## Mental health assessment and care planning

A mental health assessment is a structured conversation about experiences and support needs, not a pass-or-fail examination. It considers mood, thoughts, behaviour, physical health and daily circumstances.

Explain your concerns and goals and ask about involving a supportive person when appropriate. This information helps develop a shared treatment, support and follow-up plan.

Diagnosis may take more than one appointment. Ask about confidentiality and its limits. Immediate danger to yourself or others requires emergency help.
