.

### 2. Choosing type and size

The type, size and material are chosen according to the expected duration, the state of the urethra and any history of previous surgery or stricture, with intermittent or suprapubic catheterisation preferred in specific situations the doctor determines.

### 3. Antisepsis and local anaesthesia

The area is carefully disinfected and a sterile field applied, and an anaesthetic lubricant is instilled into the urethra to reduce pain and spasm. Correct antisepsis at this stage is the single most important step in preventing later infection.

### 4. Insertion and securing

The catheter is advanced gently until urine flows, which confirms it has reached the bladder, then the balloon is inflated with the specified volume only. It is never inflated before flow is confirmed, and the tube is never forced against resistance.

### 5. Drainage, instructions and plan

The drainage bag is connected and kept below bladder level, emptying and daily hygiene are explained, an expected date for removal or change is set, and a plan is made to treat the underlying cause of the retention.

## About this service

A urinary catheter is a flexible tube placed to drain urine from the bladder when normal voiding is not possible or when urine output must be measured accurately. It is inserted either through the urethra, which is the usual route, or through the abdominal wall below the navel as a suprapubic catheter when the urethral route is blocked or unsafe. In practice there are three patterns: an intermittent catheter inserted and removed in the same session, an indwelling catheter held in place by a small balloon, and a suprapubic catheter placed through a minor image guided procedure.

Its best known indication is \*\*acute urinary retention\*\*: a sudden inability to pass urine with severe pain and lower abdominal distension. This is an emergency that needs prompt drainage rather than waiting. In men past middle age the commonest cause is benign prostatic enlargement, and here a clarification reassures many people: \*\*benign prostatic enlargement is not cancer and does not turn into cancer\*\*; it is a common age related change. Equally, \*\*urinary symptoms alone — a weak stream, hesitancy, frequent night time voiding, a sense of incomplete emptying — do not mean cancer\*\*, and early prostate cancer usually causes no urinary symptoms at all. The two are therefore not separated by symptoms or by reading, but by the examination and assessment a doctor decides upon.

Other indications include monitoring urine output in critically ill patients, certain operations, injuries, inability to empty the bladder for neurological reasons, and drainage of an obstruction while its cause is treated.

The limits are clear: \*\*a catheter drains urine, it does not treat the cause\*\* of the retention, and it is not a treatment for incontinence nor a convenience for care, because using it without indication does harm. Every additional day with a catheter raises the risk of catheter associated urinary infection, so the rule is to keep it for the shortest possible time. On the wider subject of men health, \*\*erectile difficulty can be the first sign of undiagnosed arterial disease or diabetes\*\*, so mentioning it to a doctor is not something to feel awkward about; it is also a check on your heart, and it deserves medical assessment rather than being ignored or self treated.

## Preparation

If you suddenly cannot pass urine and have pain with lower abdominal distension, do not wait for an appointment and go to the emergency department, because acute retention is not managed at home. Before a planned insertion, tell the doctor about any previous urethral or prostate surgery, a known urethral stricture, previous difficulty with catheter insertion, or recurrent urinary infections, since these change the catheter type and technique. Mention your medications, especially blood thinners, and any allergy to latex, antiseptics or local anaesthetic. Do not drink large volumes beforehand without instruction, and empty as much urine as you can if you are able. Wear loose clothing, bring a companion if you are elderly or have limited mobility, and ask how to care for the bag at home before leaving the clinic.

## Aftercare

Wash your hands before and after touching the catheter, clean the exit site with soap and water once daily and after opening your bowels, and dry it gently. Always keep the drainage bag below bladder level, empty it before it fills, never let the tube kink or be pulled, and secure it to the thigh. Drink enough fluid unless your doctor restricts it, and never flush the catheter, pull it out, or inflate or deflate its balloon at home. Seek care immediately for \*\*fever, chills or flank pain, which may indicate a kidney infection\*\*, \*\*urine drainage stopping with pain and lower abdominal distension\*\*, \*\*heavy red bleeding or clots blocking the tube\*\*, \*\*the catheter coming out or slipping\*\*, \*\*pain or swelling in the testicle or scrotum\*\*, \*\*very cloudy foul smelling urine with confusion or lethargy in an older person\*\*, \*\*severe persistent urethral pain\*\*, or \*\*continuous leakage of urine around the tube\*\*. Do not postpone the appointment for catheter removal or change.
